    Summary: The first results of a study of child injuries within theframework of the 3-01-351 research project have proved comparablewith the WHO and INSERM observations. They both suggest that theproblem is of paramount importance not only from health but alsofrom socio-economic aspects, especially in medium and highlyindustrialised countries, but the literature data warn that theproblem is becoming ever more serious also in developingcountries. In a global review of the first results of variousrelated studies, the authors draw attention to a great manyshortcomings in the understanding and origin of child injuries inthis country but also in more developed countries, although theWHO and INSERM have given an invaluable contribution to theproblems of the epidemiology and morbidity rate of injuries,those two key problems of accidents in children. A commonconclusion of a great many investigators, including the author's,is that a common definition of accidents in children is lacking,resulting in the lack of a common methodology in the follow-up ofthe phenomenon of injuries themselves. One of the essentialreasons for a poor follow-up of child injuries is the inadequatemotivation among health workers and also the shortage of medicaldocumentation in health institutions. In a comparable review ofthe literature it is evident that in many developed a specialimportance is given to the legislative aspects of child injuries,which unfortunately is not the case in this country.

    Title: Assessment of the authors's own studies of the use of the pediatric trauma score in the treatment of the head injured child

    Summary: The authors analysed the possibilities of how by the pediatrictrauma score (PTS) to exactly predict the degree and severity ofhead injuries in children with or without multiple bone fracturesand most easily identify a seriously injured child and ensureadequate transport to the corresponding medical centre. The studywas conducted at the University Child Surgery Unit of theInstitute for Mother and Child Health in Zagreb. It included 113children admitted to the Intensive Care Unit within 12 hoursfolowing injury. The study lasted 18 months. The results haveshown that within the PTS distribution there is the threshold ofthe injury seriousness amounting to 8 scores. Below this valuethe seriousness of the injury is significantly increased(P<0.01), representing a mortality threat. The conclusion is thatat the PTS value below <= 8 scores, the head injured child shouldbe treated in the corresponding medical centre. All the childrenwith the PTS value of 2 scores ended in death (P<0.001), whilethe survival scale value amounted to 3 scores. The authorsconclude that the use of the PTS method can detect the presenceor absence of significant craniocerebral injuries and improvethe survival rate. For this reason, the adoption of the PTSassessment and the proper treatment of the severely injuredchild, as well as the adequate transport to a correspondingmedical centre considerably minimize the invalidity and mortalityof the traumatized child.

    Summary: Modern civilization futher reports the appalling endemic aspectof children trauma to a major extent originating from roadtraffic accidents, which at the same time make the majoretiological factor in polytrauma and mortality in children. The paper deals with statistical data to illustrate the fact ontrauma-related mortality in children in the countries of Europe,the USA, and Japan, supported by the respective analysis of agroup of examinees treated at the Clinic for Children Orthopedyfor injuries sustained in road traffic accidents. The paper promotes the opinion that legislation makes the keyin prevention of children injures, particulary from RTAs, and itis therefore indispensable to promote such regulations toeffectively act upon the reduction of children trauma inCroatia.

    Title: The infection of war wounds in children by Acinetobacter calcoaceticus

    Summary: In the intensive care unit of the Institute for Mother and ChildHealth in Zagreb between October 1991 and February 1992 16children were treated suffering from wounds obtained in the wareffected areas of Croatia. The average age of the children was8.3 years. The war wounds were most commonly caused by fire armsand parts of mines and shells, and in 56% cases there were woundsto the pelvis and lower extremities. In 9 children the woundswere infected, and Acinetobacter calcoaceticus was isolated. Thiswas treated with the broad spectrum antibiotic imipenem. Theinfection was successfully treated in 80% of patients, but intwo children the wound became reinfected and Acinetobactercalcoaceticus reappeared. On the basis of their own tests theauthors recommend imipenem as the antibiotic of choice in thetreatment of war wounds in children infected with Acinetobactercalcoaceticus. It was also seen that treatment should continuefor more than 15 days, otherwise there is a great possibility ofreinfection and recolonization of the bacteria. Treatment withimipenem significantly reduced and avoided the formation ofaminoglicoide resistent strains.
