Summary: The urinary stone is a very frequent health problem. Theepidemiological studies show increase in the frequency of theupper urinary tract stones in developed countries. Differentauthors report that urinary stones are cause of hospitalizationin 1-3 percent hospitalized patients or 15-24% patients aturological department. A general estimation is that in developedcountries 10% of male and 4% of female population suffer fromthe renal stone disease. The urinary stone have a high recurrencerate as high as 80%. The urolithiasis occurs most often in theperiod of the person highest activity, between the age of 30 and 50 years. The investigation of urolithiasis was performedfrom the various aspects. However, the pathogenesis ofurolithiasis is unknown so far. In the treatment of urolithiasis exist two principal treatment approaches. The first treatment ofurolithiasis is surgical. This treatment has been improved by theusing new techniques like the Extracorporeal shock wavelithotripsy ESWL. The second is medicament treatment, which mainaim is prevention of the urinary stones. The long lasting effectsof the ESWL treatment on the human body, particularly on therenal function has not been investigated so far. In 90% of thefrequent stone former a metabolic disorders could be find. Thus,the metabolic elaboration of the stone former gives usopportunity to prevent the urolithisais. In the world existsenormous interests for the investigation of the urolithiasis withthe goal to introduce pathogenesis of the urolithiasis and to introduce a methods for the finding the risk factors inpathogenesis of the urinary stones. Because that the Multicentricprogram of the urolithiasis investigation in the Republic ofCroatia has the justification and the significance.

Research goals: The goals of this proposal are investigation of the pathogenesis, the ways of diagnosis and the prevention of the urolithiasis. We will make emphasis on the pathogenesis of the urolithiasis in frequent stone former which does not have a known metabolic disorder in the background and where we are not able for the medicament prevention of the urolithiasis. We will investigate the epidemiological situation of the urolithiasis in the Republic of Croatia, and the role of microclimate factors in the forming of the urinary stones. These data we will use for the making the epidemiological map of the urolithiasis in the Republic of Croatia. In our clinic we performed the ESWL treatment of the urolithiasis and we are able to follow up effects of this treatment on the human body with special emphasis on the renal function. We have large group of the patients treated by the ESWL and in whom has been used endoureteral device "double J". Thus, we are able to follow up these patients. Our goal is to continue our investigation and by the multicentric approach to find out more about this great problem of the national pathology. Our findings and experience will make improve in the work of primary medicine.
