Summary: Obesity is a very frequent occurrence with unavoidable
consequences on health,social and psychological conditions etc.
Morbid obesity, i.e., surplus of 40% of normal weight in cases of failed
conservative treatmens, besides certain criteria, can be an indication for
surgical treatment. Surgical treatment includes the following:
1.Malabsorption procedures. Various methods of jejunoileal bypass shorten
the passing andd reduce the resorption of food with consequent weight loss.
These methods have been mainly abanoned because of possible liver daamage.
2.Gastric restrictive procedures. Vertical and horizontal gastroplasty and
gastrointestinal bypass reduce possible food intake and cause weight loss.
These treatmens are effective and without serious complications. Vertical
banded gastroplasty is frequently applied. The main goal of the very
treatment is to create a small permanent stomach reservoir and a properly
measured baanded outlet (passing of ingesta).
3.Biliopancreatic diversion. This is aa combined method in which food input
is reduced, because the distal stomach is put out of function. Fats and
carbohydrates are decreaased because of reduced contacts with bile and
pancreatic juices.
4.Various additional treatments (adipectomy, liposuction, abdominoplasty
etc.).
Considering a great number of surgically treated cases, his own experience
and numerous literaature references, the author considers the perfomance of
determined surgical treatmens in indicated cases of morbid obesity,
appliable and justified.
