Summary: Background: The Euporean Genera Practice Research Workshop
conducted an Internatinal study in which it tries to compare the attitudes
and knowledge of general practice patients about AIDS in different European
countries. The study tried to answer the following questions: Is there an
AIDS anxiety epidemic? Do patients trust their general practitioners? What
are the patient,s source of information? Are they well informed?
Method: The survey was carried out in several European countries, including
Slovenia, with uniform questionnaires, which contained basic information on
patients clinics and their answers to the fields of questions mentioned.
Results: The percentage of people worried about spreading of AISD was
considerably higher than expected, but we cuold hardly speak of any
epidemic of fear. People generally trusted their general practitioners very
much, they also often expressed a strong wish to talk. Television become an
important source of information. People from all countries are generally
well informed how the disease is spread, but many still do not understand
which contacts are not dangerous, which was very pronounced especially in
Slovenia.
Conclusions; The study has shown a relatively high proportion of worried
patients, though without excessive anxiety and great confidence in their
general provetitioners and in the media, especially television.

Summary: The social awareness of a socety can be assessed by
observing how it cares for those who "can,t"(can,t wak, can,t work, can,t
pay).Health care suplly has long been recognized as a high pririty in
establishing social security and equality. For centuries ethics of the
medical profession have imposed strict rules to protect those who are
unable to pay for service. All the same, the problem of the treatment of
the health uninsured patient has become very interesting in the last few
years. interest has increased with progression of economic crisis all
around the world. Special interest can be noted in the USA where the
goverment is concerned with a great number of uninsured people and in
Eastern Europe where economic crisis goes together with reconstruction of
economic system. It is expected that changes in health system will result
in an increasing number of health uninsured patient in Croatia. This survey
is planned to explore how general practition is prepared for confrontation
with patient who "cannot pay for service" and how they will respond. The
four level of influence on this situation have been recogniyed: social and
economic development of the country (determines the proportion of those who
"can,t pay" in society) the health insurance system (determines the
Alternative way of payment), the role and status of GP in the health system
(remuneration system and proportion of private patients in GP service), and
finally characteristics of the GP and the members of his team. The focus of
the survey will be on the least level: how a GP,S characterictics and
characteristics of member of his team influence the solution of the
problem. The method os survey will be observation in real situations by
simulation of sickness performed by trained observer. In a pilot study a
woman of about 30 Years of age will pay the role of a patient with dysuric
complaints who noticed blood in the urine. She will also be supplied with
fake urine sample conteining blood. The protocol for observation will be
prepared in a manner to note if the receptionist allows her to contact the
doctor, what the doctor says/does, wheather she is examined properly,
whether therapeutic choice is influenced by the fact that she "can,t pay"
and how she is treated in general. It is expected that observations from
pilot study willprovide sufficient material for qualitative analysis of the
way GPs respond to the observed problem.

Summary: In this paper the characteristics of the management of
hypertonics have been studied in general practice, with special emphasis on
prescribing antihypertensive drugs. From medical records of 200 registered
hypertonics parsons, data were collected on encounters to the doctor,
values of arterial pressure and prescribed antihypertensive drugs. In
accordance with the development of knowledge on the ratinal pharmacotherapy
of hypetension and the need to ratinalize the total health expediture od
drugs during 1991 and 1992, significant changes have made in the regulation
of drug prescriptions covered by the Health Secutity Fund in the Republic
of Croatia. Thus in this paper the analysis and comparison of the data of
prescribing antihypertensive drugs for 1990 and 1992 have been made.
Hypertonics make up 13,9% of the total population registerd by general
practitioners. Out of 200 hypertonics 31 (15,5%) did not have any
encounters with their GPs during a 1 year observation, whilw 19 (8,5%) of
them were treated at home due to more serious illnesses. During 1990 117
(60.6%) hypertonics and during 1992 160 (80%) hypertonics were recorded
with arterial pressure ( in the medical records). Data on drug therapy were
recorded for 86,5% hypertonics in 1990 and 92,5% in 1992. According to the
number of the total prescribed antihypertensive drugs, 60% of hypertonics
used one group, around 30% two, and around 8% tree groups, while in both
years of the observation there was only one patient who had
antihypertensive drugs prescribed from four groups. The pattern of
prescription for antihypertensive drugs was done according to WHO
recommendations ; most frequently prescribed are Ca channel antagonists,
diuretics, beta-blockers, and also ACE inhibitors. A regular critical
analysis of the management of special groups of patients is a prerequisite
to improving the quality of professional work in general practice. This
study is an example of the problems-oriented evaluation of professinal work
in general practice.
This study showed that through critical analysis of prescription of
antihypertensive drugs, one can evaluate the management of hypertonics and
improve their standard of management in general practice.

Summary: The paper consists of suggested contractual arrangements
between general/family practitioners and Croatian Health Insurance Fund.
Suggested contract is inspired by specific work conditions in primarz
health care and is suitable for physician. The suggestion is commented and
compared by evaluation of European expiriences in health insurrance
contract arrangements.
