    Summary: A newborn with teratogenetic effects of valproat is reported. Theinfant had minor craniofacial and sceletal defects and a closedneural tube defect with teratoma. The use of valproat duringpregnancy is mostly associated with open neural tube defects.This fact is important for counselling women on valproat therapyduring pregnancy, bacause prenatal ultrasaund andalfa-fetoprotein screening can fail to detect this abnormalities.

    Summary: We are reporting two healthy newborns who acquired staphylococcalosteomyelitis of the calcaneus and calcaneus and talus,respectively, due to infected heel puncture or due to directinoculation to the calcaneus by periostal penetration duringcapillary blood sampling for neonatal screening. Description of the proper technique of the heel puncture and necessaryprecautions are given.

    Summary: In 1989 we examined current patterns of the prophylaxis of thehemorrhagic disease of the newborn (HDN) in 15 matarnityhospitals with 34232 deliveries this year(78,9% of total numberof deliveries in Croatia). Selected prophylaxis were given in 10maternity hospitals ( 25399 newborns, 74,2%) and only in 5 (8833newborns, 25,8%) to all newborns. The rate of unselectiveprophylaxis of HDN in developed countries is much higher(70-99%), and in Croatia we should extend this preventivemeassure to all newborns.

    Summary: In the light of current controversial oppinions on the lumbarpuncture (LP) in neonates, the frequency and usefulness of thefirst diagnostic LP for the diagnosis od meningitis andintracranial hemorrhage in critically ill newborns wereinvestigated. Eightyfive neonates were examined . The firstdiagnostic LP was performed in 26% of all hospitalised sicknewborns., in the same proportion in newborns <2500 g and >=2500g. The cerebrospinal fluid was normal in 58,82% LP. Meningitiswas diagnosed in 8,23% and in 32,94% samples there were >1000/mm3erythrocytes and were difficult and inaccurate forinterpretation. One in 11 LP gave a diagnosis of meningitis. The frequency ofpurulent meningitis in all sick newborns was 1,5% and 8,2% innewborns with sepsis. In 28 newborns with >1000/mm3 erythorocytes in the CSF, in 32,14the diagnosis of the primary subarachnoidal bleeding was set, in39,29% the diagnosis of intraventricular hemorrhage and in 25% itwas not posible to to make differencial diagnosis betweentraumatic tape and primary subarachnoidal hemorrhage. The use of LP as an invesive diagnostic method could be lessendin neonates by a brooader and timely application of theultrasaound brain scan and the finding of betterclinical/laboratory signs of sepsis and meningitis.

    Summary: The incidence, the mode and results of treatment of congenitalanomalies of digestive system in newborns are explored. Fromcritically ill patients in five-years period 4,14%(61) had majorcongenital anomalies of digestive system. Neonatal mortality ofthis patients was 16,% and postneonatal hospital mortality was6,5%.Most common malformation was tracheoesophageal fistula(23,1%), then gastroshisis (21,3%), then anorektal anomalies(13,6%), intestinal malrotation (12,1%), omphalocele (7,6%) andother anomalise less then 5%.91% of pacients underwent surgicaltreatment, 35% were mechanicaly ventilated and 57% have hadcomplete parenterale nutrition.

    Summary: We analyse 81 tracheal aspirate of 44 newborns ventilated first7 day of life. 25% od patients had radiological evidence ofpneumonia but 73% of aspirates were bacteriologically positive.The high rate of colonisation of endotracheal tubus duringventilation make serial cultures of endotracheal tube aspiratesdiagnostic tool of limited value in determining the aethiology ofpneumonia.

    Summary: In one of 9 patients with haematogenic osteomyelitis the standardblood culture was positiv on bacteria (Enterobacter sp.) Thespecial culture on genital mycoplasmas give positive result in aELBW infant (900 g) with clinical and radilogical signs ofosteomyelitis. We propose to use the special cultivation ongenital mycoplasmas in newborns with osteomyelitis and negativestandard blood culture.

    Summary: Sick and premature neonates are at increased risk for not beingscreened because of miscommunication between of maternity wardand NICU, feeding status or interference of specific treatmentsand medications. In 1992 in our III level NICU 28,70% ofhospitalised sick newborns escaped from screening. 38,70% of themdied befor the 7th day of life and other 43,55% suffered fromcongenital malformations. Among sick newborns who underwentneonatal screening during the same period only 4,57% died andonly 17,64% had congenital anomalies. We consider early death and congenital malformations as anadditional risk from dropping out from the screening program insick neonates. To avoid it, we urge to adopt the recommendationswith fixed time (24h<7 days) for sample collection regardless offeeding status or antibiotic treatment.

    Summary: 118 neonates suspected for sepsis/meningitis had obtained specialblood cultures for Mycoplasma/Ureaplasma. Twentyone (17,8)infants harboured U.urealyticum (9o,5%) and M. hominis(9,5%).They had sepsis, meningitis, acute or chronic pulmonarydisease,osteomyelitis, foetal hydrops and necrotisingenterocolitis. We conclude that infections with genitalmycoplasmas are common in our critically ill newborns and shouldbe considered as a part of their routine evaluation.

    Summary: The incidence of bacterial sepsis in Neonatal Intensive Care Unit (NICU) was estimated by retrospective analysis of 631 patientrecords during 1991 and 1992. Sepsis was diagnosed in 12,6% patients. Distribution of the incidence showed higher risk of septicemia in premature newborns and/or in infants who underwent aggressive procedures. 71% had one, 28% two and 1% more than three episodes of sepsis. Earlu onset septicemia (first 72 hours) was noted in 43% and late onset in 57% of patients. 47% episodes were monobacterial, 5% polymicrobial and in 48% no pathologic agens was isolated. The most frequent causative agens of early onset sepsis were beta haemolytic Streptococci and S. epidermidis, while Klebsiella pneumoniae, S. aureus, Enterobacter, E.coli and S.epidermidis mainly caused late onset infections.

    Summary: 236 neonates suspected for sepsis/meningitis had obtained special blood and/or cerebrospinal fluid(CFS) cultures for Mycoplasma hominis/Ureaplsma urealyticum. 14,4% harboured U urealyticum and 3, 8% M.hominis in their blood and/or CSF. Three infants out of 34 with positive U.Urealyticum in their blood have positive tracheal tube aspirat and radiologic and clinical signs of acute or chronic pulmonary disease. 34% newborns with positive blood culture have U.Urealyticum or U.urealyticum and M.hominis in their CSF. Six of this 11 infants have cytological and clinical signs of meningitis, other five were asymptomatic. In two prematures, after exclusion of other concomitant bacterial or fungal infections, we considered the diagnosis of osteomyelitis on the ground of U.Urealyticum in blood and other specific clinical and radiologic changes. One infant had fetal hydrops with bilateral hylothorax and other, with M.hominis in blood culture, necrotising enterocolitis with intestinal perforation. Remaining 16 infants with mycoplasmas in blood culture were septic, with different organs involment, but we could not ascribe that to this micro-organisms with certainty. We conclude that infections with genital mycoplasmas, especially with U.Urealyticum is common in our critically ill newborns and should be considered as a part of their routine evaluation.

    Summary: In a serie of 250 patients with main clinical symptoms (coarse facial features, visceromegaly and/or mental retardation) , nine diagnosis were made using the methods of urin screening (MPS total excretion and electrophoresis, TLC of oligosaccharides) and by determination of the catalytic concentration of lysosomal enzymes in leukocytes and in skin fibroblast culture. Seven patients have some of mucoplolysacharidoses and two of sphingolipidoses. Thin Layer chromatography of serum aminoacids was performed in 507 hospitalized children. One or more amino acids (most commonly glycin and alanin) were found to be increased in 46 of these children. Two patients of tyrosinemia typ 1 and 5 cases of phenylketonuria were identified.
