Summary: Proposed project comprises research in some of the specific
and unspecific immunological parameters in several lines of infectious
diseases: viral hepatitis (A,B,D,C,E other non-A, non-B, non-E viruses),
acquired immunodeficiency syndrome (AIDS) or generally HIV infection,
Lyme borreliosis, pertussis, infections of respiratory and digestive
tract, infections with herpes viruses, rubella virus, tick born
meningoencephalitis virus and by Toxoplasma g. Analysis of
immunomediators like TNF, Interleukin 1, and nearly connected endotoxin,
is directed in the first place on patients with meningitis and some other
septic conditions, as, untill now, the best defined model for such type
of investigation. Precise follow-up and analysis of specific immunologic
parameters enables the only possible exact diagnosis in the first two
illnesses (viral hepatitis, AIDS - HIV infection) and advances it
essentially in other infectious diseases. Detection of presence, dynamics
and correlation of immunomediators and endotoxin with clinical picture
and prognosis of illness in septic condition and purulent meningitis is
in the very beginning, but so far informations indicate their essential
role in pathogenesis of illnesses listed above and a possibility of their
usefulness in practical purposes.
Research goals: The goal of proposed research is to detect: 1.
specific andunspecific immunological parameters - antigen/antibody
system,subpopulation of T and B lymphocytes, immunomediators; 2. in
whatextent immunologic events can give precise characterization of
momentarily pathogenic condition of patients, serve as a base forexact
diagnosis and if the constellation of these parameters canindicate outcome
and prognosis of the disease; 3. in what extentthese parameters can be
used as orientation for the beginning oftherapy and evaluation of their
efficiency or application ofadequate prophylaxis in prevention of some of
above mentionedillnesses. In the field of viral hepatitis the object of
intensive researchwill be: simultaneous appearance of HBsAg and anti-HBs,
specificimmune complexes, Pre S1 and Pre S2 systems, isolated appearanceof
HBsAg with out adequate antibodies (HBV2 virus). Antibodies to HCV and HEV
and genetic material of HCV what enables earlydiagnosis and beginning and
monitoring of efficiency of hepatitis C therapy. The only possibility of
exact determination of contact with HIVis analyisi of immunological
markers. Determination of its kinetic may point at the outcome of infect
and be a finger-post for thebeginning of therapy before the symptoms of
disease becomeevident. Special problems are individuals with undefined
resultsof this parameters. The main support in making diagnosis of Lyme
disease is theanalysis of specific antibodies. Determination of
antibody/antigen enables rapid and exact diagnosis respiratory and
digestive tract infections, infections due to herpes viruses, tick born
meningoencephalitis virus and infections with Toxoplasma g. Contribution
of proposed research has accordingly a scientificand a practical meaning.
Scientific contribution consists inprecise determination of condition and
dynamics of immunologic parameters in the course of mentioned infectious
diseases.Determination of correlation of these elements with
clinicalstatus of patient, course of disease, choice and efficiency of
therapy and outcome of disease could give very practical contribution.
