Summary: Using the urodynamic investigation of urinary bladder and
sphincter we explore: 1. Continent procedures in replacement of bladder and
sphincter (in malignancy and total affunction) with detubularised bowel or
aloplastic matherial tending to create the continent low pressure
reservoir. 2. The assessment of erectile insufficiency and obstructive
azoospermia with suggestions for adequate treatement. 3. Possibility for
creation of plausible expert system.
Keywords: urodynamics, cystectomy, incontinence, male sterility, urinary prosthesis, expert system
Research goals: If we summarize our intentions, the aim of our
research could be defined in three points: 1. Defining of the suitable
surgical parameters in the usage of alloplastic material (endoprotheses)
and detubulated bowel segments in surgical healing of heavily damaged
bladder and sphyncter. The aim is to establish a completely continent low
pressure reservoir which would be controlled by urodynamic, ultrasonic and
Doppler investigation. The expected contribution would be manifested in
clinical treatements applied on the patients with certain malignant tumors
or denervated bladders, e.g. paraplegics. 2. Another group would include
patients with the secondarz male sterility due to obstructive azoospermy or
complete neurogenic denervation. This would be followed by the functional
dynamic investigation of corpora cavernosa, ductus deferens or seminal
vesicles according to the indication and disease defined. The aim is to
establish clear diagnostic and treatment protocol, which would be based on
the relation between (pressure-volume-flow) parameters registered by
urodynamic device, ultrasound and Doppler. 3. Continuation and
finalization at Urodynamic expert system. Due to various interpretations of
the urodynamic parameters by nonexperts in the veryfield, the aims are
construction and realization of the computer program, which would give the
most probable diagnoses, and direstions for the further treatements. Other information about the project.
