    Summary: Detrusor instability represents unsettled therapeutic problem.The incidence of this voiding dysfunction is between 30-50%of women who present for urodynamic investigation. It is the commonest cause of urinary incontinence in postmenopausal women.The key for rational pharmacotherapy is understanding of basic physiology and innervation of urinary bladder and urethra. Our understnding of pathophysiology for unihibited bladder contractions is insufficient and therefore the treatment is often ineffective and empirical. Drugs which are currently used in the managment of DI inhibit bladder contractility and/or increase outlet resistance.Propantheline and emepronium bromid as well as combination of imipramine hydrochloride and propantheline bromide are ost frequently used drugs for inhibiton of bladder contractility. Local estrogen therapy and seldom alpha adrnergic agonists are used for increasing of outlet resistance also there is a lack of quality controlled clinical trials for evidence of such efficacy.The result of pharmacotherapy of DI are dissapointed. The mayority of drugs are not superior to placebo. Since the therapeutic drug efficacy is correlated with side effects it is essential to increase dose gradually until symptoms are relieved and side effects are tolerable. There is a lack of high qualitz controlled clinical trials which will determine drugs with increased efficacy and acceptable side effects.Further researches on molecular bases of uninhibited bladder contractions wiil additionaly explain the ethiology of this voiding dysfunction and thus make possible to offer patients more than placebo. In the article are presented and discussed drugs which are currently used in the therapy of DI according to their efficacy, acceptability and potential side effects.

    Summary: Residual urine was found in the majority of stress incontinence patients and these patients had significantly more residual urine than control group.The incontinent group had significantly more cases of urethrotrigonitis,cystitis and chronic recurring cystitiy than the control group. 350 patients with stress urinary incontinence were examined during 5-years period.The control group consisted of 148 continent gynaecological patients of approximately same age.In the incontinent group we found normal findings in 11%, urethrotrigonitis in 47%,cystitis in 18% and reccuring cystitis in 24%.In the control group we found normal findings in 80%, urethrotrigonitis in 12%,cystitis in 6% and reccuring cystitis in 2%.Cystoscopic findings of incompentent ureteral orifices in 350 patients with stress urinary incontinence were:78,86% normal,8%first degree of incompetence,4%second degree and 9,14%third degree. In control group we found:98% normal opening and only 2%first degree incompetence. Urinary infections in women is about 80%caused by Escheria coli and their L-forms after long term therapy with antibiotics.
