Summary: To replace J131-HSA as a marker of plasma volume with Tc99m-HSAwe assesed blood volume on a group of 23 healthy female subjectsaged between 20 to 40. The good correlation between two methodswas established (r=0.961; 10 minutes values). The escape factorfor J131-HSA and Tc99m-HSA was assessed (10 minutes 1.015ń0.0064and 20 minutes 1.254ń0.0895, respectively). The representativeblood sample for Tc99m-HSA is in 20th minute and for J131-HSA in10th minute. We concluded that method with Tc99m-HSA is reliableand reproducible.

Summary: The aim of our prospective study is the functional examination ofthe cardiovascular (CV) system in hemodialysed patients beforeand after anemia correction by Erithropoietin (Epo). We usedechocardiographic studies and radionuclide ventriculography atrest and during exercise. After correction of anemia thesignificant decrease apeared of the initially high cardiac output(CO) and cardiac index (CI) from 7.55ń3.15 to 6.3ń1.8 and from4.2ń1.7 to 3.6ń1.0. Initially raised peripheral resistence index(TPRI) of 2582.3ń2097.3 dyn/sec/mý has not changed significantlyas well as the mean diastolic (DBP) and the mean blood pressure(MBP). There appeared the significant decrease of endodiastolic(EDV), endosystolic (ESV) volume (p<0.005) and of CO (p<0.005).The desrease (n.s) of the mean values of the LVM was significantfor the subgroup of the patients with the initially greather LVM.The ejection (EF) remained unchanged (73.9%). The functionalcapacity of the CV system was increased from 4 metabolicequivalent (MET) (V O2 max. 14.0 ml/min/kg) to 6 MET (V O2 max.20) (p<0.001). The blood volume has not significantly changed. Weare concluding that the hemodynamic effects of the anemiacorrection are favourable. The caution is necessary in followingthe patients with initially lower values of the LVM. The changesof blood volume, blood presure, the rapidity of rise and thetarget values of Hgb and Htc should be individually monmitored toavoid pressure or volume overload of the heart. The defineconclusion on the satisfying effects of the therapy with Epo onthe CV morbidity and mortality should be postponed for the largerprospective studies.

Summary: In 19 hemodialysis patients with severe renal anemia (meanhemoglobin (Hg) 73q6 g/l) left ventrikular function was investigated with radionuclide ventriculography at rest and during exercise twice: a) before recombinant human erythropoiethin therapy (rHuEPO Th) and b) after 3.4q1.4 months when target Hb reached 100 g/l. After treatment significant reduction in cardiac index (CI) at rest (4.99 before vs. 3.81 l/min/mć after, p<0.01), and exercise (7.89 before vs. 6.49 l/min/mć after, p<0.01) was found. The fall in CI was due to reduction in EDV after rHuEPO Th (p<0.05) at rest and exercise and reduction of HR at rest (p<0.05). EF, ESV, blood volume and BP remained unchanged after rHuEPO Th. While PER and PFR did not changed, TPFR was significantly prolonged after Th at rest and exercise (p<0.05). The patients showed a significant rise in thair maximal work load (52%) and exercise duration (53%) after Th, although changes in maximal HR and BP did not achieved statistical significance under Th. No significant correlation was found between the duration of hemodialysis, increase in Hb and improvement in cardiac function and exercise capacity. We conclude that correction of anemia by rHuEPO improved cardiac function and increase exercise capacity in hemodialysis patients.

Summary: In this investigation involved some cardiac and non-cfardiacparameters at rest and after exercise in a selected group of 35women patients whose age varied from 20 to 40 years and whosuffered from the floride Grave's disease. The control exemines(N=35) corresponded with the diseased group in age and sex. Whileat rest the hyperthyroid group shows no significant increase ofCO with unchanged SV which indicates that the increased CO is tothe largest extent the result of the increased HR. This can beexplain by the increased sympatic stimulation. The increasedsympatic stimulation also leads to the increased PRA. Also the EFis increased as results of the increased contractility inhyperthyroid group. The exercise was done by means of bikeergometer in duration of 10 minutes with the intensity od 50 to75 W in the supine position. ERPF value in the hyperthyroid groupwas higher than it was with the control group (but its ratio doesnot increase in relation to the total CO). It showed nosignificant changes after the exercise in both tested groups. Both groups showed an increase in EF, HR and CO after exercise, but all these parameters were even more pronounced in the controlgroup. The healthy group had also a significant increase in SV.The PRA in the hyperthyroid group was higher in rest with atendency of increasing in exercise. The application of the betaadrenergic blocators reduces significantly the PRA both in restand in exercise. This makes us conclude that the increased valueof the PRA is the effect of the increased activity of thesympathic system in hyperthyroidism. The systolic blood pressure, the pulse pressure and the double product showed significantdifferences in the both groups as regards their metabolic statesas well as after exercise, whereas it increased significantly inthe control group. There was no significant difference in rest inproportion of the ERPF/MV between the observed groups, but afterexercise the difference was significant, and it was greater incontrol group. No interdependence was faund betweenthe PRA and the ERPF in the diseased group. The results of theinvestigations show that some parameters of cardiovascular systemof the diseased group had a poor reaction in rest but thay werepronounced after exercise.
