    Summary: The paper describes an information system designed to collectepidemiological data relevant for assessing the risk ofinfectious disease, primarily tuberculosis. It accounts forCroatia's epidemiological situation and compares it with that ofother European countries. The workers exposed to field living andworking conditions, are higher risk groups. They run not only therisk of other disease, but as well of Legionnaire's disease.Other topics covered are risks of TB among immigrants to Croatiaand among the young health workers treating and diagnosingtuberculosis. Measures for controlling and preventing TB aredescribed.

    Summary: The paper describes an information system based on which data oninfectious disease trends in Croatia is gathered. A part dealswith its war-time operation and the introduction of dailyreporting on infectious diseases. The latter includes diseasemonitoring among refugees and displaced persons. 1991 saw the theepidemiological situation in Croatia dealing primerily withtuberculosis. It examines the influence of war developments onthe risk od disease and changed incidence and prevalence ofdisease resulting from vast population migrations.

    Other: Rad je izvještaj o epidemiološkoj situaciji koja se procjenjujena osnovu dugogodišnjeg praćenja zaraznih bolesti u Hrvatskoj. Uovom sistemu praćenja, prati se i kretanje legionarske bolesti,tako da je čitava metodologija procjene rizika u populacijiHrvatske i ona koja se primjenjuje za procjenu rizika odlegionarske bolesti u rutinskom informacijskom sistemu, te naosnovu podataka Registra oboljelih od legionarske bolesti.

    Title: Antibodies to Legionella pneumophila and Legionella micdadei in patients at Haemodialysis unit

    Summary: There has been one case of pulmonary tuberculosis, in whomdirect immunofluorescent examination of bronchial washingrevealed L. pneumophila as well 52 of the 124 patients in theCenter were randomly choosed. Teir sera were taken for estimationof antibodies to L. pneumophila and L. micdadei. 3 patients(5,8%) had antibodies to L. pneumophila. The two of them hadantibodies to type 1 L.pneumophila with titers of 1:64 and 1:256.The third patient had antibodies to type 6 L. pneumophila withtiter of 1:128. No signs of the illness were observed. 4 patients(7,7%) had antibodies to L. micdadei with titer of 1:64. Theresults obatained in the group of dalysis patients were comparedto those obtained in the group of 336 vounteer blood donors fromCroatia. No statistically significant differences were found.

    Summary: The paper reviews Croation and foreign studies on the etiology ofLegionnaire's disease. It explains legionnella's habitat,favorable conditions to its growth and replication and its routesof transmission. Also cited is our discovery of a hithertounknown transmission route of the infection, i.e. compressed airsystem. Along with a description of susceptible population, alsogiven are other characteristics of a legionnaire's diseaseoutbreak and necessary epidemic control measures based on ourexperience of controlling such outbreaks in hotels. There is aspecial emphasis on preventive antiepidemic measures to forestallthe outbreak.

    Summary: Totally 131 standard plate count bacteria were isolated fromdrinking water and untreated surface water and identifiedaccording to a shema developted to permit rapid, simpleclassification of microorganisms to genius, species , or group.Actinomycetes and Aeromonas species were the two most commongroups of bacteria in chlorinated distribution water. Aeromonasspp. and Acinetobacter spp. were the two most common groups ofbacteria in raw water. Water that contained high densities ofbacteria known to be antagonistic to coliforms had low coliformisolation rates. The membrane filtration technique forenumerating bacteria recovered significantly higher numbers thanthe standard pour plate technique.

    Summary: The paper discribes an infectious disease notification system. Itis a necessary basis for the earliest possible observation ofrisks, the earliest possible intervention and for outbreakcontrol measures. Also, it is the only way to check theeffectiveness of the measures taken in controlling an outbreak orachieving prevention. The next topic is the actual operation ofthis information system. Some hazardous diseases, likehemorrhagic fever, malaria, cholera and Legionnaire's disease,require notification of helth authorities by phone, fax ormessenger besides the conventional way. Nationally (CroationNationl Institute of Public Health), the prime purpose of theentire system is to give an insight into the epidemiologilcalsituation, enable interventions in certain circumstances, connectepidemiological events across the Republic of Croatia. It is alsonecessary to link it with the world-wide information system topermit an exchange of information regarding the epidemiologicalsituation.

    Summary: In the period 1989-92 1,153 patients aged above 7 years anddiagnosed with primary pneumonia were admitted. 15 (1.3%) of themhad Legionnaires' disease. Legionella pnumophila was confirmedserologically in all by using indirect immunofluorescence (IFA)method. In one patient it was also shown by directimmunofluorescence in cough swab. Whereas diagnosis was used onlyin 185/1153 pnumonia cases, the causative agent was proved in8.1%. All cases were sporadic. 14 males and 1 female wereinvolved. The patients mainly manifested the signs of atypicalpneumonia. While all had a fever above 39o C, 11 had atemperature of 40o C. All patients also suffered from headacheand dry cough; 11 patients had myalgia. On the other hand,stabbing chest pain and catarrhal symptoms were present in onepatient only. Sedimentation was accelerated in all. Increasedlevels of transaminase, creatine phosphokinase and lacticdehydrogenase were reported in 50%. Electrophoresis revealedincreased alfa-2 values in all patients. Hyponatremia wasobserved in 12 cases. A C-reactive protein in excess of 200 mg/Lwas found in 5 of the 6 in whom the study was done. Since apartfrom epidemiological indicators, some clinical indicators arealso capable of pointing to the legionnaire's disease,etiological diagnosis needs to be done more often.
