Title: The role of sympathetic dysfunction in blood pressure regulation in uremic patients treated by continuous ambulatory peritoneal dialysis.

Summary: The role of autonomic nervous system dysfunction in theregulation of arterial blood pressure in 4 male and 10 femaleuremic patients, age 35-65 years (57+-9 years), treated bycontinous ambulatory peritoneal dialysis (CAPD) for 6-40 months(20+-12 months), was studied. The integrity of the entireautonomic reflex arc was checked using Valsalva 's test and theorthostasis challenge test. In parallel, efferent sympatheticfunction was tested with the handgrip test. A significantly decreased Valsalva ratio was found in 7 patients(50%), and it was normal in only 2 patients (14%). The handgriptest was subnormal in 5 patients (36%) and abnormal in 7 patients(50%). On the contrary,the orthostatic challenge test showednormal compensatory reaction in 10 patients (72%). The evaluationof three autonomic tests showed combined (afferent+efferent)lesion in 6 patients (43%), predominant afferent lesion in 3patients (21%) and in three patients predominant efferent lesion.In 2 patients (14%) there was no end organ response. Despite significant autonomic nervous system dysfunction inuremics on CAPD, as demonstrated by these results, blood pressure regulation in the supine and upright position was adequate inmost patients (72%), indicating a relatively greater role ofother blood pressure-regulating factors..

Summary: A 49-years old patient treated by hemodialysis for 14 years wasdescribed.Because of bone pain, histological finding of mixeduremic bone disease and markedly elevated parathormone thepatient was subjected to subtotal parathyroidectomy. Four yearslater he developped multiple metastyatic calcifications near leftelbow and clavicula. His parathormone was in normal range , as well as calcium, whereas phosphate was elevated and osteocalcinmarkedly decreased in comparison with pre-treatment values. It isconcluded that a mild parathyreoid hypersecretion is necessaryto maintain bone metabolism in uremia. Due to the slow rate ofbone metabolism, calcium and phosphate, instead of beingincorporated in bone, were deposited in the soft tissues.

Summary: An 18-year-old man suffernig from uremia was hospitalized becauseof severe skeletal pain and multiple exostoses. The patient hadbeen treated for 5 years by regular hemodialysis; primaryoxalosis was the suspected cause of uremia. Transiliac bonebiopsy and subsequent histomorphometric analysis of undecalcifiedbone specimen as well as specific aluminum staining revealed apicture of oxalosis combined with osteomalacia and a markedaccumulation of aluminum at the front of mineralization. Thiscombination of histological features of oxalosis and aluminumbone disease has not been described yet and suggests a new typeof mixed renal osteodystrophy.

Title: Ticlopidine improves dialysis clearance of solutes in uremic patients by reducing blood clotting in dialyser fibers

Summary: Ticlopidine, a platelet aggregation inhibitor was tested ,in adouble blind comparative cross-over study versus placebo, in 51dialysed uremic patients who had increased dialyser bloodclotting (>25 fibers clotted/dialyser). At the end of a 7-daytreatment period with 250 mg daily, the clearance of urea,creatinine and phosphate was determined at 30 and 210 minutes ofdialysis, as well as the number of fibers clotted at the end ofdialysis. Ticlopidine improved dialysers clearances for urea,creatinine and phosphate from 165+-41 to 182+-35 (p<0.01),135+-37 to 143+-35 (p<0.05), and 120+-36 to 130+-35 (p<0.05)ml/min, respectivelly, at 30 min of hemodialysis and a similareffect was seen after 210 minutes of hemodialysis. The number ofdialyser fibers clotted after dialysis was reduced by ticlopidinetherapy from 110+-48 to 15+-8 (p<0.01). Ticlopidine reduced theinitial dialysis-induced drop in leucocyte count by 20% (p<0.05);no change in platelet or erythrocyte count was observed. Two outof 51 patients experienced an adverse reaction from ticlopidine(cutaneous haematoma and minor gingival bleeding). We concludethat ticlopidine is an efficient and safe drug for dialyseduremic patients since it can reduce blood clotting and therbyincrease dialysis efficiency.

Summary: To estimate the prevalence of antibodies to the hepatitis C and B virus (HCV and HBV), 101 patients receiving renal replacement therapy and 75 staff members were tested includins detailed history, HBV-markers and anti-HCV antibody screening. 38% of pts were anti_HCV positive and 40% of these had anti HBs antibodies. Markers indicating only HBV infection had another 18% of pts. All staff members were anti-HBC negative although 45% were anti-HBc positive. Age, sex and blood transfusion history had no influence on prevalence, but polytransfused had a significant increase in prevalence over other pts (p<0.05). During 2-year follow-up a decline in prevalence of HB markers was observed in contrary to a steady increase in anti-HBC seropositivity from 30 to 38%.

Summary: A CAPD patient with secondary hyperparathyroidism was treated successfully with oral calcitriol once weekly. The dose was 4 to 8 mcg weekly. After 3 months of treatment the serum level of parathormone was reduced by 75% i.e. from 39.2 to 9.8 pmol/L. The serum levels of calcium and phosphate were kept in the normal range with the use of oral calcium carbonate 1.5 g daily. There was no side effects of this treatment.

Summary: A total of 143 patients treated by chronic dialysis were included into the study on prevalence of carpal tunnel syndrome. 123 patients were treated by hemodialysis and 16 by peritoneal dialysis. Clinical signs of carpal tunnel were found in 23 patients , 19 treated by hemodialysis and 2 treated by peritoneal dialysis (16%), the syndrome was not found in patients receiving dialysis for less than 5 years.The incidence of syndrome increased with the dialysis duration. In 16 patients (70%) the syndrome was found bilaterally. The incidence of carpal tunnel syndrome in dialysed uremics is much higher than in the general population i.e. 16% vs. 0.1%.

Summary: In our dialysis center , 28 of 148 patinets (19%) have clinical signs of carpal tunnel syndrome. None of patinet undergoing dialysis for less than 5 years have any signs of carpal tunnel syndrome, but 22 of 32 patients (69%) of those who have been treated for more than 10 years by dialysis have developped the syndrome. Seven patients who underwent surgical decompression had clinical and electrophysiological improvement, but four had a relapse after three years.

Summary: A randomized comparative study was conducted to evaluate theclinical efficacy of cefpiramide in comparison with a standardmonotherapy with ceftazidime in uremic patients with severebacteriologically confirmed infections at different sites causedby pathogens susceptible to both antibiotics. Thirty-two patients were randomly alocated to two groupsreceiving either 2.0 gram cefpiramide or 1.0 gram ceftazidime foran average of ten days. Patients were followed after therapyclinically and bacteriologically for another 14 days. In addition to renal failure, the patients(cefpiramide/ceftazidime) had significant concurrent diseasesincluding diabetes mellitus (3/4), anatomical abnormalities ofthe urinary tract (2/4), and cardiac decompensation (2/5). Mostof the patients were treated by either hemodialysis (4/12) orperitonela dialysis ( 8/5). Infections sites (cefpiramide / ceftazidime) included chronicexacerbated pyelonephritis ( 4/7), peritonitis on peritonealdialysis ( 7/5), bronchopneumonia (2/3), sepsis (3/4) ,soft-tissue abscess (1/1) and Douglas' anscess (1/1). The isolated infecting microorganisms were Esch.coli only in 4and 3 cases in cefpiramide and ceftazidime group, respectivelly,whereas the other isolates were predominantly multiresitantmicroorganisms such as Enterobacter aerogenes, Providencirettgeri,Klebsiella pneumoniae, Pseudomonas aeruginosa andProteus mirabilis. Other isolates were gram-positive cocci. Bacteriological success was achieved in 9 out of 15 patients incefpiramide group versus 12 out of 17 patients in ceftazidimegroup; the difference was not statistically significant. 6patients in cefpiramide group and 5 in ceftazidime group wereconsidered bacteriollogical failures being equally distributedbetween recurrence, persistence and superinfection. Cefpiramide and ceftazidime werte found to be equally efficientin the treatment of severe bacterial infections in uremicpatients. Considering the specific patient population, resistanceof infecting organisms and the sites of infection, the resultsobtained may be regarded as very favourable. The tolerability toboth antibiotics was very good.
