Summary: The influence of HLA-B27 antigen on clinical, radiological
andlaboratory features of rheumatoid arthritis (RA) was studied .The group
with B27 antigen comprised : 4 males and 20 femalesaged 28 to 71 and a
group of patients free from B27 antigenconsisted of 2 males and 21 females
aged 26 to 69.
RA patients possessing the B27 antigen did not differ regardingthe age of
the onset of disease, the distribution of affectedjoints during the
follow-up (3-10 years), the development stagesof the disease and the
presence of rheumatoid factor whencompared with B27 negative RA patients.
the arthritis of the radiocarpal joints (p<0,01) wassignificantly more
often the first sign of the disease inpatients with B27 antigen. In these
patients low back pain andmorning stiffness in the low back were twice as
frequent as theywere in patients not possessing this antigen. The
clinicalpattern of the affected spine was also found more frequently inB27
carriers ( p<0,05). B27 positive RA patients showed also theclinical
pattern of sacroileitis (SI) (p<0,05) and x-ray SI(p<0,01) significantly
more often than the patients free fromthis antigen .In addition, symmetric
arthritis of the periferaljoints was more often diagnosed in patients free
from B27antigen.

Summary: Immunologic reactivity of patients with psoriatic arthritis
andthe influence of the activity of psoriatic arthritis onimmunologic
reactivity were analysed.The group of psoriaticarthritis patients, aged 17
to 65 years, consisted of 36 malesand 13 females with confirmed diagnosis.
51 age- and sex-matchedvolunteers served as the control group (32 males and
19 femalesaged 21 to 62 years). The following parameters of
immunologicreactivity were analysed: number of leucocytes, percentage
ofthe lymphocytes in peripferal blood, number and percentage of
Tlymphocytes with positive CD3, CD4 and CD8, number and percentageof B
lymphocytes with surface IgG, IgA and IGM, number andpercentage of large
granular lymphocytes (LGL), lymphocytyctransformation by
phytohaemagglutinin and phagocytic index. Theauthors found statistically
significant reduction of the numberand percentage of CD4 T lymphocytes,
number and percentage of Blymphocytes with surface IgG as well as
significantly decreasedlymphocytes transformation and phagocytic activity
of peripheralgranulocytes in patients with psoriatic arthritis in
comparisionto control group.

Summary: Incidence of ankylosing spondylitis in HLA-B27 antigen
carrierswas studied in 176 examinees (102 men and 74 women) aged 15 to74.
All the subjects underwent clinical examination of thelocomotor system,
x-ray of the sacroileitis joints andthoracolumbar passage, ,as well as some
laboratory tests(sedimantation rate, immunoglobins).
Pain in the joints was registered in 47 (26,7%) of the cases andfound
significantly more frequent in women than in men. Theswelling was observed
in 28 (16%) of the subjects. Pain in theboth heels was present in 22
(12,5%) of the cases. Clinical signsof manubriosternal synchondrosis were
found in 39 (22,2%), morefrequently in women than in men. Iridocyclitis was
observed in 9(5,1%) while pain in the cervical spine was registered in
96(54,5%) of the cases, most of them women. 132 (75%) of thepatients mostly
women again, suffered from lower-pain, whilemorning stiffness in the lower
back (lumbar spine) wasexperienced by 90 (48,9%) patients. Pain in the
sacroiliacaljoints was palpated in 83 (21,6%) of the cases. Inclination
ofthe lumbar vertebrae was limited in 138 ( 78,4%) and reclinationin 100
(56,8%), while the breathing index was lower in 130(73,9%) of the cases.
Sacroileitis, most frequently of stage II,was x-ray confirmed in 82
(46,6%). Changes of the thoracolumbarpassage were observed in 23 (13,1%) of
the cases. The diagnosisof ankylosing spondylitis was found in 12 (6%) and
suspect in 10(5,6%) of the patients.

Summary: Possible associations between HLA-DR1, HLA-DR4, HLA-B27
antigensalone or in combinations, and laboratory, radiological findingsand
features of clinical status were analyzed in 175 patientswith rheumatoid
arthritis. HLA-DR1 was associated significantlywith the absence of
vasculitis, Raynaud 's phenomenon,tenosynovitis and sacroileitis in
seronegative patients. HLA-B27was associated significantly with the less
swollen joints andtenosynovitis, and with the presence of iritis,
sacroileitis inseronegative patients, shoulder involvement and reduced
Schober'stest. HLA-DR1 in combination with HLA-B27 was
associatedsignificantly with the presence of fourth developement stage
hipinvolvement and increased levels of serum C reactive protein.HLA-DR4 in
combination with HLA-B27 was associated significantlywith the presence of
coxitis and feet small joints involvementand reduced Schober's test.
HLA-DR4, HLA-DR4 in combination withHLA-DR1, and HLA-DR4 in combination
with HLA-DR1 and B27 were notassociated significantly with differences in
the presence of anylaboratory findings, radiological changes or other
features orclinical status.

salts

Summary: The objective of this research has been to determine the
effectsof gold salts (Tasuredon, Byk Gulden) on the development of IgG,IgA
and IgM in patients with rheumatoid arthritis. The groupbeeing tested
comprised 8 men and 35 women, their average agebeing 50, the average
disease cours 9,5 years. During asix-months continous application of
Tauredon, the development ofimmunoglobulin, classes IgG, IgA and IgM has
been observed inregular intervals, being determined by
electroimmunodiffusionand by immunoelectrophoresis of the same
immunoglobulin classes.
The evaluation results of six-months treatment by Tauredon showedthe
significant effect of gold salts on the decrease of IgG(p<0,007), IgA
(p<0,001), and IgM (p<0,001) i.e. the gold saltsconsiderably decreased the
immunoglobulin level in the serum,without the same immunoglobulin classes.

Summary: The aim of research was to establish effect of gold-salts
(underprotected name "Tauredon" Byk Gulden) on the dynamics ofrheumatoid
factor, detected with Waaler-Rose test, at petientswith rheumatoid
arthritis. Following the dynamics of rheumatoidfactors was performed during
6 months continued application of"Tauredon" on a group of patients with
certain diagnosis ofrheumatoid arthritis. The group consisted of 8 men and
35 women,with age profil of 50 years.
With continous observation of patients and their laboratoryfindings
(Waaler-Rose test) in regular time intervals it wasfound out that
"Tauredon" has significant influence onoscillation of rheumatoid factor,
detected by the method ofhemaglutination of sheep erythrocites.

Summary: The objective of this research has been to determine
theefficancy of Na-aurothiomalate, under the trade name Tauredon, onthe
development of circulating immune complexes (CIC) in patientswith
rheumatoid arthritis. During a continous six-monthsadministering of
Tauredon to a group of patients with thediagnosis of rheumatoid arthritis
various parameters have beenobserved. The group comprised 43 patients, 8
men and 35 women,their average age being 50 years. By continous
observations ofboth the patients and their laboratory reports in
regularintervals during six-months administering of Tauredon, a
positiveeffect of Tauredon on the development of the CIC IgM
values(p=0,006) has been provided, i.e. reducing their level in
thepatients' serum. Owing to great individual differences in thedecrease of
the CIC IgG values, it has been difficult to provethe consequences of this
fall (p=0,086). The imposibility oftesting the significant decrease of the
CIC IgA values is due tosmall number of patients with positive CIC IgA
values.

Summary: HLA-system antigens were introduced into clinical
rheumatologyfor the first time in 1973 when it was proved that antigens
areconnected to some rheumatic diseases (ankylosing spondylitis,Reiter's
syndrome). According to the data from literature HLAantigens have not been
investigated in PHO. Therefore, weundertook to research the frequency of
antigens of the HLA-Blocus in patientswith PHO.
Our research covered 18 patients. Their average age was years.
The antigens of HLA-B locus were determined on the lymphocytesfrom the
peripheral blood by the lymphocytotoxic test.
The HLA-B12 was discovered in 8 of 18 patients (44,4%).

Summary: The characteristics of the pain were investigated in 99
ankylosing spondylitis patients ( 62 men and 37 women) from 17 to 70 years
with various developing stadies of sacroileitis. The analysis of the
sacroileitis pain characteristics revealed the dominance of appearing in
rest ( at night and by day rest) and obligatory morning lowback stiffness.
The intensity of pain was more severe in women, more dull, located
inguinally, in low abdomen and symphysis. The pain propagated on more
distant locations ( the tigh and the leg), shortly or for longer period in
one and /or both legs. The provocating factors of intensification and
propagation of the pain were commonly metereologic fators and rest during
night and/or day. The evaluation of pain characteristics could be of great
benefit in diagnostica of the sacroileitis or ankylosing spondilytis which
usually are not concerned with special significance.
