    Summary: During the first two years of the three years long prospective study in the Istra and Primorje-Gorski Kotar counties, Croatia, 10627 newborns were born. Prenatal screening for hepatitis B (HB) infection was carried out in all pregnant women, and 107 (1%) of them were HB surface antigen (HBsAg) positive. 106 children from HBsAg positive mothers were given appropriate intramuscular dose of hepatitis B immunoglobuline (HBIG) and 20 Šg recombinant hepatitis B virus (rHBV) vaccine in the maternity ward. The second and the third dose of rHBV vaccine were given 1 and 6 months after the first one. On the first control 1 month after birth all controlled children (94 from 106 passive and active vaccinated) were HBsAg negative and all but one anti-HBs positive; only at six months of age this anti-HBs negative boy has been anti-HBs positive at preventive level of 11 IU/L anti-HBs. After the vaccination in maternity ward 12 children (11.3%) did not come to control for various reasons. From cohort of 106 passive and active vaccinated children in the maternity ward 64 (60.4%) were completely vaccinated three times and all of them were at the age of 6 months HBsAg negative and anti-HBs positive. Geometric mean titre (GMT) of anti-HBs were in 51 children below 1000 IU/L (11 to 857 IU/L), in 9 above 1000 IU/L and in 4 we do not have quantitative results of anti-HBs positive serums; the anti-HBs titres did nit significantly differ (p>0.05) between breast-fed and bottle-fed children. The side effects have been observed 9 times during the 369 injections (2.4%); they were one day elevated temperature to 38.2ř C and/or transient mild inflammation at the injection site. The historic risk factors for HBV infection, as defined by USA Centers for Disease Control were found in 43% HBsAg positive women and that confirm the need for screening of all pregnant women for HBsAg.

    Summary: A prospective study was performed to determine whether the risk factors for hepatitis B, proposed by Centers for Disease Control (CDC), USA, are reliable predictors for the hepatitis B surface antigen (HBsAg) carrier state in an obstetric population. In the period between January 1, 1991 and December 31, 1992, all pregnant women from geographically defined areas of the Istra and Primorje-Gorski Kotar counties, Croatia, were routinely screened for hepatitis B surface antigen (HBsAg). Among 10,627 pregnant women, 107 (1%) HBsAg positive cases were registered. History risk factors recommended by CDC were recorded in 46 (43%) out of 107 HBsAg postitive women. The other 61 subjects had no recognizable risk factors. The screening of pregnant women for HBsAg only on the basis of the CDC recommended history guidelines, would have left 57% of our HBsAg positive mothers undetected and therefore their children unvaccinated against hepatitis B infection. Our results confirmed the need and value of the new CDC recommendations about routine prenatal screening of all pregnant women for HBsAg until hepatitis B vaccine is included in the scheme of compulsory vaccination of all newborns.

    Summary: During 1991 and 1992 all pregnant women were sistematically screened for hepatitis B infection in the district of Istria and Rijeka. Among 10627 pregnant women, 107 (1%) hepatitis B surface antigen (HBsAg) positive were registered. Checking for reliability for historic risk factors recommended by Centers for diseases control (CDC) USA, they were found in 46 out of 107 (43%) pregnant women. If HBsAg positive pregnant women were screened following only the CDC recommendations, more than 50% of them would have not been discovered, and their children would have not been vaccinated. Our data support the recommendation for routine HBsAg testing of all women during their pregnancy.

    Summary: During a 3-year long prospective study between January 1, 1991 and December 31, 1993 all pregnat women were screened for hepatitis B surface antigen (HBsAg) in the counties of Istria and Primorje-Gorski Kotar. Among 15558 women 134 (0.86%) were HBsAg positive. In 1991 out of 5396 tested pregnant women (0.9%) were HBsAg positive, in 1992 out of 5231, 57 (1.1%) were HBsAg positive and in 1993 out of 4931 tested pregnant women 27 (0.55%) were HBsAg positive. Newborns of HBsAg positive mothers were passively and actively vaccinated in the maternity ward. All but three newborns (from 134 HBsAg positive mothers) received an appropriate dose of hepatitis B immunoglobuline (HBIG) and all 134 received 20 Šg recombinant hepatitis virus (rHBV) vaccine. 51.2% of the newborns received HBIG 12 hours after delivery and 93.9% within 48 hours. Low percentage of the historic risk factors for hepatitis B infection among our HBsAg positive pregnant women confirms the need for screening of all pregnant women for HBsAg until the vaccination (hepatitis B vaccine) of all newborns should become a part of the standard battery of pediatric immunisations. Yearly prevalence of 0.55% to 1.1% HBsAg positive pregnant women in investigated counties corresponds positively to the also actual "cost-effectiveness" question of routine screening of all pregnant women and vaccination of the newborns of HBsAg positive mothers.

    Summary: A comprehensive programme was conducted to reduce the incidence of hepatitis B viirus infection and its seqel in infants born of hepatitis B surface antigen-positive (HBsAg) mothers. In the prospective study during the period between 1 January 1991 and 30 September 1992, all pregnant women were screened for HBsAg in the counties Primorje-Gorski kotar and Istra, Croatia. Durimg the observed period 9203 pregnant women gave birth to 9286 newborns. There were 94 HBsAg-positive cases (1.02%) among mothers and they gave birth to 95 newborns. All newborns of HBsAg-positive mothers received passive and active immunoprophylaxis in the maternity ward. Interviewing the HBsAg-positive mothers, the physicians obtained information regarding the presence of any of the Centers for Disease Control risk factors for hepatitis B virus carrier state. Historic risk factors were identified in 35 (37.23%) HBsAg -positive pregnant women. That confirms the need for screening of all pregnant women for HBsAg until universal hepatitis B immunisation of infants would be accepted.
