    Summary: Clinical check up extends on ultrasonography, iv. urography and computerized tomography. Angiography is also necessary in order to show the vascular tumor anatomy. Choice of therapy is distated bu elementary prognostic factors: histological type, primary tumor volume, relations to normal renal tissue, regional metastases and number of distant metastases. Therapy:1. Cancer without distant metastatic disease: radical nephrectomy ( primary control of the artery and afterwards of the veins, renal extirpation together with the outer fat tissue and radical retropritoneal lymphadenectomy). 2. The advenced and metastatic disease: simple nephrectomy, regional lymphadenectomy and adjuvent hormonal or immunotherapy, in spite of quite unclear information about effectiveness of adjuvant therapy. 3.Tumors in solitary kidney or bilateral tumors: tumor enucleation or renal resection. Conservitive surgery is also possible in case of existence of contralateral kidney when the tumor is assymptomatic and of diameter smaller than 3 cm, well bordered, without regional metastases and histologically clear tumor cells.4. Solitary metastases: pulmonary solitary metastases appearing in a certain period of time after nephrectomy are an indication for extirpation. Pulmonary metastases appearing in the time of initial diagnosis have pearing in the time of intial diagnosis have a poor prognosis and shall be trated symptomaticaly. Bone metastases respond very good on radiotherapy. 5. Advenced patients age: a simple nephrectomy with regional lymphadenectomy only, without adjuvant therapy. 6. Poor general condition advanced disease, in general poor prognosis: kidney embolisation, eventually an adjuvant therapy.

    Summary: Classifications of malignant tumors is based on the known prognostic factors. Its aim is to make possible a choice of optimal therapy, estimation of disease'pronosis, control of own results and exchange of informations. Today, generally accepted is TNM system of classification, approved by EAU, WHO, EROTIC and AUA, although the EROTIC refused to accept the last TNM version from 1987. T is a sign of primary tumor, N are regional and M distant metastases. With prefix p, these signs became histopathologic classifications. There is no ideal system of classification and also the TNM has its advantages and failures. The most important advantage of few last versions is a division between clinical ( temporary ) and histopathological ( definitive) clasification. Between failures the one which is not only stubbornly repeated, but also more and more developed, is further complication instead of simplification. In such a way, modern classification constantly is going over possibilities of practical urologist, whom the classification on the first place is aimed for. According to special organs, in case of renal cell carcinoma for example, TNM separates veno-cavel thrombus, although this complication does not play a role in disease'prognosis. In case of bladder cancer, "papilloma" is again omitted as a special identity, although it is a most common bladder tumor. On the other side, one is stubbornly insisting on superficial and eep muscle invasion although every days praxis repeatedly demonstrates the impossibility for such a division. In case of prostatic cancer, there are no special remarks, but it seems that tumor grading is in a way neglected. Relating first of all on the possibilities of practical urologist, it seems that the most important aim of classification has to be determination of tumor volume and of histopatological grade in case of parenchyma tumors. By transitional cell carcinoma, the most important factors would be deep if invasion of the wall of diseased organ ( "staging") and cytohistologic grade of cellular differentiation ("grading"). In addition, for every urogenital malignancy, an important prognostic factor is also the existence and number of regional and distant metastases. In this chapter, the TNM system is compared with some other systems of classification which are still in use.

    Summary: The Osijek General Hospital was founded in 1739. Today, it serves medical needs of approximately 960.000 inhabitants of eastern Croatia. It has 1606 hospital beds and 2508 employees of which 332 are doctors. During the war of Serbia and the Yugoslav Federal Army against Croatia, in the time period from June 27 to September 1991, the Hospital was attacked from the neighboring Army garrison. The garrison was just across the street, at a distance of 50-100 meters. The heaviest attacks occurred between September 14 and September 17, 1991. At that time the Hospital was hit 56 times by mortar shells, 21 times by tank shells, 17 times by rockets from multiple rocket launcher, and many times by bullets from light weapons. Direct fire killed one and wounded three persons working at the Hospital.

    Summary: The cold wind of war started to blow in this area in May of 1991, with the massacre of Croatian policemen in a village 45 km from Osijek-Borovo Selo- one of the first Chetniks' strongholds. From that time Osijek has been living with the war as its only reality. Direct attacks on the town from all nearby terorist strongholds started in September 1991. Both heavy artillery and warplanes were used. Ever since that time, for 9 months now, killing of civilians and destruction of cultural heritage and material goods continues. Never in its past had Osijek been attacked in such a barbaric and cruel manner. The consequences are horrid: 774 persons killed, 346 of that number civilians ( 25 aged 18 or younger, and 8 aged 14 or younger); 3,763 persons wounded, 1,353 of that number civilians ( 132 aged 18 or younger, 67 aged 14 or younger); Up to now there are 1500 war-invalids registered, 80 % of them physically handicapped; Destruction of infrastructures is immense. In spite of frequent direct attacks on health care institutions, essential health care was continually provided.The consequences of this war are extremely severe and the healing process-in all the segments-will be both long and difficult. In the short-term there is a need for help in repairing or rebuilding and basic furnishing of so many destroyed and robbed homes on currently occupied territories. In the long-trem, the best form of help would be to invest in the development of the town, following organisational and developmental principles implemented in European towns.

    Summary: The study group in this work comprised 518 males and 482 female,mean age 45.25 years treated by the method of extracorporealshock wave lithotripsy(ESWL). Out of 1194 stones, 27.81% weresituated in the renal pelvis and pelvicoureteral segment, 39.29%in calices, and 22.01% in the ureter. More than three stones inthe same kidney were regarded as multiple stones and registred asa single case (4.52%). The success rate of nearly 90% is same asin literature and presents ESWL as a method of choice.

    Summary: A thousand urolithiasis patients were treated by the method of the extracorporeal shock wave lithotripsy using a Siemens Lithostar second generation lithotriptor.Data obtained three and six months after the treatment were analyzed.The study group comprised 518 males and 482 females,mean age 45.25 years.Medication with analgetics and occasionally sedatives was required in 9.6% of patients.Anesthesia was administrated only inchildren,and when the method was combined with percutaneousnephrolithotripsy or ureterorenoscopy (22 patients).Out of 1194Out of 1194 stones, 27.81% were situated in the renal pelvis andpelvicoureteral segment, 39.29% in calices and 22.01% in the ureter.More than three stones in the pelvis and calices of the same kidney were regarded as multiple stones and registered as single case(4.52%).There were 6.37% of staghorn stones.The success rate of nearly 90% coincided with the results reported by other authors and once again presented extracorporeal shock wave lithotripsy as a method of choice in the treatment of urinary stones.

    Title: Destructions of health institutions in Slawonia and Baranya during the war against Croatia ( by the end of 1991)

    Summary: During the war against the Republic of Croatia in the area of Slavonia and Baranya health institutions suffered great damages. Medical centres and hospitals in Vukovar, Vinkovci, Pakrac and Nova Gradiška were practically destroyed, as well as sanatoriums in Lipik and Daruvar, while the General Hospital in Osijek and Health Center in Daruvar were badly damaged, so that the line of agression against Croatia is marked by the destroyed and damaged health institutions.During those attacks onto the hospitals of Eastern Croatia, four medical workers were killed, 12 were injured, and the fate of 2 doctors is unknown. Five patients were killed. The complete material damage on the institutions mentioned above is about 360 million DM. While it was possible, all those institutions were working, until some of them were occupied by the agressors. Mostly in cellars and shelters of damaged and destroyed civiilian hospitals, very often under the artillery and air force attackes, about 7000 wounded ( and 50 JNA soldiers among them) were treated by the end of 1991. All health institutions were properly marked with Red Cross signs. The number of those institutions and the fact that some of them ( for example general Hospital in Osijek) were shot at from the immediate vicinty, that very precise artillery and air-craft fire were used and that some of the hospitals were shot before some military important installations, points to the fact that in the war against Croatia irregular Serbian troups and the federal army deliberately destroy health institutions, even though they have been protected by a number of international humanitarian conventions.

    Summary: At the Departments of Urology at Osijek University Hospital and Sisters of Mercy University Hospital, Croatia Urologic Clinic High Medical School, Essen, and in Hussens Foundation Evangelical Hospital, Department of Urology,Pediatric Urology, Essen, Germany, 133 patients undergoing radical prostatectomy for cancer were followed up during 6 months after operation. The indication for operation included a localized disease. Retropubic prostatectomy with extraperitoneal lymphadenectomy was the generally accepted technique. Our experience corresponds to the results of other authors. There was no postoperative mortality, Vesicourethral stenosis (14.2 % )was the most frequent complication, which was higher than the published results. Although not yet accepted as a routine operation, retropubic prostatectomy when correctly performed, yields good results in localized prostate cancer, with practicaly no mortality and with an acceptable percentage of complications.

    Summary: The paper presents the results of the tretmant of the first 500patients treated with the lithotriptor SIEMENS "LITHOSTAR". In500 patients with 607 stones 521 tretmants were performed in 799sessions. Six months after the ESWL treatment 522 out of 607stones(86%) were desintergrated completely, i.e. 87.2% of thepatients were stone-free. The best results were obtained withsolitary pelvic (95.51%) and ureteral stones (95.8%).Complications due to the obstruction were presents in 10.4% andhave been solved by endoscopic procedures. These results presentESWL as the best urolithiasis therapy method.

    Summary: It seems likely that exist a variety of distinctivepathophsiological subtypes of idiopathic hypercalciuria. In thisreport we showed the results of a metabolic evaluation in 65normocalcemic stone formers with hypercalciuria.The patients wereclassified according to Jongens method. We had simillar resultslike in literature. However, we classified more patients amongcalcium leakers than in mixed groups.

    Summary: Data on magnesium excretion in stone formers are conflicting. Arecent studies indicate that magnesium deficiency may besignificant pathogenic factor in soubgroup of stone patients.Sixty nine controls and 196 patients with kidney stones werestudied. When compared to controls daily Mg excretion was higheronly in those patients who are Ca hyperabsorbens or leakers butnot in normocalcemic patients or normal controls. Our data showthat Mg excretion is closly conected with stone disease.

    Summary: Here we presents results of the ESWL treatmant of the first 500renal stone patients. Out of 607 stones, 30.48% were situated inthe pelvis pelvioureteral segment, 20.5% in lower calices and18.45% in the ureter. Over 3 stones in the same kidney wereregarded as multiple stones and registreted as asingle case. In52% of the cases the stone was over 10mm large. Six months afterthe tretmant the stones were desintegrated. The succes rate of86% make ESWL as the method of choice in the stone tretmant.

We are also able to determined hydroxyvitamin D(25-OH-D) and 1,25 dihydroxyvitamin D (1,25(OH)2 D) by theradioreceptor analysis.     Summary: The paper discuss the results of 3-months and 6-months follow upof 228 patients with ureterolithiasis treated with ESWL.Preioperative auxillary measures were required in 33% of thetreatments. Auxillary measures included ureteral cathetermanipulation in 66 patients, insertion of double-J stent in 6patients and prior percutaneous nephrostomy in 6 patients. Threemonths after the treatment 91,2% of the patients were consideredas stone free. Our experience suggest ESWL treatment as a methodof choice in ureteral stones treatment.

    Summary: Since May the 2nd 1991 until November the 2nd 1992 the total of 4425 injured persons were treated in Clinical Hospital Osijek, Croatia. A hundred and fifteen or 2.6 % had urogenital trauma, sixty-four or 56 % of them penetrating kidney lesions. The percentage of kidney lesions among the total of 4 425 injured persons was 1,4 %. Sixteen or 25 % of them were caused by gunshot bullets and forty-eight or 75 % bu fragments of mines, mortars or grenades. The most of hundred and fifteen urologically injured patients had associated injuries of some other major organ system, particularly in the abdomen. An isolated kidney injury was registered in only three cases. In 53 patients ( 82.81 % ) surgical access was the laparatomy, and in 9 ( 14,06 % ) lumbotomy ( three of them followed by laparotomy as the conjoint injury of abdomen was intraopertively detected). In two cases the thoracophrenolaparotomy was done. Nephrectomy was performed in 16 patients ( 25.00 %), and by 46 injuried persons ( 75 %) a conserving procedure was performed: renography in 28 (43.75 %) partial nephrectomy in 5( 7.81 %), and exploration in 15 ( 23.44 %). Intrahospital death occurred in eleven ( 18 % ) cases, seven of them in operating theater. An evaluation of the six months' follow-up results of the treatment by 90 % of survived patients was presented. In conclusion, the frequency of kidney war injuries was lower then usually reported in literature. Associated abdominal injuries justified surgical access by laparatomy. According to our opinion a high mortality rate was caused by numerous politraumatic cases in decompensated general condition. Long time follow-up confirmed the benefits of the undertaken conserving surgical procedures.

    Summary: In the 1991/92 war against Croatia, since May 2. 1991 until November 2. 1992 a total of 4425 injured persons were treated in Clinical Hospital Osijek, Croatia. The injuries of the outer genital organs were present in the percentage of 0.7% or in thirty-two persons. Eleven injuries ( 34 %) were caused by the gunshots bullets and twent-one ( 66 % ) injuries by the fragments of mines, mortars and granades. The different types of the genital injuries have been observed. Scrotal injuries with or without testicular involvement have been present in nineteen cases ( 59 %), penile injuries in four ( 13 %) and combined penile plus scrotal injuries in nine ( 28 %). Genital injuries were combined with others, mostly abdominal and extremities injuries in twenty-eight cases of 87.5 %. Ten orchidectomias, six testicular and corpus cavernosum reconstrucions have been performed. The corpus cavernosum reconstructions have been done by using fascia musculi recti abdominis free transplant. Intraoperative mortality was 12.5 % and mostly caused by associated injuries. Long time follow-up is under way to assess benefits of undertaken operative procedures. In conclusions, there was an imperative of extremely conservative and immediate reconstructive surgery in case of genital injuries.

It appeared more frequently in men ranging in age from 30 to 50 years, i.e. in the period of the greatest activity, pointing out not only medical, but also a social feature of the problem. There was no statistic significance of the relation between the kind of profession and work and the incidence of urolithiasis. Although it appeared more frequently in the conditions of raised rather than normal environmental temperature, no statisticaly significant difference was found. Neither the relation found between the kind of water consumed and stone formation was statisticaly significant, while daily intake of liquids was significantly related to the incidence of urolithiasis. The influence of diet habits was not found to be statisticaly significant.

    Summary: The paper discusses the results of three and six months follow-up of 220 ureterolithiasis patients treated with extracorporeal shock wave lithotripsy using the second generation Siemens lithotriptor "Lithostar". The treated group comprised 135 males and 93 females ranging in age from 14 to 73 years. Among the total of 239 stones there were 16 nonopaque stones. The upper ureteral third situated 25 % of the stones, whereas 21% of them were the middle and 54 % in the lower third. Perioperative auxiliary measures were required in 33 % of the treatments. Auxiliary measures included ureteral catheter manipulation in 66 patients, insertion of double -J stent in 6 and prior percutaneous nephrostomy in 5 patients. Three months after the treatment 91.21 % of the patients were considered as stone-free, and after six months the stone-free ratio increased to 96.23 %. The majority of the patients with stones located in midureter were treated in the prone postion. There was no significant difference of the stone-free outcome regarding the ureter level. It is our experience that ureteral stones respond very well to "in situ" treatment. We believe that " push & bang" or ureteroscopy techniques should be applied only when " in situ" treatment fails.

    Summary: One thousand urolithiasis patients were treated with extracorporeal shock wave lithotripsy using the second generation Siemens lithotriptor "Lithostar". The data analyzed were obtianed three and six months after the treatment. The trated group comprised 516 men and 462 women, at the mean age of 45,25 years. Pretreatment analgesics and occasionally sedatives were required in 9,6 % of the patients. Anesthesia was administered only in children and when ESWL was combined with percutaneous nephrolithotripsy or ureterorenoscopy ( 22 patients). Among the total of 1194 stones, 27,81 were situated in renal pelvis and pelvicoureteral segment, 39,29 % in calices and 22,01 % in ureter. More than three stones in pelvis and calices of the same kidney were regarded as multiple and registered as a single case ( 4,52 % ). There were 6,37 % of staghorn stones. The success rate of almost 90 % was similar to the results reported by other authors, once more affirming ESWL as the method of choice in the tratment of urinary stones.

Three months after the treatment 91.21 % of the patients were considered as stone-free, and after six months the stone-free ratio increased to 96.23 %. The majority of the patients with stones located in midureter were treated in the prone position. There was no significant difference of the stone-free outcome regarding the ureter level. It is our experience that ureteral stones respond very well to "in situ" treatment. We believe that "push & bang" or ureteroscopy techniques should be applied only when "in situ" treatment fails.

    Summary: Three thousand urolithiasis patients were treated with extracorporeal shock wave lithotripsy using the second generation Siemens lithotriptor " Lithostar". The data analyzed were obtained three and six months after the treatment. The treated group comprised 1554 men and 1446 women, at the mean age of 45.25 years. Pretreatment analgesics and occasionally sedatives were required in 9.6 % of the patients. Anesthesia was administered only in children and when ESWL was combined with percutaneous nephrolithotripsy or ureterorenoscopy (66 patients). Among the total of 3582 stones, 27.81 % were situated in renal pelvis and pelvicoureteral segment, 39.29 % in calices and 22.01 % in ureter. More than three stones in pelvis and calices of the same kidney were regarded as multiple and registered as a single case ( 4.52 % ). There were 6.37 % of staghorn stones. The success rate of almost 90 % was similar to the results reported by other authors, once more affirming ESWL as the method of choice in the treatment of urinary stones.

    Summary: Metabolic evaluation of patients with urolithiasis includesevaluation of the parathyroid glands. A such study have not yetbeen done in the area of north-east Slavonia and Baranja. In thestudy participated 95 healthy subject aged 20-70 yeras. The serumparathyroid hormone(PTH), plasma and urine c-AMP were determinedby RIA methods using commercialy avaible kits. Calcium, phosphateand creatinine were determined by standard methods with automaticanalyzer. Like in literature, we did not find a significantcorrelation between the values of serum immunoreactive PTH andfractions of c-AMP in urine. We found that RIA determination ofPTH is better indicator of parathyorid gland function.

    Summary: In this study the influence of working envirovment on occurenceof renal calculi has been examined, with special attention to theinfluence of high temperature. Investigating 456 workers at hightemperature 22 persons suffering from urolithiasis have beenfound (4.82%). In the control group of 2626 workers, working under common condition, urolithiasis have been found in 168persons (6.39%).The dfference is not statisticaly significant.The density of urine has been measured and the density isstatisticaly significant,but apsolute values are affected withdaily lqud uptake. Our results suggest that heat does not affectprocess of urolithiasis.

    Summary: One of the disorders present in stone formers ishyperparathyroidism (HPT). Therefore. the exact determinationhormone status of parathyrioid glands is of grate imporatance.The aim of this study was to investigate by means ofradioimmunologic and biochemical methods the frequency of primaryand secondary HPT in urolithiasis patient. The study comprised215 patients with proven urolithiasis and the goup of 70 healthyindividuals. Our results are lke in literatue, the frequency ofprimary HPT associated with urolithiasis is 5%. In our patientspermanent hypercalcemia was the key finding. However, ourexpirience suggest there is not individual test for diagnosisprimary or secundary HPT in patients with urolithiasis.

    Title: Occupation and conditions at work as risk factors in the development of urolithiasis

    Summary: There are nultiple epidemiological risk factors in the development of urolithiasis. This study is dealing with the role of occupation and conditions at work as risk factors in the occurence of stone disease. The examinees were men working in several factories in the region of Slavonia and Barania. They were administered the general questionnaire of the Multicentric Programme of Investigation of Urolithiasis in Croatia and were asked about age, occupation and conditions at work. Anong 1.434 examinees (avarage age 39,7 % +/- 10,7 yr) 73 (5,1 %) had urolithiasis (average age 42,3+/-8,9 yr). Most patients with stone disease were workers working under conditions of high temperature ( 14,9 %) and were more frequently affected than heavy physical wokers who were not working at high temperature. The office workers were not working at high temperature. The office workers were less affected ( 4,3 % ) than dirvers (5,7%), but not significantly. Psychological testing of drivers by the method of Cornell index showed that stress too might be a risk factor in the occurence and development of urolithiasis.

    Title: Incidence and risk factors of stone formation in working population in Osijek community

    Summary: An epidemiological study of incidence and risk factors of stone disease was carried out in 28 firms of the Osijek community. In this tudy 7833 working individuals, making up 15,68 % of working population of the Osijek community have been examined. The subject were questioned about personal and family history of urolithiasis. Certain risk factors related to occupation, conditions of the raised temperature in the working environment, alimentary habit and fluid intake were examined. Urine pH and urine osmolality were measured. Urine sediments were examined for cristalluria. Urolithiasis has been found in 466 ( 5.95 %) subjects. Urolithiasis appeared more frequently in men in the age of greatest life activity and productivity, from 30 to 50 years of age. The influence of family heredity was statistically significant ( P smaller than 0,05 ). Urolithiasis appeared more frequently in certain professions, but no statistic relation has been found between the higher frequency of urolithiasis and the kind work ( P higher than 0,05 ). Urolithiasis has been found in 51 ( 6,51 % ) subjects working in the conditions of raised temperature in the environment and in 415 ( 5.93 %) subjects working at normal temperature although the difference was not statistically significant ( P higher than 0,05 ) The water from public aqueducts was used by 60,80 % of subjects, and 4,47 % of subjects concummed mainly mineral water. We failed to find the relation between the type of water consummed and the higher frequency of urolithiasis ( P higher than 0,05), but the intake of liquids frequncy of urolithiasis ( P smaller than 0,05). Out of all 61,94 % of subjects concummed mainly protein food. Statistically, there was no relation between the consummation of mainly protein food and the higher frequency of urolithiasis ( P higher than 0,05). The high frequency of urolithiasis ( 5,95 %), high relapse rate ( 40,77 %), and the apperance of urolithiasis in the age of greatest life activity and productivity shows that urolithiasis in an important social and medical problem of working population in the Osijek community. The proved relationship with certain risk factors points to the fact that we should, in collaboration with health-services of firms, improve the prevention metaphilaxis, diagnostics and treatment of urolithiasis.
