Summary: Urinary icontinence is more frequent in women than in men.
There are different types of urinary incontinence which can be the
resultof multiple interrelated factors such as neurologic disorders,
neuromuscular dysfunction or inflammatory disease. But the most frequent
cause of urinary incontinence is weak or impaired anatomic support of the
urogenital system with considerable contribution of disorded local
innervation(stress or static incontinence). The incidence of stress
urinary incontinence in women over 45 years is 20-30%. The disease is
serious and its consequences could be difficult. Due to anatomic and
topographic changes which caused descending of the bladder base and
represent a reservoir for bacterial multiplication and their ascension
leads to progressive pyelonephritis. The result of long timetherapy of
uroinfections according to antibiogram without elimination of residual
urine is developing of resistant bacteria and progressive pyelonephritis
which leads to dialysis and death.Disorders of the anatomic support of
urogenital system caused mostly by birth which role is important in
etiology of stress urinary incontinence. Negative factors increased in war
conditions because of inadequate delivery and postpartal care. We expect
higher incidence of morbidity. In project, our research is based on
insufficiently known interrupted innervation of the lower urinary tract
disorders. The dillema is how to improve conditions of urethral neurologic
disorders, when operation is needed. By electronic urodynamic
differentiation and electroneuromyography of voiding dysfunctions wecould
be closer to optimal therapy in every case.
Research goals: 1. Investigate insuffitiently known and unknown
factors offunctional disorders of micturition in women. 2. Optimal
differentiation of relevant symptoms with moderndiagnostics and
objectivisation for selected therapy. 3. Selection of surgical procedures
for treatment which less thanothers disturbes innervation of functional
unit and causes theleast damage for patient's sexually and work life. 4.
Systematization of methods for conservative therapy accordingto the
precise verification of parametars to which it is applied. 5. How to
completely avoid iatrogenic infections of thelower urinary tract. 6.
Summarily coefficient of the disease necessary for actualevaluation of the
condition.It would be the sign-post to approachfor complexity of therapy.
7. Explanation of patophysiology in local innervation of theurethra and
urinary bladder in stress, urge and mixed urinaryincontinence in women. 8.
Explanation of disturbances on the leverl of alpha and betareceptors of
the urethra and urinary bladder. 9. Finding out surgical methods for
treatment of stress urinaryincontinence which do not intensify neurologic
disorders. 10.Systematization of drugs and procedures for
conservativetherapy according to precise established etiology of
voidingdisorders. 11. Creating a standard vaginotonographic curve among
normal andimpaired pelvic floor in the women of the same age.
