    Summary: Oculo-cardiac reflex during polygraphic registration (EEG, ECG)was performed in 24 patients with past history of syncopalattacks. A high sensitivity in the investigation of vagusexcitability was found. 22 patients showed bradicardia of morethan 30% of the slowing of the heart rate. The most intensivebradicardia with asystolia which lasted 3 - 10 seconds wasregistered within the first 15 seconds from the benining of thetest demonstrating the vagus hyperexcitability in the examinedchildren.

    Summary: Aim: 32 children with trisomy 21, Down's syndrome regular type,were detected in the municipality of rijeka and in five Istrianmunicipalities (Pula, Poreč, Labin, Rovinj, Pazin) from 1986 to1990. During the same period, no cases of Down's syndrome weredetected prenatally. In view of that, the aim of this work was tofind out whether this fact was due to the negligence of theexisting indications for prenatal diagnostics, to the parents'refusal to undergo testing, or to something else. Methods: Chromosome studies in all recorded cases of Down'ssyndrome were carried out in cultures of peripheral bloodlymphocytes following the usual procedures, and G-bandingtechnique was performed. With the view to obtaining various data,the authors carried out a survey among the parents of Down'ssyndrome children. Out of the 32 cases, 29 couples-parents(90.6%) agreed to participate in the survey. Results: The incidence of trisomy 21 was 1.48/1,000 newborns(minimal mutation rate 7.4 x 10 -4) in the municipality of Rijekaand in Istrian municipalities. The mean age of their mothers was28.4 +/- 7.39, and the mean paternal age was 32.14 +/- 8.14. Conclusions: The most important indication - the mother's age-was obviously neglected in 15.6% cases, but because of the highincidence of trisomy 21 in the group of women younger than 35, itwould be necessary to introduce prenatal screening tests (i.e.,measurement of maternal serum alpha-fetoprotein, and humanchorionic gonadotropin) for all pregrancies in view of applyingaimed prenatal diagnostic methods.

    Summary: During the past twenty years a considerable progress in thecomprehension of the clinical, neurophysiological anddevelopmental aspects of the particular forms of epilepsy inchildren was made. This brough about the attempts to definebetter and more rigorously the particular epileptic syndromes inthe childhood, especially regarding the long-term outcome andprognosis ("bening" and "serious" epilepsies, i.e. "functional"and "lesional" forms). With no hesitation the age of the patientcan be considered as the factor of a primary importance in theknowledge of the clinical picture and the type of the seizure,which has an important influence on the development and prognosisof the disease.

    Summary: The object of this study was to investigate clinical andepidemiologic characteristics of head trauma in childrenhospitalized in the Clinical Hospital Center Rijeka in the period1987 - 1990. In a retrospective review we separated 70 childrenwho satisfied our criteria for head trauma - hospitalizationbecause of clinical diagnoses of coma, contusion, commotion,skull fracture, intracranial hemorrhage or combination of these.From the total number of children who suffered head trauma (70),44 were boys and 28 were girls, the mean average age being 6.3years (+/- 3.8). Traffic accidents had been the cause of headtrauma in 38 children (28 as pedestrians and 10 as occupants ofvehicles), and falls or impacts to the head in 32 children. Therewere no differences in the incidence of head trauma according toage, but the most common cause of head trauma in the age range 0- 4 was falls or impacts to the head (16 children), in ages 5 - 9traffic accidents as pedestrians (14 children). In the ages 10 -14 there was an equal distribution of causes for head trauma. Themost common clinical diagnosis was contusion, with or withoutskull fracture - 40 children, then intracranial hemorrhage(extradural hematoma, subdural hematoma, their combination andintracerebral hemorrhage) - 12 children. The mortality was 7.1%(5 children). We discussed the influence of head trauma on thegeneral mortality in childhood and it was noted that there is noorganized system or multidisciplinary approach to childhoodtrauma. We emphasized the extreme need and importance oforganizing a pediatric trauma register.

    Summary: The term "benign epilepsy" refers to those reccurent convulsive disorders whose natural course is predictable toward definitive remission in at least 90-95% of cases before reaching adulthood, without leaving any residua.During the past decade, a number of additional syndroms of benign epilepsy have been reported occuring in neurologically and mentally intact children. These include benign epilepsy with centrotemporal spikes (BECTS), benign psychomotor epilepsy (BPE) or benign epilepsy with affective symptomatology, benign partial epilepsy with occipital paroxysms (BEOP) and benign partial epilepsy with extreme somatosensory evoked potentials. As a rule, the seizures are brief and infrequent, usually responding well to antiepilepgtic drugs and remitting before adulthood.

    Summary: The benign epilepsies of childhood are, according to neurophysiological criteria, partial functional epilepsies of childhood (i.e. cebntrotemporal epilepsy, benign psychomotor epilepsy, benign epilepsy with occipital paroxysms, and benogn epilepsy with extreme somatosensory evoked potentials). Their prognosis is predictable toward definitive remission in 90-95% before reaching adulthood, without any residua. The treatment of choice is carbamasepine.

    Summary: In an attempt to determine wheather the clinical data obtained by primary survey may be used as early outcome predictors in children who had sustained head trauma, children aged 0-14 with clinical diagnoses of coma, contusion, comotio, skull fracture, 43 children hospitalized between 1987 and 1990 were reviewed retrospectively. The analysis demonstrated that the early significant predictors are the pupillary appearance and reactivity as well as Glasgow coma scale. Motor response graded by Glasgow coma scale did not achieve statistical significance ( z=1.5, P>0.05) as a predictor of outcome. Our results showed that the significant predictors in descending order of preference are: pupillary reactivity, pupillary appearance and GCS. Unfortunately oculocephalic and oculovestibular reflexes in the period of research these tests were not used as routine ones during a primary survey.

    Summary: The knowledge of basic mechanisms in the generation of convulsive disorders plays a very important role in understanding the basic principles of classification, diagnostics and treatment of convulsions. Although all the mechanisms of the convulsive disorders have not yet been clarified, all of them have to be observed through the processes of neuronal excitation and inhibition, which are the basic principles of the function of the central nervous system. The primary disturbance is probably due to the modulation of voltage-dependent ion channels involved in action potential propagation and extra- and intracellular ion concentration regulation, which is followed by disregulation in excitatory and inhibitory neurotransmission. All clinical manifestations of epileptic brain activity should be observed through the frame of the functiona brain anatomy. In the last two decades great efforts have been made to better understanding the neurophysiological, clinical and developmental aspects of epilepsies, which altogether have a major effects on the course and prognosis of epilepsy in children.

    Summary: Kainic acid (KA) is an excitatory glutaminergic agonist that induces epileptic seizures. The purpose of this study was to record EEG activity and the behavioral pattern changes after an intrahippocampal KA administration. Experiments were performed on adult Wistar rats. The animals were anesthezied and placed in a stereotaxic apparatus. Surface screw electrodes were placed over the frontal and parietal cortex. KA was administrated into the left dorsal hippocampus. The EEG activity was recordered in a basal conditions and 5, 10, 15, 30 and 60 minutes after the KA aplication. During this period complex behavioral pattern was also observed. EEG samples were digitized and subjected to computerized frequency analysis. The KA-induced excitatory changes in EEG activity were followed by complex behavioral syndrome that lasted for several hours. It has been concluded that intrahippocampal injection of kainate may be a useful experimental model for investigations of partial seizures with complex simptomatology (temporal lobe seizure disorders).

    Summary: Although existing anti epileptic drug (AED) are generallyconsidered to be efficious, many patients never achieve adequateseizure control. Uncontrolled seizures increase the risk offurther seizures in the future and may even lead to permanentbrain damage. Lamictal is a chemically novel treatment forepilepsy. It was discovered in 1978. An extensive clinicaltrials programme in patients with the treatment of resistentepilepsy demonstrated a reduction in generalized tonic-clonic andpartial seizures not satisfactorily controlled with other AEDs.Testing the efficacy of lamotrigine (LTG) in pediatric patientswith refractory different seizures types, we observed the reduceseizures despite the short duration of therapy in all fivetesting children.

    Summary: The group of 32 children aged 1 - 15 years with hypernasal speechwas examined clinically and the electromyography of soft palatewas done. In the first group, the patients with hypernasalityafter tonsillectomy were analyzed showing the neurogenic lesionin M.levator veli palatini which was attributed to the operativetrauma. In the second group of patients with congenitalvelopharyngeal insufficiency, electromyography showed themedially located areas without action potentials and normalactivity in other parts of soft palate which establish thediagnosis of submucous cleft palate. Operative treatment i.e.reconstruction of palatal musculature was done.
