Title: Effects of imipramine on the continent zone of urethra: a
clinical and urodynamic study

Summary: A total of 113 women with stress incontinence were treated
withimipramine in a daily dose of 75 mg through a period of 2
months.Fifty-one (45,1%) women stated that they were continent
aftertreatment with imipramine, while 14 (12,3%) had a slightimprovement.
Forty-eight (42.4%) women declared that theycontinued to be incontinent,
and the treatment was stopped. Thestudy suggests that the women with a
suspension defect of thebladder neck react considerably more weakly to the
conservativetherapy in contrast to women without a suspension
defect.Imipramine leads to a considerable prolongation of the continentzone
of the urethra (2,1+-0,48 cm in the lithotomy positionbefore the treatment
and 2,69+-0,43 cm after the treatment withimipramine, respectively
1,42+-0,45 cm in the stress conditionbefore the treatment and 2,61+-0,53 cm
after the treatment, whichrepresent a statistical significant difference).
In the authorsopinion the conservative treatment has its place in
treatingstress incontinence of urine only in highly selective
cases(patients without any suspension defect of the bladder neck).

Summary: Twenty-five patients with retrograde ejaculation / loss
ofemission were treated with ephedrine sulfate or imipraminehydrochloride.
l7 of them suffering from both diabetes andretrograde ejaculation and
treated with ephedrine or, in casethat ephedrine failed to convert
retrograde ejaculation intoanterograde ejaculation, with imipramine.
Positive results wereobtained in 5/17 (29.3%) patients, i.e. in 3 (17.6%)
and 2(11.7%) patients on ephedrine and imipramine, respectively. Adaily
dose of ephedrine was 50 mg, and of imipramine 75 mg,during a four-week
period.
In the group with retroperitoneal lymphadenectomy, aftertreatment with
ephedrine, only 1 (12.5%) had the retrogradeejaculation while the rest of
patients (N=7) continued to havethe lack of semen emission.
These 7 patients were treated with imipramine and 3 of them(42.8%) obtained
the anterograde ejaculation.
In 1/3 of patients with retroperitoneal lymphadenectomy anddiabetes, with
retrograde ejaculation or loss of semen emission,the conservative treatment
can offer improvement or conversion toanterograde ejaculation.

Summary: A total of 36 female patients with genuine stress urinary
incontinence underwent Raz needle suspension with fixation of suspension
sutures to the iliopectineal ligament (transvaginal Burch procedure). By
fixing suspension suttures to the iliopectineal ligament we expected to
achieve a static suspension independent of everyday patient activities.
considering the small number of patients and limited followup, our results
revealed continence in 80 to 85% of the patients after 3 years. We believe
that fixation of suspension sutures to the iliopectineal ligament can
favorably influence long-term results of needle suspension in the treatment
of female stress urinary incontinence.

Summary: A sigmoidorectal pouch was constructed in 20 patients (18
with invasive bladder cancer, 2 with complete urethral destruction with
multiple vesicovaginal fistulas). The rectal-dynamic studies showed that by
detubularization and reconfiguration of the sigmoid colon and rectum, we
obtained a low-pressure reservoir, with the high-pressure contractions
eliminated. Preoperatively, the basal rectum pressure was 21,4 cm H2O;
rectum contractions reached a mean of 27,6 cm H2O (maximum 48 cm H2O).
Postoperatively (9-36 months) the mean basal pressure of the pouch was 19,3
cm H2O (p>0.05; t test comparison with the preoperative value), with a mean
contraqction value of 19,1 cm H2O (p<0.05). all the patients experienced
continence day and night with a pouch empting frequency of four times
during the day and once a night, after 9 months. By fixing the pouch to the
promontory or psoas muscle, without compromising the blood supply to the
pouch, the risk of ureteral kinking and upper urinary tract dilatation were
considerably decreased. The price to be paid for almost perfect continence
was hyperchloremic acidosis in most patients.
