Summary: The cell cycle of tumor cells will be studied by means of
flow cytometry. The abnormalities in DNA contents, the dynamics of
17-beta estradiol and izocytric dehydrogenase and the levels of steroid
hormone receptors will be assesed as the prognostic factors in certain
malignant gynecological tumors. These tests would contribute to better
understanding of the biological behaviour, character and agressivity of
these diseases. They would also contribute to planning the extent, means
and duration of therapy, ways of follow up and improving the five year
survival of the patients. The study will encompass premalignant and
malignant lesions of the uterine cervix, endometrium, ovaries, uterine
sarcomas and trophoblastic disease. Fifty to 100 patients will be
enclosed in every investigated group.
Research goals: The aim of this study is to learn more about the
behaviour and characteristics of malignant gynecological tumors using
three different laboratory methods seldom used abroad and never before in
our country. There were never used before concurrently and together in the
same patients. The additional differentiation of the grade of malignancy
of the tumors is important for several reasons: 1. to get to know better
the biology of these tumors and 2. because the criteria of malignancy used
to determine the prognosis and therapeutic strategy are not perfect,
influencing thus the outcome of the disease. The diagnostic methods that
will be used are: 1. Flow cytometry - to study the tumor cell cycle,
because the malignant cell proliferation is mostly accompanied by the
changes in genetic expression and DNA contents. The analysis of the
histogram enables the insight into the grade of proliferation, mitoses and
tumor cells ploidy. 2. Monoclonal antibodies - in the premalignant and
malignant changes of the endometrium, the dynamics of steroid hormone
receptors, 17 beta estradiol and izocytric dehydrogenase will be
evaluated, 3. Comparison of the prognostic importance of the determination
of tumor cell cycle (by flow cytometry), steroid hormone receptor levels
and 17-beta estradiol and izocytric dehydrogenase levels with other
clinical prognostic factors in different tumors, with the aim to estimate
the therapeutic approach, natural history and the outcome of the malignant
disease.
