    Summary: The aim of this study was to analyze the labor outcome perinatal mortality and morbidity in 18 years period from 1975-1978 (n=135), 1982-1986 (n=116) and 1987-1992 (n=207), retrospectively, and to evaluate gathered results in managing 458 preterm breech deliveries. Main cause of perinatal mortality was intracranial haemorrhage in all three period. Hyaline membrane disease was the second cause of perinatal deaths. Statistically significant reduction of perinatal mortality from 20% in first studied period to 7.3% in the most recent period has been established by employing cesarean section instead performing vaginal delivery. Cesarean section mode of delivery when dealing with preterm infant and breech presentation seems to be choice to avoid mortality and high morbidity related to birth asphyxia and trauma.

    Summary: Study was undertaken to compare the effectivness of vaginal application of prostaglandin E2 and iv oxytocin infusion for ripening the cervix, induction of labor and its characteristics and outcome, in patients with PROM and unripe cervix at term. Seventy seven patients were treated with PGE2 vaginal gel which was administered in initial dose of 1 mg and maximal dose of 5 mg, 6 hours after PROM. The intervals between PROM and active stage of labour and delivery were significantly shorter in PGE2 vaginal gel group than in oxytocin group. PGE2 vaginal gel treatment significantly reduced percentage of operative deliveries.

    Summary: Coagulation changes often occur in pregnancies complicated with hypertension. Forty-seven pregnant women with chronic hypertension and previous stillbirths and/or intrauterine growth retardation (IUGR) or abrupto placentae were prospectively studied. Out of 24 pregnant women in whom the prophylatic heparin therapy was introduced, in 19 patients the blood pressure was decreased and in 5 the blood pressure values had increased. The gestational age at delivery in the treatment group was 38,4+1,6; 38,4-1,6 weeks, in the control group 36.4+3.7; 36.4-3.7 weeks which makes a statistically significant difference between these two groups (F=5.85;p<0.02). A statistically significant difference was found when compared the duration of the heparin therapy and its effect on the decreasing blood preasure (F=5.9;p<0.05). The overall mean birthweight among the controls was 2516+1186.8; 2516-1186.8 grams and in the treatment group new borns were statistically significantly heavier 3106.5+775.7; 3106.5-775.7 grams (F=4.1;p<0.05). There has been found a correlation between fetal growth and fibrinolysis. With fibrinolysis slowing down a higher birthweight occurred (r=0.65;p<0.0001).In this study a statistically significant correlation coefficient (r=0.4;p<0.004) was found between FDP and neonatal birthweight. Prophylactic use of heparin to prevent the worsening of chronic hypertension during pregnancy and improve fetal growth and perinatal outcome in those pregnant women who already had recurrent stillbirths and iugr with eclampsia or placental abruption seemed to offer a good prognosis. During heparin treatment in pregnancy coagulation factors reach normal values. Most evident are the increase of the fibrinogen concentration on the one hand and the lowering FDP with slowdown of fibrinolysis on the other.

    Summary: Coagulation and fibrinolytic systems are subject to considerable changes during pregnancy, the changes including an increased production of fibrin and a diminished capacitz of its removal. Fibrin plays a key role in the morphological adjustment of uteroplacental blood vessels and the maintenance of the maternal and fetal circulation integrity. Damages in the coagulation system in the course of a disordered (disturbed, irregular) pregnancies could lead to intravascular coagulation.

    Summary: This study represents a retrospective analysis ofpregnancies with chronic arterial hypertension and their outcomes. The aim was to evaluate the influence of arterial hypertension on 101 essential and 109 cases of secondary hypertension in comparison to the control group consisting of499 normotensive pregnancies. According to the obtained data, 27.7% of the women with chronic hypertension had proteinuria, 61% had bacteriuria and 58.6% had superimposed EPH gestosis. The occurrence of EPH gestosis among the experimental group (X*X=282.8%;p<0.001). The outcomes of pregnancies associated with chronic hypertension were: 19% preterm deliveries compared to the controls in wich only 9.2% preterm deliveries occured (X*X=14.4;p<0.001). Newborns from pregnancies with essential hypertension werw significantly heavier, weighing 3177+734; 3177-734 g, than those from pregnancies with secondary hypretension, which weighted 2578+932; 2578-932g. Perinatal mortality was higher in the study group and significantly higher in the pregnancies with associated secondary hypertension (30.3%) than in pregnancies associated wizh essential hypertension (15.8%).

    Summary: Total of 132 women with epilepsy were confined in the period from 1978-1989. Their pregnancies and outcomes were analysed. The special aim was to fon out if the anticonvulsive therapy has correlation with the occurrence of fetal malformations in the studied group of women. In 43.9% of pregnant women with epilepsy, methyl-phenobarbitine as an anticonvulsive drug was administered, while carbamazepin was applied in 13.6% cases. A combinations of phenitoin and phenobarbitone was prescribed in 18.9% of cases. Primidone was the drug of choice in 8% cases and 5.3% of patients were treated with various combinations of anticonvulsive drugs. Hyperemesys, threatened spontaneous abortion and premature labor complicated significantly more pregnancies in patients with epilepsy than on controls. Pregnancies from the studied group were terminated by the cesarean section in significantly more cases (11.2%) than in the controul group (5.4%). Newborns from mothers with epilepsy had a statistically lower birthweight (3173+575; 3173-575 g) than those born from healthy mothers (3376+510;3376-510g). Fifteen newborns or 11.2% were born with congenital malformationa, while among the control group of newborns onlz two were malformed. It is notoced that the newborns from mothers treated with phenitoin and phenobarbitone had dysmorphic anomalies of the face more frequently. The drugs mentioned above interfere with the methabolism of K vitamin and as a result of this interreaction, mothers and newborns can suffer from coagulation disorders. In conclusion it is important to mention that no anticonvulsant drug seems to be absolutely safe when used during pregnancy since each of them has a teratogenic effect on the fetus.

    Summary: The macrosomia seen in infants of diabetic mothers is thought to be related to fetal hyperinsulinemia. The autors studied infants of healthy gravidae in order to ascertain whether hyperinsulinemia might be present. They evaluated umbilical vein blood glucose, insulin, and HGH in infants of normal size weighing less than 4,000 grams (n=122) and 43 newborns weighing more than 4,00 grams, all of whose mothers had uncomplicated pregnancies and normal OGT tests performed according to WHO recommendations. Comparing the age, height and weight gain in pregnant women and duration of pregnancies, statistically no significant difference was found between the two research groups. Values of insulin estimated in the umbilical vein of newborns weighing more than 4,000 grams were significally higher than those in newborns weighing less than 4,000 grams (F=11.25;p<0.001). The authors conclude that hyperinsulinemia is present in a population of infants weighig more than 4,000 grams of healthy mothers. They believe that either the beta cells of the fetal panscreas are more sensitive to apparently normal levels of glucose, or perhaps some other unmeasured secretogogues are elevated in the mothers. The authors speculate that, as in infants of diabetic mothers, the observed hyperinsulinemia may lead to abnormal fetal growth.

    Summary: The study was designed to reveal dhe daily, weekly and seasonal rhytm of births. It was analyzed on the basis of 38,240 term deliveries (36,858 spontaneous, 1,382 programmed). The daily rhytm of spontaneous and programmed births was characterized by more deliveries ended during day time (60.5% respectively 89.3%), than during night time (39.5% respectively 10.7%). The weekly rythm of term deliveries was significant with maximum of spontaneous births on Monday, Wednesday and Friday (average 10,6) and by a drop of newborns especially Sunday (8.7), but also on Saturday (9.4). Similarly, weekly rhytm of programmed deliveries was remarkable by a drop of births on Sunday (0.1) and Saturday (0.3), and with a maximum on Monday (0.5). The monthly rhythm of births was characterized by a maximum of term deliveries in March (339.9) and August (339), and by a minimum in February (288.9) and October (298.2). The seasonal rhythm of programmed deliveries was characterized by a drop during summer(p<0.05). The pre-term deliveries were distributed continuously equally during the whole year by an average incidence of 5,6% from 4,1% in May to 6,9% in June.
