Summary: Peripheral nerve injuries still present significant problem
ofdiagnosis and management. Today's wartime experiences show thatthe most
common peripheral nerve injuries are those caused by lowand high-energy
missiles. For optimal treatment of this type ofinjuries, it is necessary to
define the degree of injury.Acceptance of microsurgical techniques and
opticalmagnifications, using a microscope or magnification loops hasenabled
treatment of injured nerves by various surgicalprocedures. The timing of
the nerve repair is critical indetermining the functional outcome, and it

Summary: Fascia has a well vascularized surface, and when it is
coverdwith a spli skin graft, it provides the thinnest possible flap.The
authors present their own experience with the use of theforearm
septofascial flap in 23 patients. A free septofascialflap was used in 15
patients and an island flap in 8 patients.Seven days later, only 25% of
patients had complete take of thesplit skin graft, while in 60% of cases
there was only partialtake of the graft. The forearm septofascial flap
proved to be agood and reliable method of reconstruction in those parts of
thebody where thin cover was required. Constant anatomy and
minimalpostoperative complications are great advantages of the
forearmseptofascial flap when compared with other fascial flap.

Summary: The development of the microsurgical technique was
closelyfollowed by better reconstructive procedures. Extensive resectionof
the tumors in the head and neck area faces us with thefollowing problems :
reconstruction of skin and oral or nasallining and reconstruction of bone
defects. Free flaps give us theopportunity to perform it as a one-stage
procedure. Radialosteocurtaneous free flap is an extremely versatile and
effectivemethod for reconstruction of composite defects. By folding it
wecreate a "double-face" flap which we use for a three
dimensionalreconstruction.

Summary: In this article the authors emphasize that the knowledge
interminal ballistics is essential for understanding thepathophysiology of
war wounds. They present their own experiencesin treatmenrt of high-energy
war wounds in 75 patients, treatedin the Institute of Plastic and
Reconstructive Surgery. Patientswere dividie in three groups with regard to
the time ofdefinitive reconstruction, using local or free
microvascularflaps. About 12% of patients underwent flap reconstruction
inacute phase, associated with low complication rate and theshortest
hospital stay. The group II comprised 18% of patientsand considering the
number of complications presented the mostunfavorable time for
reconstruction. Flap reconstruction inchronic phase resulted in substantial
prolongation of thehospital stay in 82% of patients.

Summary: We present our experience in the treatment of war wounds in
174patients treated in the Institute of Plastic and ReconstructiveSurgery,
Department of Surgery, Clinical Hospital Center inZagreb. The wounds were
divided into four categories depending onthe type of injury and the
extension of the soft tissue defect.Patients were placed in one of two
groups depending on theirprimary treatment and time of definitive
reconstruction. Group A comprised 79 patients who were initially treated by
plasticsurgeons and whose reconstructive procedure was done within 5days.
Sixty-nine (87%) of the patients in group A had only one ortwo debridements
before definitive closure and stayed in hospital20 days or less. In group B
, 59 (62%) of the patients requiredthree or more debridements and remained
in hospital more than 21days ( p< 0,001 ). Proper primary treatment and
earlyreconstruction result in significantly shorter duration ofhospital
stay and lead to more effective rehabilitation and recovery of patients.
