Summary: Diagnostics of psychological disturbances is based on knowledgeof normal psychological life and psycho pathological changes aswell as on the knowledge of biological and social influences on adetermined person. Separate monitoring of psychological functionsin real life ins almost impossible. In fact, the wholepsychosomatic reaction is an entirety. Based on these principlesthe World Health Organization classifies alcoholism as a deseaseand its withdrawal syndrome, delirium tremens and other alcoholicpsychoses, as complications of this disease. among these threeclinical entities there is such a significant cause-consequencelink that considerations of the ethiology of this group ofdiseases cannot be separated from the analysis of the causes andconsequences of alcoholism. Latest literature on this field is reviewed in this paper.Statistical data on the occurrence of alcoholic psychoses in therecords of the University Department of Neurology, Psychiatry,Alcoholism and Other Dependencies of "Dr.M.Stojanović" UniversityHospital , Zagreb during a 10 year period are also presented.Theoretical considerations of psychiatric complications ofalcoholism from pharmacodynamic, biologic and sociologicalaspects are also reviewed.

Summary: In order to get better insight in cognitive functions andhaemodynamic changes in mayor depression, 35 patient wereexamined by means of ERP and TCD. The severity of depression wasgraded on Hamilton depression rating scale. The results haveshown significantly diminished mean amplitudes and prolongedlatencies of P300 at all electrode registration points indepressive patients, compared to normal (p). Statisticallysignificant decreasing of cerebral blood flow velocities werealso observed in depressive patients, compared to normalcontrols. These disturbances are significantly connected with cognitiveevaluation of stimulus, as a second stage of perceptualprocessing. Significant changes in P300 amplitude and latency areassociated with some changes in cerebral blood flow velocities indepressive patients.

Proceedings title: Proceedings of 32 nd International Neuropsychiatric Symposium
    Language: engleski
    Place: Graz, Austria
    Year: 1992
    Pages: from 101 to 101
Meeting: Postgraduate Course and 32 nd International Neuropsychiatric Symposium, 14.-20 June, 1992.
    Held: from 06/14/92 to 06/20/92
Summary: Brainsteam auditory evoked potentials (BAER) abnormalities ofwaves IV-V were reported mostly in patients having upper pontineor mid brain lesions (Starr and Hamilton, 1976; Howard andRositer, 1977). Straummanis et al. found longer BAER latenciesand lower amplitudes of waves II and III in chronic marijuana(THC) users. The aim of this study was to determine whether therewere any evident brain dysfunctions in heroin addicts, (accordingto DSM-III-R criteria), with the mean age 27 (+-3.6) and meanduration of addiction 3 years, and thirty normal, healthysubjects of approximately same age were taken in the study. Alladdicts and all controls were tested by means of delivering theclick stimuli, of 0.1 ms duration, presented monoaurally at 10and 20 Hz stimulation rate (60 dB NHL) with piezoelectricearphone. The contra lateral ear was masked with white noise of30 dB lower intensity than that of the click, and 2000 sweepswere averaged twice for each polarity in Neuroscience Neuromappersystem. Bipolar recordings were taken between the ipsilateralearlobe and the vertex. absolute wave latencies I-IV and I-V andpeak-to-peak amplitudes of waves I and V were analyzed. Theresults of the two groups were compared by means of One-wayAnova. The results of BAER variables in the group of drug addictswere abnormal and proved to be of longer latencies and loweramplitudes compared to those of the normal controls. Patientswith longer dependency duration proved to have longer interpeaklatency III-V and lower amplitude of the wave V.

Proceedings title: Abstracts of 9th World congres of Psychiatry
    Language: engleski
    Place: Rio de Janeiro, Brazil
    Year: 1993
    Pages: from 185 to 185
Meeting: 9th World Congress of Psychiatry
    Held: from 06/06/93 to 06/12/93
Summary: We compared 42 patients diagnosed as maior depression (group A)with 45 schizophrenic patients (group B). Differences betweenthese two groups of patients concerning the biochemicalparameters as well as characteristics of evoked endogenepotentials (ERP) were analyzed. The results have shown that theblood levels of serotonin, prolactin and 24 hours concentrationof 5-HIAA are rather low in the group A, especially in thosepatients with strong suicidal tendencies. Opposite results havebeen observed in the group B which are similar to the findingobserved during the manic phase of bipolar affective disorder. Astatistical significant decrease of amplitude P 300 and prolongedlatency have been found in the patients from group A. In theschizophrenic patients the P 300 amplitude were heterogeneous, but with predominantly decreased tendency and withoutprolongation of latency, suggesting some changes in precognitionprocess.

Title: Correlation Between Mental deterioration and Cerebral Vasoreactivity (CVR) in Patients with Multi-infarct Dementia (MID)

Proceedings title: Abstracts of 9th World Congress of Psychiatry

    Place: Rio de Janeiro, Brazil
    Year: 1993
    Pages: from 280 to 280
Meeting: 9th World Congress of Psychiatry
    Held: from 06/06/93 to 06/12/93
Summary: The origin of MID is still controversial. The impairment of CVRmay be responsible for MID progression. Therefore, 45 patientswith MID (according to DSM-III-R Hachinski score +7) wereanalyzed by psychological rating scales (FMMS, GBS, SCAG) andTranscranial Doppler (TCD) with acetazolamide (AZT) test. Theresults shawed the impaired TCD findings in all patients beforeAZT stimulation, indicating vascular origin of disease. After AZTstimulation the reduced cerebral vasoreactivity was observed inpatients with moderate and severe mental deterioration, SCAG+56,GBS+52 and FMMS-25 (p-0.01). indicating exhausted CVR, possiblydue to reduced metabolic brain demands. In conclusion, AZT testcombined with TCD is helpful in clarifying the hemodinamic originof MID.

    Held: from 06/09/91 to 06/14/91
Summary: Patients with diagnosis of depressive psychosis were studied. They suffered from a clinically severe form of depression, requesting in-patient treatment. This group of male patients was admitted to the Department ofClinical Psychiatry of "Dr.M.Stojanović" University Hospital fromJune 1988 till the end of June 1989. In total, 42 patients were examined by a battery of clinical andlaboratory tests. The results of the Study indicated that the results of somebiochemical tests - such as 5-HIAA in the 24 hour urine, bloodserotonin level, blood prolactin level - were close to thelowered values of the laboratory standards, and in correlationwith the Hamiltons depressive scale and Becks depressionquestionnaire, they take the position, in the distribution of thefindings, of high depression scale. The findings were incorrelation with the severity of the depressive picture of thedisease.

Summary: This is a study of clinical and biochemical disturbances inalcoholics with clinical picture of withdrawal syndrome anddelirium tremens treated in the Intensive Care Unit of"Dr.M.Stojanović" University Hospital, Zagreb. Two groups werestudied, each numbering 30 examinees aged from 20 to 65 years, one including the cases with delirium tremens and the other thosewith withdrawal syndrome. The frequency of the following parameters: hypoglycemia, alkalosis, acidosis, erytropenia, decreased significantly greaterin patients with the clinical picture of delirium tremens inrelation to those with withdrawal syndrome. The prognosis of delirium tremens is significantly improvedthanks to the intensive care and administration ofpsychopharmaca. Mortality is decreased to minimum and in thestudied groups there were no lethal cases.
