Summary: A total number of 4000 patients older than 6 years of age
with the most frequent diagnosis of acute respiratory catarrh (2/3) and
of pneumonia (1/3) were observed and treated during the period from 1991
to 1994. The intention was to make the clinical diagnosis based on the
laboratory methods which are considered to be the most important for
making diagnosis and for treatment. In patients with respiratory catarrh
(in 2/3-3/4 of them) the proved agent was some of respiratory viruses
(adeno, parainfluenza, RS, enterovirus) and in patients with pneumonia
the causative agent was proved in 47% of them (Mycoplasma pneumoniae
15.6%, Chlamidia psittaci 11.7% Streptococcus pneumoniae 9.6%, Coxiella
burnetti 5.6%). The incidence of atypical (nonbacterial) pneumonias was
2.3 times higher than the incidence of typical (bacterial) pneumonias. On
the basis of some clinical parameters (cough, catarrh of respiratory
tract, pain in the chest), on leukocyte count in the peripheral blood, and
on the basis of C reactive proteins (CRP) values we could classify 90% of
pneumonias as bacterial and nonbacterial ones respectively. This is
important for making the choice of antibiotic before the causative agent
is proved or (even more frequently) when the etiologic agent is not
possible to be proved (about 1/2 of treated patients with pneumonia).
Research goals: The goal of observation was to determine the
clinical and laboratory parameters which could reveal the nature of the
disease specially when the causative agent was not proved.
