Summary: Histology of skin lesions and demonstration in them of
Rickettsiaconorii by direct immunofluorescence test (DIF) are presented
inl3 patients with Mediterranean spotted fever (MSF). Thelymphohistiocytic
vasculitis which dominated the picture is notspecific, however, it could be
suggestive for the diagnosis ofrickettsiosis. By DIF we demonstrated
rickettsial coccobacillaryforms in all the patients:in l2 macular lesions
and in one "tachenoire". The diagnosis was also confirmed by
indirectimmunofluorescence test in each case. DIF test was shown to
besensitive, specific and reliable in early diagnosis of MSF.

Western immunoblotting demonstrated antibodies tothe 120-kDa surface
protein in all 20 positive serum samplesexamined and to rickettsial

Summary: During summer Mediterranean spotted fever (MSF) is
prevalent inthe subcostal part of Croatia (North Dalmatia) as well as
inother areas of the Mediterranean coast. We compared thespecifitiy and
sensitivity of complement fixation (CF), latexagglutination
(Latex-R.conorii), microimmunofluorescence(micro-IF) and enzyme-linked
immuno assay (ELISA) for detectionabd measuring antibodies against
R.conorii in 78 sera from 46patients with/or suspected Mediterranean
spotted fever. Theseroreactivity with SFG antigens containing the
Rickettsiaconorii-antigen(s) as a common determinant, was positive in
allfour serological tests suggesting that Rickettsia conorii was
theprobable agent of infection in our patients.

Summary: Aims. To analyze the epidemiological, ecological and
clinicalfeatures of Mediterranean spotted fever (MSF) in North
Dalmatia,Croatia.
Methods. For diagnostic and seroepidemiological purposes
themicroimmunofluorescence assay was used. Ticks collected werescreened for
rickettsia-like microorganisms by the hemolymph test(HT).
Results. The overall sero-prevalence of antibodies to Rickettsiaconorii in
our study was 40% with almost equal distribution inall age groups. Nine
different tick species were determined withthe most common Rhipicephalus
sanguineus (65.3%). As much as70.2% of the collected ticks were HT
positive. The clinicalspectrum of MSF in 50 patients was characterized by a
great number of atypical exanthems. A significant number ofasymptomatic
infections (35%) were detected by sero-conversiondiagnostic titers in 48
individuals with a history of recent tickbite.
Conclusions. Our results suggest the possibility of an antigenicvariant of
R.conorii that differs from those circulating in otherparts of the
Mediterranean basin.
