    Summary: Dry chemistry has been accepted as an important technology in medical laboratories forr many years. Many evaluations of this technology have been undertaken by reputable laboratories, the results of which were excellent when compared with conventional wet chemistry analysis. This book contains a detailed overview of the current knowledge in the field of dry chemistry and a description of various commercially available dry chemistry systems as Ektachem, Reflotron, Seralyzer, Cobas Ready, Drichem, Opus and Stratus. Since this newly introduced and used dry chemistry methods has a tendency to be a diagnostic tool not only in medical laboratory but also in physician offices, the book underlines the importance of knowing the the bases of analitical, biochemical and pathobiochemical proceses.

    Summary: A crude rat liver extract called hepatopoietin, isolated from regenerating rat livers and from plasma 14 h after partial hepatectomy, induced a two-fold increase in DNA synthesis in hepatocytes in primary culture but an increase only in the S phase of cell cycle. The blood plasma of patients with liver diseases contained a liver growth-promoting factor similar to hepatopoietin. Human plasma hepatopoietin showed the same properties (acid- and heat-stable, Mm 38+5 kD) but was some what more effective than hepatopoietin isolated from rat liver, i.e.2.5-3.5-fold, in inducing proliferation.

    Summary: The aim of this study was to isolate a restrictible DNA, i.e. an adequately pure and intact DNA for Southern blot analysis or analysis of restriction fragments from whole blood samples. The quality of isolated DNA suitable for digestion with restriction enzymes was estimated by digestion of isolated DNA with restrictive endonucleases HIND III, Bam HI and Eco RI, and the restricted fragments were separated electrophoretically in 1% agarose gel. The results suggested that the DNA obtained by the procedure described appears to be essentially pure and suitable for numerous applications, when even stored at -20 C for 6 months.

    Summary: In the course of the investigation of hepatopoietin, we discovered that, in vivo and in vitro, the tripeptide Ser Asp Lys-Ac (SDK-Ac) in minimal amounts inhibits the proliferation of hepatocytes very intensively. The results on Wistar rats (100 +5g), subjected to 2/3 partial hepatectomy, and rats with Yoshida liver tumor, both given an i.p. injection of 1 ug SDK-Ac dissolved in 1 ml saline, showed that the tripeptide exhibited a 47%-57% decrease in the proliferation activity of the remnant liver cells 10 h after partial hepatectomy. The mean amount of ascites and the mean number of Yoshida cells in the ascites were less than 20%.

    Summary: A rat liver containing an active substance, hepatopoietin, isolated from liver remnant 50 h after partial hepatectomy, displays a capacity to stimulate DNA synthesis in hepatocyte primary culture. Further purification and identification of the crude hepatopoietin extract were achieved using Sephadex G-100 chromatography, SDS-PAGE, IEF-PAGE and two-dimensional electrophoresis. Specific antibodies to the crude rat liver hepatopoietin extract were produced by rabbit and tested by Western blot.

    Summary: Human plasma hepatopoietin (hPHP) extract was isolated from 72 liver disease patients and 25 healthy volunteers. Protein bands of hPHP extract were electrophoretically separated bz SDS-PAGE modified so as to allow good separation of proteins between Mr 30.000 and 50.000. The electrophoretic pattern showed three protein bands in this range, i.e. bands A,B and C, with Mr of about 41.700, 38.000, 33.800, respectively. Positive band A was found in 92 %, band B in 97%, and band C in 85% of patients. In the control group, 80% of subjects had positive band A, 88% band B, and 80% band C. When the bands were compared to the results of biochemical parameters commonly used to test liver function, they showed the highest frequency of band A in patients with ALT and LDH values bz 3SD above the mean, whereas the highest frequency of band B was found in patients with TB, AP, AST, GGT, AFP and CHE values by 3 SD exceeding the mean value of the reference interval. Band C was observed to have lowest frequency, regardless of the biochemical parameter evaluated.

    Summary: The Ektachem 500 clinical chemistry analyzer is a discrete random access analyzer performing most common clinical chemistry tests including electrolyte measurement by direct potentiometry. The system evaluated accordimg to the ECCLS protocol, showed that the imprecision, accuracy and linearity of the methods are satisfactory. The results measered by above system corelate very well with those obtained by conventional wet chemistry methods. Besides, the component of dry reagent slide to eliminate the endogenous (hemoglobin, protein and bilirubin) as well egzogenous (therapeutic drugs) interferences, allows the tests to be performed with high analytical precision, accuracy, sensitivity and specificity which is of utmost importace for every day routine and research work.

    Summary: Over the past 20 years, recombinant DNA technology has developed into a potent and sensitive technique for the study of human diseases, not only immensely improving the basic knowledge about molecular disturbance in human diseases, but also ever more influencing the diagnosis of the diseases. A diagnostic procedure using the recombinant DNA technology includes the following steps: isolation of target DNA or RNA and preparation of their probes, molecular hybridization, hybrid detection and interpretation of the results. The methods of DNA and RNA isolation, quality testing of the isolated nucleic acid, molecular hybridization, hybrid detection, and eventually, results interpretation are descriebed. The most frequently used methods of DNA technology and their diagnostic applications are also presented. It is emphasized that succesful use of DNA technology in a clinical laboratory requires well defined protocols, high-quality and reliable procedure, proper education of laboratpry professionals to perform the methods of recombinant DNA technology and proper training of the physicians in the results interpretation.

    Summary: Molecular diagnisis has provided new possibilities in the diagnosis and treatment of diseases from a wide spectrum of medical specialties. Molecular diagnosis is defined as a new field of laboratory medicine using nucleic acids as reagents in the analytical procedure and as a sample which has to be analyzed. Molecular diagnosis techniques are "molecular" techniques, as they rely on the primary structure of the analyte, e.g., linear codes of the bases occurring in the analyte, the on the total amount, size of ploidity, viscosity or base content. The in vitro development of amplification techniques (PCR) has increased sensitivity of the methods using DNA probes by 10 to 10.000 times, above the limits of clinical laboratory techniques used to date.

    Summary: 50 h after partial hepatectomy, the rat remnant liver was homogenized with water and hepatopoietin extracted by acidifying to pH 5.5 and boiling at 95 C. Further purification was done by chromatography on Sephadex G-100 and 4 fractions within the range between 33000 and 43000 D were collected. Bioactivity was tested on primary hepatocyte culture by incorporation of 3H-thymidine into hepatocyte DNA. Results showed: (1) 2 to 3 fold increase in radioactive thymidine incorporation in DNA and 2) partialpurification of rat liver hepatopoietin extract provided hepatopoietin of a higher activity.

    Summary: The proliferative activity of hepatopoietin extract isolated from liver patients (n=29) was tested by inoculation of hepatocyte and measurement of 3H thymidine incorporation into hepatocyte DNA. The mean proliferative activity was 149% (range,96%-217%). Results of flow cytometry revealed the cell cycle proceeding from phase G1 to phase S to differ between hepatocytes inoculated with hepatopoietin (mean %Go/G1=73+2.93, %S=20.8+4.96,and %G2+M=5.07) and those without inoculation (mean%Go/G1=89.5+3.5, %S=10.5+2.8, and %G2+M=1.47+0.36).

    Summary: Hepatopoietin was isolated from rat liver remnant 50 h after partial hepatectomy and further purified by gel filtration on Sephadex G-100. Purified bioactive hepatopoietin fraction was the only fraction containing proteins of about 40000 D and separated on SDS electrophoresis according to Laemmli, produced a band at about 40.000 D. The hepatopoietin extract isolated from blood ofliver patients, when SDS-treated and separated according to its molecular weight, on SDS electrophoresis showed the identical band as that of rat hepatopoietin.

    Summary: Human plasma hepatopoietin (hPHP) extract was isolated from 81 liver patients and its proliferative activity was compared to the crude rat liver hepatopoietin extract isolated 50h after partial hepatectomy. The hPHP extract proliferative activity was 169% (115%-201%), 149.3% (120%-172%) and 196%(152%-246%) for primary and secondary carcinoma, and chronic liver disease, respectively, whereas the proliferative activity of the rat hepatopoietin extract was 100%. Results of DNA flowcytometer analysis showed an increase in cellular DNA content in the S phase of the hepatocyte cell cycle too.

    Summary: Human plasma hepatopoietin (hPHP) extract was isolated from 38 liver patients. The localization of active hepatopoietin substance in crude hPHP was done by SDS-PAGE. Electrophoretic pattern showed three protein bands in the range of Mr 38000+/-5000, i.e. bands A, B, and C, Mr about 41700, 38000, and 33800, respectively. 82% patients had positive band A, 95% bandB, and 63% band C. In total, 97% of patients had one of the protein bands positive. The bands were compared to the results on biochemical parameters commonly used to test the liver function.

    Summary: From sera of 72 patients with liver disease, a crude hepatopoietin extract was isolated and its protein bands were separated by SDS-PAGE. Three protein bands in the range Mr of 38000+/-5000, i.e. A about 41700, B about 38000 and C about 33800, were compared to the results of transaminases, alkalinephosphatase, GGT, CHE, LDH and total bilirubin. The SDS-PAGE bands A, B, and C were found in 87.5%, 95.8% and 79.2% patients, respectively. The frequency of hepatopoietin bands in patients with values of biochemical parameters higher than 3SD of the mean ranged between 70%-100%.

    Summary: This study was undertaken to evaluate the electrophoretically separated hepatopoietin bands in patients with liver cirrhosis and compare them with biochemical markers of the liver state. Human plasma hepatopoietin (hPHP) extract was isolated by the method of Goldberg at all and Topi' et all, from 30 patients with confirmed liver cirrhosis and 25 healthy volunteers. protein bands of the hepatopoietin extract were electrophoretically separated by SDS-PAGE. The SDS-PAGE elpherogram showed three protein bands, i.e. band A, B, and C, Mr of about 41.700, 38.000 and 33.800 respectively, in the range between Mr of 30.000 to 50.000. 92% of patients had positive bands A, 97% band B, and 85% band C. In controls 80% had positive band A, 88% band B, and 80% band C. Compared to the results on biochemical parameters the bands showed the highest frequency of band A in patients with ALT and LDH values by 3 SD above upper reference limit. The highest frequency of band B was found in patients with TB, AP, AST, GGT, AFP and CHE values by 3SD higher than the upper reference limit. The lowest frequency was observed with band C in patients, regardless of the biochemical parameter evaluated.

In the cirrhosis group, 73.3% of patients had positive band A, 93.3% band B, and only 60% band C. In controls, 80% of healthy volunteers had positive band A, 88% band B, and 80% band C. The results suggested impared balans between the factors with proliferative and inhibitory activity.

In emergency laboratory, with a program of analysis covering the needs of internal, pediatric, neurologic, urologic and surgery clinics, we developed a sophisticated computer program, as an expert system allowing clinical decision making on the basis of emergency laboratory findings. In this program, analytes from five categories; electrolytes, metabolities, enyzmes, hematology related tests and parameters of the acido-base balans were combined so that their absolute values in time and their mutual relationships presented panels for making diagnosis of different emergency states caused by cardiac injury, hepatic or kidney injury or states related to glucose metabolism disturbances.

    Summary: A crude liver extract called hepatopoietin, isolated fromregenerating rat livers and from rat plasma 50 h after partialhepatectomy, induced a two-fold increase in DNA synthesis butonly in the S phase of cell cycle, suggesting its proliferativeactivity. The plasma of patients with primary and secondaryhepatocarcinoma and those with chronic liver disease contained aliver growth promoting factor similar to hepatopoietin. Humanplasma hepatopoietin showed the same properties but inducedproliferation more effectively, i.e. 2.5 -3.5-fold, increasing the cell cycle S phase too.

    Summary: The extract of calf liver after partial hepatectomy was analysed, presuming that it should have contained an active substance with hepatopoietin-like action. Biologic activity of the calf hepatopoietin extract (cHTP) was assessed by radioactive (3H-thymidine incorporation into hepatocyte DNA) and non-radioactive method (BrDU incorporation into hepatocyte DNA), which revealed the calf liver to contain an active substance with hepatopoietin-like action. The highest biologic activity of cHTP was shown in concentration of 0.1 ng/mL, which was ascertained by both methods (p>0.05), suggesting the non-radioactive method, as a less risky and adequatelt sensitive method, to be the method of choice for the study of hepatocyte proliferation. The biologic activity of cHTP was also confirmed by the determination of the catalytic activities of the AST, ALT and LDH enzymesin primary hepatoczte culture. cHTP was found to increase the catalytic concentrations of all the three enzymes, among which AST showed a statistically significant increase in the presence of an optimal concentration of cHTP. With the use of SDS-PAGE, cHTP was found to contain three protein bands identical to those observed in rat hepatopoietin in the region of molecular masses of 41.700, 38.000 and 33.800 D.

    Summary: Evaluation on Kodak Ektachem 500 dry chemitry system was performed according to the protocol of the European Committee for Clinical Laboratory Standards (ECCLS Document Vol. 3, No. 2, 1985). The analytical system Ektachem 500 the multi chanel, discrete random access clinical chemistry analyzer was evaluated for imprecision, accuracy, linearity, sensitivity and specificity. Endogenous interferences (hemoglobin, protein lipoprotein and bilirubin) as well as egsogenous interferences (therapeutic drugs) were also considered. Practicability of the Ektachem 500 system in a daily and busy clinical hospital emergency laboratory with a turnover of about 2.000 analyses per 24 h was also assessed. Results statistically evaluated by Student-t test, linear correlation and Passing-Babloch method for comparison of analytical systems, showed that the system is precisse, accure, sensitive and very specific and it is suitable for medical laboratories who needs a rapid workflow a large number of samples.
