    Summary: Data were collected on the age and sex structure of patient consultations with general practitioners. In comparison with natinal consulting patterns, the general practitioners in this survey probably cared for a population which was biased towards older parsons since in the towns especially, younger people are cared for by specialists in occupational health or by paediatricians and scool doctors. This study has shown that despite these administrative arrangements, general practitioners were providing some primary health care for children in all areas. This is a significant argument for the reorganization of primary health care towards the family concept and for the re affirmation of general practice. Data on consultation and aspects of the work in general practice provided in this study are more complete than official statistic data. Data on referrals, describing not only the quantitative but also the qualitative characteristics of refferals, will be used in postgraduate and continual education of general practitioners, notably to improve the quality of the care provided.

    Summary: Many European countries are leading serious scientificdiscussions about the place and role of general practitioner indifferent health care systems. A significant contribution tothese discussions is given by the New Leeuwenhorst group - theEuropean group for the promotion of general practice throughlearning and teaching. Most of the authors agree that it is easier to define the sphereof the activity and scope of work of the GP than it is to assessthe quality of this work and reasons for this work. Having analysed the work, discussions, analyses and views ongeneral practice in the last 6-7 years of the New Leewenhorstgroup, a conslusion can be drawn that there exist two, sometimesopposite approaches in general practice, and two views on it as ascientific disciline. One approach is mostly present in countrieswhere GPs are owners of their own consulting rooms (regardlesswhether health care in that country is privat, socialized orcombined). In the centre of the interest is the relationshipbetween GP and the patient and the problem of postgraduatetraining of GPs with the aim to have a more qualitative generalpractice. This group includes Germany, Switzerland, TheNetherlands, Scandinavian countries, Great Britain and partlyAustria and France. In the other group are countries where general practice, besidesthe mentioned problems is trying to solve also problems relatedto the health care of the populetion because health care isorganized on the comunal level. This group includes Yugoslavia,Portugal, Spain, Czechoslovakia, Hungary and partly some othercountries. Analysing both approaches, the authors tried to summarize themost important characteristics and views and give features ofgeneral practice in European countries.

    Summary: Objective: to measure the time used by a patient to explain the reason for his/her visit to the General Practitioner, when he/ she is permitted to talk freely and without interrruption. Design: A descriptive stufy where only previous known patients, who had a new reason for a consultation, were considered. Setting: Visit to General Practitioner,s office in Slovenia and Croatia. Participants: Six General Practitioner (two Slovenes and four Croatians) who recorded the time spend on 480 consultations. Measurements and main results: The period of time measured was that used by the patients, at the begining of the consultation, to reply to the doctor,s question: " What can I do for you today?" Avarage time used by the patients was 28.6 seconds. There was a link between the older age group and longer time period. There was no gender differences. Conclusions:Patients are able to explain calmly and without interuptions the reason for visit. Interruptions by the doctor are probably unnnecessery and do not save time.

    Summary: The study investigeted the reasons for encounters (RFE) and the use of International Classification of Primary Care (ICPC) which describes RFE and the process of GP,s work. Data on 150 regular patients were recorded by the GP and the GP specijalist during their daily work into the questionnaires prepared for this study. Data on sex , age, social and family status, RFE, diagnoses and actions taken to solve the patient,s problems were collected. The following distribution of RFE was noticed in our study group: complaints and symptoms in 50% of patients, diagnostic and preventive component in 18%, tretment procedures and medication in 13.3%, test results in 9.3%, administrative reasons in 2.7%, refferals in 6.% and diagnosis or disease component in 0,7%. The analysis indicates that RFE are primarly determined by patient,s characteristics and by general practice itself. Statistically significant differences were observed between the patterns of work of GP and GP specialist, both inRFE and in the applied procedures. The differences are probably caused by different parsonal attributes of a doctor, level of professinal education, professional experience and the continuity and longitudinality of care for the same population. This preliminary research on the use of ICPC has shown that it can well desceribe the specific characteristics of general practice.

    Summary: The study tries to evaluate the relatioship between the physician and the patient in general/family practice. The survey lasted tree months. During this period every new patient in general practice office was interviewed by physician. The question was: " Who os your physician?" while the answer were written down in their original form. The results were analyzed by qualitative analysis method. A few groups of answers were recognized. Almost 90% of patients understood the question and 55.9% named their general practitioner as their physician. One patient in the four names their practice instead of the GP and one in the twenty their specialist. One of the eight does not know the answer to the question. It is statisticaly significant that the patient usually know the name of the physician who spent 10 or more years with the same population while it is not so when the period is shorter. The results indicated that the physician personality is more important to the patient than the facility where he is working. But it still remains to evaluate this phenomenon by further studies.

    Summary: The aim of this study was to find out if any significant changes occured after introducing the free choice of general practitioner (GP) into primary health care. The determination of patients, choice and GP, attitude regarding this matter were tested. A survey was conducted in 1992 by distributing questionnaires to 71 GP,s and 292 patients who chose these GPs.The questionnaires were composed selectively for GPs and the patients, and they all answered them anonymously. The group of GPs consisted of 37(52.1%) GPs with vocatinal training and of 31 ( 43.7%9 GPs without vocational training..The patients evaluated the GPS characteristics as very important in choosing a particular GP.Of medium importance were the surgery characteristics ( where it was situated, work organisation, equipment), and recommendation were the least important. most frequently, the patients chose a particular GP because he was already the GP of one of the family members. The GPs assessed the famoly to be the most important determined for a patient to choose a particular GP. Surgery characteristics (distance and work organisation) were evaluated as being of medium importance. The recommandations of a friends were of medium importance, and the recommandatins of medical staff were not important, according to the opinion of the surveyed GPs. The only occurence detected in the survey was that of a " return" phenomenon, in which patients returned of their old GPs, which was direct consequence of the implementation of the free choice principle into health care practice. The results of this study match the results osf similar studies already published, and they are argumented theoretically. These results contribute to the policy of the free chioce of a GP in primary health care, and stimilate further investigations.

    Summary: Estimates of the needs for education of phzsicians in Croatia must be based on analysis of population health condition and age structure, on the country,s economics capacities on which the employment of those specialist depends, and on estimates of realistic educational capacities of the Croatian medical schools. Basic analysis of mentioned parametars indicates that in the upcoming decade there are no possibilities of more extensive employment. Thus, it is obvious that it is necessary to educate only a certain number of physicians: those which will serve as replacement for decreased, retired or physician leaving for other positions. To keep in mind- presently the unemployed physician constitute a number which usually satisfies two-year need for new employment. thus it is recommended that the number of enrolled medical students in Croatia be reduced to around 400 students yearly. Together with a proper selection of candidates at admission, these would guarantee that about 320 studentswould be graduating eventually. This number does not include students from abroad, yat their number is almost inconsiderable according to the past few years and their studies are financed from separate fund. Medical studies should not be financed according to the number of enrolled students, because it stimulates larger enrollment, but according to the actual cost of theeducatinal programme. Financing the programme, and not the number of stuidents would improve its quality and stimulate a larger number of highly skilled and competent students. Medical studies are expensive not only because of the lenght of the programme (6years) but also because of the nature of these studies. During medical studies the student has to acquirea number of skill and knowledge and adopt attitudes which require training in a variety of, often, very expensive institutions.

    Summary: A qualitative study was carried out related to those patients only who came to their doctor requiring a drug prescription. It was supposed that for the patients such a visit to their doctor was not only a "blank" consultation, but it actually implied a hidden desire to collaborate more deeply with their doctor. We tried to established how doctors really fcomprehend such type of communications. Of 63 doctors, 90,5% refused the patient's demand for a prescription, and 9,5% prescribed the demanded medicine. 63,7% of the patients agreed with the doctor's refusal to prescribe, and this only proves that prescribing a drug was not the basic motive for visiting the doctor. Analysis showed that not one doctor had a through interview with his patient, and only 16,5% offered an additional control check-up of patient. Other used the model illness-diagnostics-medication, and while giving short advice to the patient prescribed or refused to prescribe the medicine.

    Summary: The purpose of this study is to anatyse the implementation of free choice of doctor with special emphase on characteristics of the process which could be important for the future privatisation in general practice service in the Republic of Croatia. During 1992, 71 General Practitioners and 292 of their patients have been interviewed. General Practitioners were asked to fill in the prepared questionnaire: personal information, location of practice, the personal list size, and the reasons because of which thez were chosen bz the patients,according to their own assessment of importance. The patients were asked torecord age, sex, the reason which determined their choice of a doctor and the way in which they choice the doctor.In a study group of GPs average list size was 2094 but great difference in list size were noticed from 450 minimum to 5000 maxsimim. The portion of the whole families or two or more family member on GPs list was estimated to have been 50% on the average. Family was the most important factor both for GPs and the patients in choosuing a doctor. The patients evaluated the specific characteristics of GPů,s work as very important in choosing a particular GP. During 1992 only 7% of patient did not choose their former GPs; the phenomenon of inertia of patients in choosing a doctor is well known and described. According to results of this study we can expect that the majority of the registered patients on the GPs list will choose the same GPs, no matter whether GPs work in private or pablic service. In the implementation of the private practice in the general practice servise the prerequisite for getting the licence should be reconsidered. The most important prerequisite is the adecquate education and capability of the physician of private practice to provide comprehensive, family orented and continuous health care for patient and his family.

    Summary: There is no doubt that home health tretment, care and rehabilitation constitute the program of activity of a good team of family doctors in the primary health care. Paying attention to a family, plurigenerational support to a patient, such a team could evaluate in the most appropriete way the successfulness of home health treatment, care and rehabilitation of a patient, due to her-his phase of illness, functional status and the social medical security of the patient, as well as of the family itself.     Summary: In general, two concept in European countries differs according to tradition and current status of family practice.In general, two concepts dominate and they have their representatives in two generations of family practitioners. The generations are less linked to the age of family practitioners, but more to the age of family practice as recognised medical discipline. The first generation consists of those who do not fit into anything that already exists in medical world and they do not have opther choice but to create something else. They spend their lives in cutting out something new and struggle all the way with suspicion whether family practice worth is that much. They are still seeking final confirmation for the great job they have done and they look for it at goverment level. All these characteristics remain on " Cristofer Columbus life story"- they can be called COLUMBUS generation. The second generation finds themselves in already developed medical discipline that fits their taste.They do not search for place under the sun in existing medical world,they create an entire new enviroment and leave the others to accept their innovations. Their confirmation will be general public acceptance of criteria they have launched. Therefore, they could be called NOBELS generation, according to ALfred Nobel. IF COLUMBUS efforts give fruit family practitioners can find themeselves as goverment employed clercs. As paretns often do, they can earn the legacy that inheritors do notwant:they can make NOBELS become a part of establishment.

    Summary: In according with the great social, economic and political changrs in the Republic of Croatia after 1990 significant changes have been in the organisation and functioning of health system. Before 1990 primary health care was divided into four main parts. It was delivered by occupatianal medicine to employees, by school medicine to school children and students, by pediatricians to preschool children and by general practice to retired people, elderly and unemployed. In spite of this administrative regulation GPs in rural areas and in some towns (such as in Health Centre Novi Zagreb) cared for population of all age groups and worked as family practitioners. Approximaly 30% of all GPs cared also for preschool children. In 1990 the free chioce of a doctor in primary health care has been introduced. In this paper the analysis of the implementation of the free chioce of a doctor in the general practice in the Republic of Croatia has been made. The special emphasis has bbe put on the influence of the free choice of a doctor on reorientation of general practitioners to family practitioners. During 1992, 71 GPs and 292 of their patients have been interviewed. Out of 71 GPs interviewed, 50 (70.4%) were females, 37 (52.1%) were specialists ofgeneral practice, and 52 (73.3%) of them had their practice situated close to place of living of their patients. Great differences in the size between the GPs examined were noticed from 450 (minimum) to 5000 (maximum) patients. Average list siye according to regulation of Croatian Institute for Health Insurance is 1500 (minimum) to 2100 (maximum) patients. All of GPs examined had whole families on their list and 50 % of them cared for preschool children. The portion of whole families or two or more members on the GP,s list was estimated to have been 50% in average. Out of 292 patients,165 (56.5%) chose their GP because he cared for patient,s family. The family was identified as the most important factor which determined the choice of a doctor both by patients and GPs. The observed changes in the structure of the list of patients - increasd number of GPswho cared for preschool children and increased portion of families on GP,s list - confirmed the acceptance, implementation and development of family approach in general practice in Republic of Croatia..

    Summary: In the last decade primary health care become the most prominent strategy in restoring the link between hospital-oriented and disease-oriented medicine and realistic health needs of the population. However, the reorganisation of health care system by introducing the primary health careconcept appears to be a very difficult task. Such a conceptinterferes with huge hospital systems, but also with already defined outpatient/first line doctors,facilities and manpower. In1980 the new Health law (influenced by Declaration from Alma-Ata in 1978) introduced PHC into the healthsystem in Croatia. At that time being general practice was already recognised as medical speciality in Croatia and vocatinal training for GP had been organised for 20 years. GP s were trained todeliver family medicine care developed in the way that the number of GPs increased from 1597 in 1980 to 2296 in 1990 (rate 1.43) and the number of specialists who worked in primary care increased from 522 in 1980 to 1047 in 1990 (rate 2.0). Beside other specialists, the number of occupational health doctors alone increased alone in the rate of 1.63 (545 doctors in 1980 and 893 doctors in 1990). The number of gynecologists, practices changed in the rate of 1.43 (108 practices in 1980 and 155 practices in 1990), the number of pediatricians, practices changed in the rate of 2.43 (122 practices in 1980 and 297 practices in 1990) while the number of GPs, practices changed in the rate of only 1.19 (792 practices in 1980 and 947 practices in 1990). These figures show how inclusion of medical specialists in primary care slows down the development of GP service. Numerous gynecologists and pediatricians diminish the role of family physician in outpatient care. Apart from these figures othjer experience show how the introduction of PHC jeopardizes " personal touch" of family doctor in relation with patient ( what is essential for the family medicine.) Expiriences from Croatia raise the question whether or not the PHC concept goes along with family medicine concept.

    Summary: We analyzed the roleof family health service in the health care system . its advantages are emphasized. Whether this kind of health care will be accepted and realized does not depend on uor readiness but on the legislator as wll. He must accept our suggestions, from them into the law and persist upon them. Criticism that this system will influence in any way the primary health care does not have any qualified or scientific basis. In this report we did not use our or foreign literature to support our views. We added it at the end our work in order to convince those who strictly belive in experimental works. in order to avoid any responsibility for the failure of another law, what happened a lot in our history, we organized this Symposium to have a possibility to express our reflections and views openly and clearly. we belive that many will respond to this call and that the adequate action will be taken in which we shall all be included.
