Summary: The aim of this project is to study particular phases of
bone marrow transplantation in order to improve the results of treatment,
especially of malignant diseases. Programs of intensive preparative
regimens for transplantation, complications and their frequency as well as
diagnostic will be investigated. The special emphasis will be given to
improvement of prophylactic approach and management of complications of the
graft-versus-host disease and infections. The value of this method in the
treatment of malignant lymphomas and immunoproliferative disorders as well
as usefulnes of the peripheral stem cell transplantation will be assesed.
The efficacy of cytokines on the function on marrow transplant as well as
their potential antitumour effect will be studied particulary in patients
with leukemia. Voluntary marrow donor registry will be made which is
necessary for assesment of unrelated marrow transplantation in Croatia.

Research goals: Aims and results I. Evaluation of conditioning
regimens and function of marrow graft with expanding BMT programm to other
diseases: 1. To compare efficacy of busulfan and cyclophosphamide to
standard total body irradiation and cyclophosphamide in allogeneic and
autologous BMT 2. To evaluate efficiency of autologous BMT in the treatment
of high grade malignant lymphomas 3. To study the effect of addition of the
peripheral blood stem cells on the marrow recovery after BMT 4. To
investigate prognostic factors influencing on marrow recovery after BMT 5.
To asses the value of peripheral blood stem cell transplantation in the
treatment of hematological malignancies 6. To evaluate the effect of
various cytokines on marrow recovery II. Evaluation of incidence,
diagnostic approach, prophylaxis and treatment of graft versus host disease
1. Diagnostic value of immunohistochemistry and finding of AgNOR bodies in
the skin and intestine of the patients with GVHD 2. Clinical and laboratory
prognostic factors influencing GVHD 3. Prophylaxis of acute GVHD using
immunoglobulin with other immunosuppressives 4. To estimate efficacy of "in
vitro" marrow treatment with cyclosporine and corticosteroid as a GVHD
prophylaxis 5. To evaluate efficency of systemic GVHD prophylaxis after BMT
6. To study the incidence of infections with resistant bacterial in
immunocompromized patients 7. To study the incidence of fungal infections
after BMT 8. To study the incidence of CMV infections and to determine
optimal diagnostic approach to those patients after BMT III. Determination
of factors influencing leukemia relapse 1. The value of chimerism after
marrow transplantation and its relation to leukemia recurrence 2. Use of
interferon alpha, interleukin-2 and LAK cells in prophylaxis of leukemia
recurrence in patients treated with marrow transplantation IV To determine
therapeutic effect of unrelated marrow transplantation 1. Establishment of
unrelated marrow donor registry 2 To estimate efficacy of HLA identical
unrelated marrow grafting Expected achievements 1. Improvement of marrow
transplantation, improvement of basic diagnostic procedures for treatment
related complications, better prophylaxis of treatment related
complications 2. Expanding of marrow grafting programme to other diseases
like malignant lymphomas and immunoproliferative disorders 3. New
diagnostic and therapeutic procedures might have benefit for other
specialties.
