    Summary: For the evaluation of patients wits prosthetic heart valvesDoppler echocardiography is superior over other noninvasivetechniques because it allows quantitative assessment of valvefunction.As a noninvasive method Doppler is safe,easilyrepeatable and provides haemodynamic data that closely correlatewith parameters obtained by invasive procedures. The purpose of our study was to evaluate Dopplercharacteristics of mechanical and tissue mitralprosthesis:Star-Edwards,Bjork-Shiley,Hall-Medtronic and Hancockin patients with clinically normal valve function.

    Summary: The size of myocardial infarction (MI) was determineted bygramequivalent(gEq) isoenzyme MB creatine kinase(MB-CK) and byECG using Selvester's QRS scoring system in 71 patients with thefirst acute MI(45 patients with anterior and 26 patients withinferior MI).The global left ventricular ejection fraction(GEF)estimated by radionuclide ventriculography was determinated inthe same group of patients in the subacute and chronic phasesafter suffering MI.The relationship between the size of the no statistically significant difference between the size ofanterior and inferior infarction determinedenzymatically(22,54+-13,78 vrs 20,90+-13,76;p=0,59 NS)The size ofMI determined by ECG was significantly larger in the group ofpatients with anterior MI(9,57+-3,36 vrs 4,47+-2,63;p=0,001).Insubacute and chronic phases GEF values were significantly lowerin anterior than in inferior MI.A good negative correlationbetween the size of MI determined by ECG and GEF was found insubacute and chronic phases in anterior and infeior MI.Thecorrelation between the size of MI determined by gEq and GEF wasweaker in anterior MI and poor in inferior MI in bothpostinfarction phases.We concluded that the QRS scoring system isa noninvasive and simple method of determining the size ofMI,especially in inferior infarcts,where enzymatic determinationsare insufficient.

    Summary: In 60 patients myocardial infarction size was determined byelectrocardiogram(ECG) using Selvester's QRS scoring system.Thesevalues were compared with the size of infarction as determinedenzymatically using gramequivalent isoenzyme MB creatinekinase(gEq)and using maximum values of isoenzyme MB-CK.Theresults showed no statistically significant difference betweenthe size of anterior and inferior infarction determined bygEq(25,19+-13,59 vrs 22,48+-14,04;p=0,12 NS) and by maximum MB-CK(125,5+-76,0 vrs 98,4+-60,7;p=0,12 NS).The size of myocardialinfarctions determined by ECG was significantly larger anteriorlycompared with the inferior infarcts(9,6+-2,9 vrs4,5+-2,6;p=0,001).In patients with anterior infarcts goodcorrelations between the sizeof infarction determined by QRSscoring system and by gEq or maximum MB-CK were found(r=0,69;p=0,004 and r=0,72;p=0,001).In patients with inferiorinfarcts the correlations between QRS score and gEq or maximumMB-CK were poor (r=0,37;p=0,02 and r=0,45;p=o,15).The causes ofweak correlations in the results of described methods in inferiorinfarcts are discussed.Thus QRS scoring system provides newnoninvasive and simple possibilities in determing the size ofanterior and in inferior infarctions of the left ventricle.

    Summary: We have investigated the effect of additional stress,caused bybombardments and war operations every day,on the frequency ofacute coronary events among civilians in Croatia. Our investigation was done in part of the Zagreb area,the capitalof Croatia,ans its surroundings in Sepetember,1991,with specialattention focused on the second half of the mont,when the firstair raid sirens were sounded.Our hospital is a tertiary carecentre with 1200 beds (240 beds in the department of intenalmedicine) with a catchment of about 300.000 inhabitants. As Meisel at al. showed,we found a higher frequency of MI duringthe study period than in the previous year(table).Deaths due toMI in patients with or without q-wave also increased,by contrastwith Meisel and colleagues' findings,no rise in deaths fromanterior MI was noted. The increased hospital mortality among patients with acute MIduring September,1991,could been due to the frequent air raidalarms.An additional stress was that lifts could not be used fortransporting patients to the shelters.These factors increased theexisting mental stress,possibly causing regfactory malignantarrhythmia or heart failure. The war in Croatia,ai addition to known casualties,also has itsvictims among patients with coronary artery disease.

    Summary: The intensive use of echocardiography increases the probabilityof correct diagnosis of amyloid cardiomyopathy,but on the otherhand increasesd the risk of erroneously diagnosing cardiacamyloidosis as hypertrophic cardiomyopathy which was the case inthree out of five our patients. In combination with EKG finding of low voltage and positive 99mTcpyrophosphate myocardial uptake,echocardiography is veryuseful,specific and sensitive method in differential diagnosis ofinfiltrative cardiomyopathy,and it is a method of choice in earlydiagnosis of cardiac amyloidosis. Echocardiography is easy to perform and it can be used forlongitudinal folow-up of patients to demonstrate the evolution ofcardiac amyloidosis which is of great importance in makingtherapeutic decisions.

    Summary: Quantitative and qualitative analysis of Q waves and QRS scoreswas performed on 69 patients during the early phase of firstmyocardial infarction (MI) and 6 months subsequently.Theregression of ECG signs of MI were compared with theenzymatically estimated size of MI,the location of MI,and withthe changes of global ejection fraction (GEF) assessed byradionuclide ventriculography.Among 57 patients with Q wave MI acomplete disappearance of ECG signs was found in 9(15.7%)Patients with MI of inferior location showed asignificantly higher reduction of Q waves (p<0.001) and QRSscores (P<0.001) than the anterior MI group.In the group of 12patients with the Q wavw MI,11 demonstrated complete regressionof MI signs.Among all Q wave and non Q wave MIs,the authors foundno significant difference in the size of MI.The median of thepercent of change of the QRS score was significantly higher(p=0.04) in the group of patients with improved GEFs than in thergroup of patients withdecreased or unchanged GEfs 6 monthsfollowing acute MI.The sensitivity,specificity,and predictivevalues for improved left ventricular function according to thechange of Q waves and ECG scores were 91%,32% and 62%;forchangesof Q waves 81%,40%,and 63% for changes of ECG scores91%,36% and 64%,respectively.In the group of patients with non Qwave MI these values were 100%,50% and,91% as a result of St-Tdisappearance.

    Summary: New knoledges of contemporary cardiology have createdpossibilities for critical reevaluation of current criteria inexercise testing and have shown its new diagnostic and prognosticvalue.A multifactorial approach to exercise testing includes ananalysis of ECG criteria and hemodynamic parameters obtainedduring exercise testing.This approach increases the value ofstress testing in diagnosis,assessment of the severity andprognosis of coronary artery disease.The inclination towardarrhythmias,detection of asymptomatic ischemia and earlydetection of a dysfunction of the left ventricle have beenemphasized.The indications for use and termination of exercisetesting absolute and relative contraindications for exercisetesting are critically disdussed.The sensitivity of exercisetesting is 60 to 80 percent and specificity level ranges between85 to 90 percent.The incidence of serious complications isdecreased about 50 percent by a careful screening of patients andappropriate safety level.

    Summary: Tha management of acute myocardial infarction has dramaticallychanged in the last decade.The conservative approuach with bedrest,oxigen therapy,pain relief and prevention ofcomplications-the former cornerstones of the treatment of acutemyocardial infarction-are now being replaced with new,moreaggresive forms of therapy,including early administration ofthrombolytic agents.With the more than 100.000 patients studiedin various clinical trials in the different parts of theworld,the use of thrombolytic therapy is probably the mostthoroughly studied medical intervention in human medicalhistory.The results of these trials undoubtly showed thatthrombolytic therapy had a major impact on reducing mortality andpreserving left ventricular function in patients with acutemyocardial infarction.The purpose of this article is to reviewcurrent new information on thrombogenesis and thrombolysis,theindications and contraindications of thrombolytictherapy,comparison between different thrombolytic agents and therole of adjuvant antithrombolitic therapy.

    Summary: In this study the incidence and the development of acute renalfailure(ARF) in heart transplant recipients is presented.Amongthe thirteen heart transplant recipients eight of them developedoliguric or nonoliguric ARF.Besides the known factors such asactual conditionn ot the patient,kidney function,peri andpostoperative compromised circulation,our results demonstrate thesignificance of postoperative cyclosporin concentration incombination with the use of other drugs.The results also show theimportance ot the way cyclosporin has been administered.theparenteral route of appliance is connected with the greater riskof higher drug concentration in the plasma than the peroralone.In connection with this is the higher incidence od adversereactions to cyclosporin given parenterally.In addition toranitidine and captopril ,which have been mentioned earlier,thefindings of our study indicate that greater attention has to bepaid to the treatment with ketokonazol given in combination withcyclosporin,since it results in decreaseed cyclosporinclearance.Other nephotoxic drugs like amphotericin also increasethe possibility of renal lesions.The incidence of acute renalfailure in this group of patients is high.Our data suggest thatthe reason for the development of ARF are multifactorial.Thesedata further suggest that a reasonable way to sove nonoliguricform of acute renal failure is to maintain the "internal balance"and that it is not neccessary to perform extracorporalčelimination of nitrogen substances,if there are no additionalcomplications(i.e.gastrointestinal bleeding).An early applicationof hemodyalisis in the oliguric form of acute renal failureenables survival in the phase,first of all,by the influence onhyperkalemia and fluid overload.

    Summary: Heart failure is a complex clinical syndrome in whose manifestation and prognosis compensatory mechanisms have a prominent role as a response of the organism to an elementary disturbance.There are five basic compensatory mechanisms:Frank_Starling mechanism,structural changes of the heart,activation of neuroendocrine mechanism,adaptation to hypoxiaand anaerobic metabolism.The interaction between the two main neurohumoral mechanisms,namely vasodilatatuion and vasoconstriction,has been drawing much of the attention recently.Vasoconstriction which evolved into maintaining cardiac output in hypovolemic state,leads to a number of deleterious hemodynam,ic and metabolic disturbancec in the heart failure.The organism tends to diminish this negative effects by changing beta adrenergic pathway and by activating vasodilatative mechanisms.Once heart failure becomes severe,vasoconstriction predominates due to a loss of normal baroreceptor activity.It is considered that too marked activity of neurohumoral mechanisms is a significant cause of disease progression.By use of contemporary drugs(ACE inhibitors,bet blockers,digitalis) excesivve vasoconstrictive mechanisms are tried to be diminished and prognosis of the disease improved.

    Summary: Our study covered five patients,three men and two women,aged 53+-13,suffering from therapeutic refractory heart failkure.Following a comprehensive cardiological diagnostic procedure ishemic dilated cardiomyopathy was diagnosed in four of them,while one of the female patients was found reccurent pulmonary embolisms.All patients underwent right herart catheterization,and on the basis of the obtained hemodynamic parameters,the peroral treatment was either adapted od modifed(digitalis,diuretic,ACE inhibitor).A cardio-selective beta-blocker was introduced in two patients,while in one of tje patients ventriculart electrostimulation was applied to accelerate the heart rate.Đright heart catheterizatin wasrepeated after seven days when the foolowing changes were observed;an increase in cardiac output(38%) and cardiac index(42%),pulmonary capillary pressure(25%) ans systemic vascular resistance(30%).Foolowing the introduction of a "new" therapy,of five patients with NYHA IV stage,three passed to NYHA II,and two to NYHA III.Owing to the progression of the disease,one of the female patients with NYHA II group,undervent heart surgery,with a good result.In conclusion by applying repeated invasive hemodynamic diagnostic procedures it is possoble to treat effectively refractory heart failure by simple oral drug administration.

    Summary: Multiple organ dysfunction syndrome(MODS)(multiple organ failure-MOF) is a new term(definition from ACCS/SCCM Consensus Conference published in Critical Care Medicine 1992. MODS is a frequent complication of systemic inflamatory response syndrome(SIRS),or sepsis-when infection is documented.It is also orecipitated by such events as trauma,haemorrhagic shock,necrotic pancreatitisd and other critical condicions.Etipathogenesis is still obscure,Multiple organ dysfunction syndrome is clinical end stage of systemic hypermetabolicresponse to injury.It represents the leading cause of death in critically ill.These patients spend more days in ICU with higher cost.The hallmark is profound is profound autocatabolism(protein stores) and high energy expenditure.There is global microcirculatory injury with the lungs as the primary target organ,but also with peripheral vascular paralyĐsis,myocardial depression,disruption of protective-gut barrier,and terminally,liver and renal failure.Source control,restoration and maintenance of microcirculation,and metabolic(nutritional) support are the basic principles in therapy of this syndrome.New therapies are exciting but there is still no concensus about benefit and improvement in mortality(antibodies against endotoxin,mediator's antagonists,antiinflamatory agents,selective digestive decontamination and many others.)

    Summary: During the period of air-raid alarms in Zagreb(September 1991) ,the influence of war-induced stress on the incidence and mortality of acute coronary arter disease was investigated.Control periods were September 1989 and September 1990.Among 2903 patients admitted to Emergency Care Units,369(13%) were examined for suspected coronary artery disease.During the same periods in 1989 and 1990,10% and 11% of acute coronary artery disease patients were recorded,respectivelly.The percentage of patients with myocardial infarction or unstable angina,admitted to Coronary Care Units during September 1989,1990 and 1991,was 49%,50% and 55%,respectively.The number of Q myocardial patients admitted during September 1991 was significantly higher than that recordedduring the same period in 1990.The incidence and mortality patterns in acute coronary artery disease patients were also examinedduring August,September and October 1991.The peak incidence of acute coronary artery disease was found in the first half of September,while the peak mortality in these patients eas foundduring the second half of September.During the second half od September of 1989,1990 and 1991,the mortality in Q myocardial patients in Coronary Care Units,was 16,7%,15,2% and 23,8%,respectively.Besides the war-induced stress,transportation of aour patients to shelters or inner parts of the hospital caused additional stress,probably contributing to the development of refractory malignant arrhythmia or heart failure.

    Summary: A 58 year woman was admitted to this hospital because of retrosternal paim followed by dyspnea which developed a few hours prior to admission,and two week history of progressive intolerance of physical effort.Echocardiography was done which revealed enlarged cavity of the right atrium(59x54 mm) and right ventricle(46 mm) of ther heart.Scintigraphy showed numerous triangular lung zones of sharply desceased or completely absent perfusion.After the diagnosis of reccurent pulmonary embolism,the patient was treated with intravenous heparin at a dosage of 25000iu a day for 10 days.Dyspnea settled within 48 hours of starting heparin,analysis of arterial blood gases became normal and the general condition of the patient improved.A repeated echocardiogram showed a significantly reduced dilatation ot the right atrium from 59x45 mm to 47x43 mm and decreased pulmonary hypertension from 110 mm Hg,on admission,to 65 mm Hg.

    Summary: As the therapeutical method,elective cardioversion was used to treat 24 pts with atrial fibrillation,including thiopenthone compared with propofol as induction agents in anesthesia.In course of various time sequences,the effects to respiration frequency ,tidal volume,oxygen saturation and heart rate were observed during DC countershock.Statistical method that has been used was nonparametric Mann-Whitney U-test. Considerin this procedure,the results proved the existing of significant differences between thiopenthone or propofol.Already during injection in the first 30 seconds of thiopentone or propofol,statistically significant differences are noticed in respiration frequency (ll.83+-l,75 vs 14.17+-3.l9,p=0.03);and ventilation "depth"(554.17+-101.04 vs 437.50+-111.04;p=0.03),two minutes after injection similar differences showed in respiration rate(1o.33+-1.23 vs 12.00+-1.78;p=0.02);tidal volume(512.50+-85.61 vs 429.17+-72.17),p=0.03)and oxygen saturation (84.58+-6.4 vs 81.50+-3.53,p=0.03).heart rate considerably is different one minute (lol.25+-18.31 vs 92.83+-43.10;p=0.04) as well as ten minutes after cardioversion (95.92+-21.51 vs 79.92+-31.13;p=0.03). We conclude that thiopenthone depress repiration less than propofol does;so we prefer thiopenthone in practicng cardioversion.

    Summary: Laboratory findings and some therapeutic approach of 94 severe war wounded,treated in Surgical ICU of "Sisters of Mercy" University Hospital in Zagreb were analysed.General data about types and causes of wounded indicate that there are no differences when compared with wounded from other wars(Vietnam,Israel).RBC count,amoong laboratory analyses,was relatively high according to the severity of wounds.The main cause was,probably,that most of wounded had injures caused by shrapnel,low velocity and energy missiles.Tissue destruction caused by shrapnel is less than those cause by bullets.Contrary,huge amounts of blood and fluids given to wounded during resuscitation and operations were necessary.Hypokalemia was marked,unexpected,problem.Additionally,it correlated positively and statistically significant with pH and base excess.It was neurohumoral consequence od shock state.Most severe wounded had reperfusion liver injury.Among coagulation tests,low number of platelets was marked.Dopamin prevention of ARF was of no value.The best ARF prevention is,as soon as possible,infucion of enough amounts of fluid,to establish good tissue perfusion.Prophylactic use of antibiotics was succesful.Stress-ulcer prevention with ranitidin was good,but caused relatively high proportion of pneumonia.Catabolism was high,much higher than in civil trauma.Special care had to be focused on parenteral and enteral nutrition.

    Summary: Predictive value of several hemodynamic parameters of LV functionin five-years survival after AIM were examined in 70 pts.The aimof analysis was to determine the predictive value of globalEF,endsystolic and enddiastolic volume,determined by radionuclidestudies in early phase,as wall as infarct size determined byenzymes and by Selvester QRS scoring system upon five-yearssurvival of these patients.Five years survival curves point that91.4% of pts have survived 1 year,86.6% 2 years,83.6% 3years,80.6% 4 years.Among those who died because of cardiaccauses 50% have died after 18 months.Multiple regression analysisindicates that EF represents by far the most significantpredictive factor among observed variables. The global EF determined by radionuclide studies in subacutephase has,among the tested indicators,the most importantprognostic value in five-years survival after AIM.

    Title: The value of following up the concentration of procollagen III peptide in assessment of the scar formation after acute myocardial infarction

    Summary: The formation of the scar after myocardial infarction(MI) means healing and health.Procollagen III peptide (PIIIP) is a serum marker of the fibroblasts activity,collagen type III secretion and scar formation.The aim of this study was to establish:if the serum PIIIP concentration is raised after MI in the period of maximal fibroblasts proliferation and activity,if the PIIIP concentration is normalized six weeks after MI,if there is a correlation between PIIIP and maximal post infarction creatinphosphokinase(CK) values and if the PIIIP values can be connected to the MI complications that are depending of the scar quality. The study included 40 healthy serum donors,and 41 patients with MI.PIIIP was measured at admission (within 8 hours of the onsetof symptoms),14 days and 6 weeks after the MI with double sandwich test using RIA-gnost set Procollagen-III-Peptide,Behringwerke AG.The difference significance between measured variables was tasted with Student t test,and the correlation with Pearson's correlation coeficient. This results showed that the healthy donors had serum PIIIP concentration within normal limits given by the RIA-gnost set producer.Within 8 hours of the MI onset,all the patients had normal PIIIP.After 14 days,PIIIP was significantly rised compared to the acute phase of MI (p<0.01).In two patients who developed post infarction aneurysm it remained within normal limits.Six weeks after MI,in the patients free of symptoms,PIIIP returned to normal values.In 7 patients who had frequent post infarction stenocardia PIIIP remained eleveted. The results of this study showed that PIIIP is a good marker of post infarction scar quality and quantity.It could be used in the clinical work to discover patients with week capabilities for scar formation after MI and therefor at greater risk to develop postinfarction complications which depend on scar quality.
