Title: MODIFIED METHOD OF NONSPECIFIC NASAL PROVOCATION WITH HISTAMINE FOR ROUTINE USE

Summary: An improved method for assessment of nonspecific nasal reactivity (NNR) was evaluated to validate it, and determine a criterion for nasal hyperreactivity. The efficacies of four different parameters to indicate nasal patency in assessment of the level of NNR were compared in 60 healthy subjects and 65 patients with allergic rhinitis. Histamine diphosphate was sprayed into both nostrils every three minutes in doubling concentrations (0.03125 to 32 mg/mL). The test was terminated when a significant change of values occurred. Our results indicate a significantly higher NNR in rhinitic patients than in healthy subjects. Our improved method for assessment of NNR by recording of NARt is a quantitative method with good reproducibility, high specificity and good sensitivity. We recommend it for routine use.

Title: RESPIRATORY SYMPTOMS AND IMMUNOLOGICAL STATUS IN POULTRY FOOD PROCESSING WORKERS

Summary: A group of 57 female workers emčployed in the processing of poultry food and 51 nonexposed control workers were studied to determine the possible relation between respiratory and immunological findings in poultry food workers. Occupational asthma was found in 5.3% of exposed workers. Exposed workers with a positive skin prick test to poultry food extract demonstrated higher prevalences of all chronic respiratory symptoms than those with negative skin tests, although the difference was statistically significant only for rhinitis (p<0.05). The most frequent skin reactions were found for poultry food extract (exposed: 66.7%; control: 25.5%; p<0.05). An increased IgE serum level was found in 19 (33.3%) of the exposed and in four (7.8%) of the control workers (p<0.01). Three exposed workers with occupational asthma demonstrated an increased serum IgE level. Our data confirm previous results indicating that occupational exposure to poultry food dust may be associated with the development of chronic respiratory symptoms and immunological changes in exposed workers.

Summary: The relationship of skin reactivity and serum immunoglobulin E (IgE) levels to the prevalence of chronic respiratory symptoms and to ventilatory capacity is examined in workers exposed to different organic aerosols. The results from group of control workers similarly tested are also presented. Workers exposed to occupational allergens had positive skin tests more frequently lthan did controls, except for soy bean workers. Workers with positive skin tests to occupational allergens had a higher prevalence of almost all symptoms than those with negative skin tests although the differences did not always reach statistical significance. Workers with positive skin reactions in general had significantly higher serum IgE levels than did workers with negative skin reactions. There was, however, no correlation between acute and chronic lung function changes and skin reactivity or IgE values. Our data suggest that although exposure to organic aerosols may be associated with frequent immunologic reractions, these findings do not predice objective respiratoryl impairment.

Title: IMMUNOLOGIC FINDINGS IN CONFECTIONERY WORKERS

Summary: Immunologic (skin tests and serum IgE) and respiratory findings (symptoms and lung function) were studied in a group of 71 confectionery workers (mean age: 35 years and mean exposure: 11 years). Most frequent positive prick skin reactions occurred with extract of cocoa (31%), followed by reactions to chocolate (9%), cocoa, hazelnut, almond (6%) and sugar (2%). Increased serum IgE levels were found in 13% of workers. The prevalence of asthma (26.1%) and dyspnea (26.1%) in workers with positive skin tests was significantly higher than in workers with negative skin tests (asthma: 2.0%, p=0.004; dyspnea: 4.1%, p=0.001). Both skin test positive and skin test negative workers exhibited significant across shift changes in lung function. No significant differences in baseline lung function or across-shift changes were found between skin test positive and negative workers. There was a high prevalence of acute respiratory symptoms during the work shift, but no significant association with immunological test were found.

Summary: In order to establish the possible genetic predisposition of asthma we analyzed 18 quantitative dermatoglyphic traits of the digito-palmar complex in 63 patients (level IgE>300 U) and 400 phenotypical healthy controls. Canonical discriminant analysis showed that the best discriminators were the b-c ridge count in males and the a-b ridge count in females, and other palmar variables. These results indicate the possibility of genetically determined predisposition of extrinsic bronchial asthma.

Summary: The relationship between index of subjective feeling of well-being (ISFWB) and ventilatory functions VC and FEV1 was examined. The sample consisted of 3 groups: chronic bronchitis (325 M, 220 F), asthma (74 M, 69 F) and healthy controls (330 M, 553 F). The ISFWB demonstrated higher values in males and females with symptoms of chronic bronchitis and asthma in comparison to controls. In the whole sample ISFWB was higher in females. A significant correlation was found between the ISFWB in female patients with chronic bronchitis and FEV 1% as well as in the control female group.

Summary: The effect of handling antineoplastic drugs was examined in 42 nurses working in an oncology department, and the same number of nurses not exposed to antineoplastic drugs acts as controls. The exposure effect was evaluated by analysis of sister chromatid exchanges (SCE) incidence and structural chromosomal abnormalities. SCE as well as chromosomal abnormalities in the exposed group were increased (p<0.001 and p<0.01 respectively). Sign test for paired sample was used for statistical assessment.

Title: THE VALUE OF CYTOGENETIC MONITORING VERSUS FILM DOSIMETRY IN THE HOT ZONE OF A NUCLEAR POWER PLANT

Summary: Cytogenetic analysis was carried out in 41 workers prior to and following regular maintenance work in a nuclear power plant. Although film dosimetry did not show the maximal annual permitted dose in any of the examined subjects, cytogenetic analysis was carried out following the work detected dicentric chromosomes in peripheral blood lymphocytes of 20 workers. According to our findings smoking habits and previous exposure to ionizing radiation had no effect on the increased number of chromosomal aberrations.

Summary: Radiation-induced structural chromosomal aberrations were the only adverse effects found in three workers overexposed to 60Co gamma-radiation. Chromosomal analyses were performed on the 5th and 75th days after exposure. Hematologic follow-up was carried out on the 5th, 12th, 16th, and 75th days after exposure. Hematologic findings did not differ over time in any of the three exposed workers. In two workers we found dicentric chromosomes, which are conventional indicators for exposure to ionizing radiation.
