    Summary: Etiology of the OLP has not been cleared up completely untiltoday,although this disease is considered to be a mucocutaneautoimune disturbance.Investigation of lymphocyte activity in thepersons with OLP has been carried out to the connection betweenlymphocyte activity and freqency of disease.It has been foundthat local immune response shows the cell mediatet immunity inpathogenesis of the OLP.This investigation has been carried outin perifery blood of patients of both sexes,28 females and 2males.T cell subpopulations CD 4 and CD 8 as welB lymphocyteswere includet by the method of Jacklson and Warner.thefunctionalability of Lymphocytes is examined by migrationinhibition, blastic transformation by the method of Sabioncelloand Dekaris.Most ofpatients showed decreasing values of Tlymphocytes.The values of B lymphocytes have not been changedsignificantly.The values of subpopulation of T lymphocytes showeddecrease from the standard values.Functional abilkity testes oflymphocytes show the incrased activity of inhibition migration in OLP.

    Summary: The use of a free gingival transplant from a donor site on the palate has shown satysfactory results in the healing abdtreatment of gingival recession.Depending on the size of themucosal grafts, a further problem arises concerning post-surgical wound protection of donor areas.Generally, there is anununified doctrine wether or not a post - surgical dressiong isnecessary. Stdied were 29 patients from whom free autogenous masticatorymucosal grafts were taken.Just before the surgicalprocedure,polyvilil stens were fabricated on diagnosis models.l5of the patients had the donor site treated with Solcoserylcomposite adhesiv paste.All of oatients had their wounds measuredwith a graduated probe on 3rd,6rd asnd l2 th day followingsurgery.At these periods no significant difference was noticed inthe rate of healing in the group treated with Solcoseryl wichconcludet that the mehanical effect of the transparent vinyl sentis sufficient in the protection from physical trauma and thermalirritation.

    Summary: Thirtythree(33)specimens from lesions of oral lichen ruber (OLR) were studied by histopathologic analysis in order to examine tendency of some histopathologic changes in oral epithelium and lamina propria in various clinical forms of OLR related to the clinical degree of keratinisation. Results have shown that various clinical types had different microscopical features with different tendency of frequency in various clinical degree of keratinisation.In erosive form of OLR increased frequency of diskeratosis(60%), hyperplasia and vacuolar degeneration of basal cells (40%) with increased lymphocyte infiltration (80%) deper in lamina propria was found. In reticular OLR increased frequency of hyperkeratosis (75%),vacuolar degeneration of basal cells and lymphocyte infiltration (60%) was found. In plaque form of OLR main feature was hyperkeratosis(100%) with increased macrophage infiltration (50%) and connective tissue induration(60%). These findings can be important factors in showing tendency of disease activity of resolution.

    Summary: Acase of a 53-year-old man who had suffered from bilateral pain less parotid gland swelling and hypersalivation during the past 5 years is presented.Needle aspiration biopsy of the right parotid gland showed features of a typical benign lymphoepithelial lesion(BLL).No features of a systemic disease were found. A diagnosis of Mikulicz's disease was made and a protocol of close long-term follow-up of the patient designed.A variety of etiologic causes in this rare condition and a diagnostic approach to the patient with chronic parotid swelling are presented.Immunologic association between BLL,Sjegren's syndrome and evolving malignant lymphoma is also discussed.Advances in the immunohstochemical technique and Southern's blot hybridization technique have allowed an early detection of monoclonal B-cell proliferation,i.e. malignant B-cell lymphoma,in the salivary gland aspirate.

    Summary: Reffering to the existing literature and American studies,the authors offer in this paper a survey of the most frequent unexpected emergency cases which may occur during the dental treatment of the patient,disregarding the incidents which may be caused by carelessness of some of the members of the dental team.The authors wish to emphasize the importance of a detailed anamnesis of each individual patient so that patient's possible belonging to same risk groups can be well established and constant readiness of the team maintaned.Also,the authors offer a survey of therapeutic procedures and drugs used in the case an anaphilactic reaction should occur,in order to apply instantaneous proper treatment and avoid panic,improvizing or loss of precious time.

    Summary: Oral lichen planus appears in different clinical forms,but,in spite of that,its diagnosis does not represent a problem since OLP comes with the characteristic clinical features.Aclinical diagnosis must be followed by laboratory tests,in order to rule out the possibility of beta-laczam antibiotics and corticosteroids applied locally. In WHO findingws OLP is listed as a precancerous lesion and,as such,must be monitored continuously. The case reported of in this article confirms a modern approach in the diagnostic and therapeutic procedures in OLP.

    Summary: Both literature data and everyday casuistics warn of harmful thermic,chemical and mechanical effects of tobacco smoking on health.Tobacco-caused changes involve the entire mucous menbrane of the respiratory tract and partially that of the digestive system. The aim of the study was to investigate the smoking-induced changes in hard and soft tissues of the oral cavity.The study included 50 long-term cigarette smokers of both sexs and 100 age-matched non-smokers as controls.Nine parameters relevant for the evaluation of dentition and oral cavity mucosa were assessed. Results showed the changes in hard(discoloration,pigmentation,blurry appearance,enamelerosion and cementumalloplastic fillings)and soft tissues(gingiva,periodontal structures) to be significantly more frequently found in smokers than in non-smokers.Poor hygiene as an additional factor,subsequent reduction of pH and frequent xerostomia,make an excellent basis for the development of dental caries.Besides the effects described above,tobacco smoking also indirectly suppresses the immune system of smokers,induces changes in the oral flora,makes a smoker more susceptible to infections and significantly increases the risk of oral carcinoma. This and other similar studies therefore suggest the need of in- depth consideration of appropriate precautionary measures(education,giving uo smoking)and posibilities to eliminate the present damages.

    Summary: A twofold random study with a placebo control was conducted.The test incolved 40 patients with acute dental pain which continued after an operation or arose as a result of an operation.20 patients were administered 50 mg of diclofenac potassium three times a day fo two days,while the other 20 patients were given placebo in same dosage.The reserve analgesic was paracetamol.The analgesic effect of the first dose(for a time interval)was measured with the aid of a 100 mm visual-analoguosscale of pain(VAS)and the subjective opinion of the patients(a four-degree scale).The results show a statistically significant reduction of pain intensity measured with the aid of the VAS in the diclofenac potassium group in contrast to the placebo.The analgesic effect of the diclofenac potassium set in after 15 minutes;after an hour,the pain was reduced six times in contrast with the initial value.All patients in the diclofenac patassiumgroup were satisfied with the theraoy and confirmed they would take the medicine again.Doctors' evaluations of the therapeutic worth of diclofenac potassium were positive in 90%of the cases as compared to 10%in the placebo group.Diclofenac potassium proved to be a very good analgesic for the treatment of acute dental pain due to its quick effect and lasting pain relief.

    Summary: w traditional concept of five veneral diseases has been extended to a list of 15 diseases.Each of these diseases is described(etiology,clinical picture and differential diagnosis).Over the past 30 years,the proportion of sexually transmitted diseases in theoverall morbidity has dramatically increased.These diseases can also have oral symptomatology.As a dental doctor may thus be one of those who will first suspect the possible disease,he should be familiar with the diagnosis and differential diagnosis of the sexually transmitted disease.He is also exposed to a professional risk of infection by a non-veneral route.The importance of proper education,preventive measures,health standards and health professional conscience for self-protection and prevention of disease teanssmision to other patients is emphasized.

    Summary: Effects of the Ga-As soft laser on clinically oral-isolated Streptococcus mutans and on temperature changes of the finger pad and ear lobe skin and tongue mucosa were studied.Bacterial suspensions of S.mutans were placed on solid blood agar plates.The center of the plates was subjected to 3-min laser treatment every 24 h during a six-day period.Then the bacteria from the target area were maintaned on a new plate and examined for survived bacteria.The use of the laser on the agar plate surface produced no bacterial or bacteriostatic effect.Additionally,temperature changes of the ear lobe skin and finger pad skin and tongue mucosa during and after laser treatment were not significantly different from the control values.

    Summary: The total number of T(CD2+)and B(CD20+)-lymphocytes asa ewll as T lymphocyte subpopulations CD4(helper)and CD8(suppressor)were examined in peripheral blood of patients with RAU,to determine their role in the onset of this disease.The study was carried out in two groups:a group of patients with acute stage of RAU and a control group of healthy subjects.Lymphocyte values weredetermined in peripheral blood by the method of flow cytometry.The obtained rsults showed decreased B-lymphocyte values in peripheral blood in patients wuth OAU in relation to healthy subjects.However,T-lymphocytes showed increased values in 1/3 of the patients,which is statistically isignificant.CD4 lymphocyte values were also increased and and statistically insignificant,while CD8 lymphocyte values were significantly decreased in patients with RAU inrelation to healthy subjects.

    Summary: Thirty-three biopsy specimens from lesions of oral lichen planus(OLP)and 18 from leukoplakia(LPL)were examined for the study of acid phosphatase activity in subepizelial mononuclear cell infiltrare(MCI).Results showed a high level of activity in all mononuclear cells of OLP in inflamation as well as in inflamation,while active dendritic macrophages were not found.Acid phosphatase activity was expressed in all mononuclear cells of OLP,distributed sub-basally and in focal accumulations deeper in lamina propria,depending on the evolutional stage of the disease,which was not observed in LPL.Interesting was the finding of active dendritic macrophages in moderate and severe hyperkeratosis in LPL,contrary to the finding in OLP.This suggested different pathogenesis of hyperkeratotic process in the two diseases.Different distribution and hystoenzymatic activity in mononuclear cells of the examined lesions provided a more detailed insight into their role and differences in the pathogenesis of OLP and LPL.

    Summary: Thirty-three biopsy samples of oral lichen planus(OLP)and 18 of leukoplakia(LPL)were examined by enzyme histochemistry to determine acid phosphatase activity in epithelial cells invarious clinical stages of inflamation and hyperkeratosis in the zwo diseases.Significantly more activated epithelial cells,as indicated by their intense reaction to acid phosphatase(AP),were found in inflamation in OLP than in LPL(p=0.02).In severe inflamation,a decreased activity of acid phosphatase was observed in epithelial cells from both OLP and LPL samples.The AP activity reaction was most pronounced in comeal and granular epithelial cells,especially in LPL,while it was negative in basal cells of both diseases.The frequency of AP-marked cells in various clinical stages of hyperkeratosis in OLP was alding in LPL.These findings suggested different epithelial cell metabolism in thesame pathologic process,inflammation or hyperkeratosis,of clinically similar diseases,which may have a diagnostic and prognostic value.

    Summary: Oralni lichen planus(OLP)je mukokutana autoimuna bolest u čijoj se etiologijispominju metabolički poremečaji,naročito poremečaji metabolizma ugljikohidrata,kao i nasljeđe vezano za antigene tkivne snošljivosti(HLA). Ispitani uzorak činile su 53 osobe oboljele od OLP kojim asu određeni HLA antigeni i nivo šečera u krvi,dok su kontrolnu grupu činile 1089 osobe koje su se podvrgle određivanju HLA antigena u svrhu dokazivanja očinstva. Rezultati su pokazali da je 41,5%svih ispitanika sa OLP-om imalo HLA DR3 i/ili 4 antigene što je znatno više od 38,o2%kolika je normalna učestalost pojave tih antigena u Europljana,odnosno od19% koliko iznosi normalna vrijednost kod zdravih ispitanika u Hrvatskoj. Postotak od 22,64 svih testiranih osoba sa OLP-om i sa HLA DR3 i/ili 4 antigenima imalo je i poremečaj metabolizma ugljikohidrata,a 7,5% ih je imalo o inzulinu ovisnu šečernu bolest tip l.Učestalost pojave te bolesti u općoj populaciji je 0,05%. Među ostalim HLA DR tipovima(ne 3 i/ili 4)također je bilo OLP ispitanika sa poremećajem metabolizma ugljkohidrata,što čini 37,73%svih ispitanika s OLP-om. Iz toga možemo zaključiti da poremećaj metabolizma ugljikohidrata zajedno sa HLA DR 3 i/ili4 antigenima igra važnu ulogu ako ne i odlučujuću u razvoju OLP-a.

    Summary: From 1991 the Republic of Croatia is faced with the problem of retaning its integrity and newly acquired freedom and therefore between 1991 and 1993 had to take care of an ever growing number of injured soldiers who were defending their homeland. Within the framework of the Surgical Clinic as well as Maxillofacial Surgery Clinic of the Hospital Clinic in Zagreb a number of 3865 injured soldiers was treated. The objective of this study was to present oral region microbiological findings as well as the manner of treatment of infections of difficult septic conditions caused by war injures in 50 injured patients.By close monitoring of the infection of the wound as well as team work of physicians and microbiologists we managed to clear the infection successfully. We applied the therapy according to NATO Mnual Instructions.We found that vancomycin is the most successful antibacterial therapy in patients with methicillin resistant Staphylococcal infection. The treatment was carried out by a team consisting of a physician,surgeon and microbiologist.

    Summary: In this work we present oral infection in 50 patients during methicillin resistant Staphylococcal sepsis.We investigated the microbiological status of inflamed oral mucosa.We obtained smears of the oral mucosa during in different stages of sepsis. In patients with rapid reversal of sepsis we obtained two smears,one during sepsis and recovery.In patients with slow reversal of sepsis we obtained more than two smears during sepsis,and one smear after recovery. We want to present the successful antibacterial treatment of oral infection during sepsis.

    Summary: The aim of the presented study was to research the consequences of pathological changes of oral mucosa following the head and neck radiation therapy,to determinate differences in the salivary gland irradiation and relations between symptoms,radiation dose,severity of xerostomia and oral pathology. The presented results point out that the rates of salivary secretion differ significantly before and after the radiation therapy,leading to reduction of the salivary secretion and severe radiation induced xerostomia.Diseases of oral cavty are consequences of this condition developed during radiation therapy. Xerostomia was the mayor oral symptom.Other pathological changes of oral mucosa were also found:gingivitis,parodontitis,stomatitis,cheilitis,leucoplakia,glossitis and glossopirosis. The interval between the radiation therapy and the occurrence of xerostomia,as well as the presence of oral disease were not clearly related to the area of radiation field or to the radiation dose.Severe worsening of oral hygiene and increase in number of patients with hard dental plaques were evident.It seems that the symptoms of xerostomia with concomitant oral diseases of varyng severity will eventually develop in almost all patients undergoing radiation therapy of head and neck tumours.
