    Summary: From March 1988 to March 1991 51 patients with metastatic gastric cancer (Phase II Study) were administered combination therapy including high doses of epirubicin aand 5-fluorouracil. Out of 51 patients who entered the study 43 were evaluable (> 2 cycles of chemotherapy). The range of the patients' age was 33-70 years (M=55,6 years). In twenty-seven patients the performance score was <2, and in sixten 3 (WHO). Twenty-three patients underwent surgery 5-38 months prior to chemotherapy. The disease was primary inoperable in 20 patients. Distant metastases were recorded in the lymph nodes (21), liver (15), peritoneum (11), pleura (5), lungs (4), skin (2), bones (2) and brain (1). The disease was measurable in 28 and evauable but not measurable in 15 patients. Combined treatment involved 120 mg/m2i.v. of epirubicin on day 1, and 1000 mg/m2 of 5-fluorouracil in 6-hour infusion on days 1, 2, 3, 4, 5. The cycles were repeated after 4 weeks. Seventeen partial and 2 complete remissions were achieved (44% response rate; confidence interval 30-60%). The average survival of the responders was 9 months, and that of non-responders 5 monaths. Toxicity was moderate, reversible and well tolerated. The presented data confirm the esablished antitumorigenic activity and tolerabilty of high doses of 5-fluorouracil in the treatment of metastatic gastric cancer.

    Summary: From semteber 1991 to August 1994 75 patients with advaced gastric cancer entered phase III clinical trial. The aim of study was to determine activity of high doses of 5-fluorouracil and epirubicin (FE) vs. the same combination+cisplatin (FEP) in that group of patients. Out of 75 patients 62 (44 males and 18 females) were evaluable (>2 cycles of chemotherapy) The range of patient's age was 27-70 years (M=56), and ECOG performance status was < 3. The treatment involved in FE arm 120 mg/m2 of epirubicin i.v. on day 1, and 1000 mg/m2 in 6-hour infusion of 5-fluorouracil on days 1,2,3,4,5; in FEP arm the same combination + cisplatin 30 mg/m2 on days 2,4 was administered. The cycles were repeated after 4 weeks. In FE arm, 33 patients were evaluable with 10 partial and 1 complete remission (33,3%,and in FEP arm out of 29 evaluable patients 11 partial and 1 complete remission (41,4%) were observed. Median survival in FE group was 6,2 mos, and in FEP group 8,2 mos. Test results so far did not reveal the statistically significant difference between the two treatmens applied. Toxicity was tolerable and reversible. Our trial being still under way, the final results wil provide a more accurate answer with regard to the value of the two administered protocols.
