    Summary: Kyphoscoliotic habitus was found in 3.6% of the adult populationin Croatia (N=4287). Within the population with kyphoscoliotichabitus 19.8% were registered with restrictive disorders. Thefrequency of kyphosis and/or scoliosis was examined in invalidpensions (N=1020). Respiratory damage was found in 27.6% in thegroup with kyphosis and/or scoliosis.

    Summary: The aim of the study was to compare the immunological alterationsthat occur in an asbestos-exposed group with asbestosis and in anasbestos-exposed group without asbestosis. Using standardmethods a significant reduction in percentage and absolute numberof T-lymphocytes was found, as well as in 80% of subjects alowered skin reaction to PPD antigen was observed. A significantincrease in the level of serum immunoglobulins was found insubjects with asbestosis in relation to healthy asbestos exposedworkers. The obtained results indicate that regular examinationof the immunological status of asbestos exposed workers could beof diagnostic and prognostic importance in the development ofasbestosis.

    Summary: In order to determine whether the appearance of occupationalasbestos is related to cleared asbestos bodies (AB) andmacrophages (AM) the occurrence of AB and AM in the sputum ofdiseased and non-diseased shipyard workers was investigated, ABwere significantly more frequent in the sputum of the diseasedgroup, while AM were more frequent in the sputum of thenon-diseased group. The findings of AB and AM in the sputum ofasbestos exposed individuals is an easy way to detect "in vivo"exposure to asbestos, and represents a good hallmark of exposurebut not a good hallmark of disease.

    Summary: The latest knowledge in pathophysiology of occupational asthma,as well as the novel guidelines for the diagnosis of occupationalasthma are reviewed. Each diagnostic method, a step in thelogical cascade in establishing the diagnosis of occupationalasthma is presented. All of these methods are applied routinelyat the Institute for Medical Research and occupational Health,University of Zagreb. Practising a subspecialist, individual andteam work approach in evaluation of each patient with suspectedoccupational asthma we have succeeded in establishing adiagnostic scheme for routine work which meets theinternationally accepted criteria, and are involved inestablishing these criteria at the European level.

Besides printed material in daily and weekly publications, professional papers, journals and books, an educational programme with videocasettes is proposed.

    Summary: The extent of management problems in invalids is emphasized. Apart from encouraging the invalids themselves to become involved in management, special measures should be taken to encourage firms to employ persons with psychophysical damage. It is suggested that a register is established for invalids at the level of region and country, in order to assist invalids to become motivated for work, employment and better status. Apart from invalids, a psychologist, physician and social worker, employers should also be included in the rehabilitation team.

    Summary: In 17 healthy volunteers, aged 17-25 years, bronchial reactivityto extracts of sage, gruzyan tea and dog rose was examined. In 15subjects nonspecific bronchial reactivity was examined bymetacholin test. Bronchial reaction was objectivized withparameters FEV1, FEF50, FEF25, Raw and SGaw. Extract of sagecaused acute bronchoconstriction at the level of the smaller andlarger airways, extract of gruzyan tea predominantly at the levelof the larger airways and extract of dog rose at the level of thesmaller airways.

    Summary: By means of the photoplethysmographic method and cold test, skinblood flow was recorded in the fingers of 42 chain-saw operatorsexposed to hand-arm vibration and 36 control subjects. Theobstructive form and loss of amplitude of the pulse wave form wasaccepted as pathological. A limit of a 90% fall in amplitudeafter the cold test is proposed as a criterion fordifferentiating normal from pathological vasospastic reactions inhand-arm vibration exposed subjects.

    Summary: Finger thermometry and cold provocation test were performed in 90chain-saw workers, grouped according to the Taylor-Pelmear scaleand 58 healthy controls. The test differentiated subjects withclinically manifest vibration-induced white fingers fromcontrols. With a discriminating theshold of 90% for recoveryrate, the method was sensitive and specific enough to verifyvibration-induced vasospastic disorders and therefore could berecommended for their surveillance.

    Summary: The paper deals with the basic characteristics of whole-bodyvibration and its adequate evaluation. Three standards foracceptable human exposure proposed by the InternationalOrganization for Standardization (ISO) and quantitativeparameters of vibration for some vehicles and machinery aregiven. The most pronounced long-term effect of whole-bodyvibration is damage to the spine. The peripheral and autonomicnervous, vestibular, vascular, digestive and female reproductivesystems are also liable to become affected.

    Summary: During a clinico-epidemiological study of the characteristics ofthe organism with increasing age, changes in the heart and bloodvessels were examined with particular attention to arterialhypertension and changes in the electrocardiogram in arepresentative sample of a population of older age. Arterialhypertension is more frequent in old age, and the danger ofcardiovascular disease is increased threefold, particularlycoronary heart disease, in relation to normotensive persons ofthe same age. The results indicate that criteria for biologicalage should be established on the basis of determined parametersof cardiac function, respiratory system and renal functions.

    Summary: The prevalence of ST-segment depression in a sample of thepopulation aged 35-54 years, amounted to 7.74%; in females 10.7%and in males 4.1%. After three years the frequency of ST-segmentdepression had increased, and in the total sample, now aged 38-57years, it was 9.56%; in females 12.6% and in males 6.25%. After13 years, in the same subjects, aged 48-67 years, the prevalenceof ST-segment depression was still more frequent in females,amounting to 19.0% and in males 12.1%. Significant correlationwas determined between ST-segment depression and a high R-waveleft (p<0.05) and Q-wave (p<0.01) in the electrocardiogram inmales, and in females significant correlation with a finding ofatrial fibrillation (p<0.05) and complete left branch block(p<0.001) in the electrocardiogram.

    Summary: The recent scientific evidence on smoking related disease showsthat breathing other people's tobacco smoke is a cause of seriousdisease including lung cancer. The paper deals with selectedoccupational safety and health regulations which are aimed atprotecting workers against passive smoking. Programs should beintroduced to stop the habit of smoking at the workplace, and investigation of the interaction between cigarette smokeand occupational exposure should be continued.

    Summary: A medical survey of 265 workers exposed to increasedconcentration of aldehydes revealed that 20-35% had symptoms ofeye, throat or skin irritation. In the group with a history ofasthma (68 U/ml) and in the group with skin disorders (mean=103U/ml) total serum IgE was increased. Smokers had higher IgEvalues (77 vs 51) and lower values of lung function tests thannonsmokers. We conclude that hypersensitive responses may beassociated with aldehyde exposure and/or smoking.

    Summary: A questionnaire on subjective complaints in connection with workconditions was applied for 49 video display operators.Non-ionizing and ionizing radiation emissions during normaloperation was within permissible levels. The results showed ahigh frequency of subjective complaints, significant differencesbetween age subgroups in some variables and significantcorrelation between the set of perceived ergonomic features andsubjective complaints.

    Summary: The occurrence of painful shoulder was examined in 40 metalworkers exposed to significant strain of the shoulders duringwork. The control group comprised workers of the same age, butwithout significant strain of the shoulders. Disease in theshoulders was diagnosed significantly more frequently in theexamined group (47.5% : 10.0% ; p<0.001). Pain in the cervicalspine was also significantly more frequently diagnosed in theexamined group (45.5% : 7.5%).

    Summary: With regard to earlier profession and method of work asignificant difference was determined in 57 examinee-invalids inwhom cervicobrachial syndrome (CBS) was the main reason for theirretirement, and 57 examinees in a control group, of the same sexand age. An unphysiological position of the body during work wasreported in 74% of the examinees with CBS and 50% of theexaminees without CBS (p<0.05). Identical movements during workwere reported in 79% of the examinees and 49% of the examinees inthe control group (p<0.05).

    Summary: By means of a clinical examination and X-ray the consequence ofmaltreatment was examined in 48 prisoners of war, previously heldin Srbo-Chetnik army camps. Only 16.7% of the prisoners had notbeen beaten. In 21% rheumatic disease was a direct result of themaltreatment in the camps. Seriously reduced work ability wasconfirmed in 51% of the prisoners, and invalidity retirement wasproposed in two subjects.

    Summary: The results are presented of an epidemiological study carried outin 1982, in which the frequency of chronic disease in thepopulation of Croatia was investigated. During the studyinjuries were also examined. The most frequent were fractures(3.9%) although there was a significant percentage of contusionsand commotion (1.5%) and gunshot wounds (1.5%). The mostfrequently injured part of the body was the hand (25.3%), thenthe lower leg (16%) and the chest and ribs (13.8%). During thewar in Croatia it is anticipated that up to 30% of injuries willbe connected with damage to peripheral nerves.

    Summary: Data are presented from a questionnaire on the work-chair whichwas carried out in 270 female workers, mean age 34.2 +/- 9.7years. Only 12% of subjects considered their work-chair to beexcellent; 9.5% considered their chair to be poor and 19.4%considered their chair to be extremely poor. In a workingenvironment with very favourable conditions 40% of the workerswere very satisfied with their chair, while in an environmentwith unsatisfactory working conditions 73% considered thework-chair to be poor.

    Summary: In addition to the standard criteria for diagnosis and follow-upof acute intermittent porphyria (uroporphyrin, coproporphyrin,delta-aminolaevulinic acid and porphobilinogen) in women ofreproductive age diagnostic procedures not previous used wereapplied. Women with acute intermittent porphyria were found tohave cortisol deficiency and elevated testosterone values. Thepresence of polycystic ovaries with anovulatory cycles was alsoestablished. It was concluded that apart from conventionaltherapy for acute intermittent porphyria cortisol compensationshould also be introduced, which has an effect on the patient'sspeedy recovery, working ability and quality of life.

    Summary: Body mass was examined in the defenders of Vukovar (N=71),Republic of Croatia. The sample consisted of a group of 33 malesmokers who had been imprisoned for 127.4+/-18.4. days (age31.6+/-7.2 y) and 38 male smokers imprisoned for 271.6+/-14.4days (age 30.8+/-6.5y.). A negative deviation from body massnomogram was registered in prisoners of war in relation to thecomparative group (5-16%).

    Summary: Reference values for FVC and FEV1 have been developed on a sampleof 909 healthy adult nonsmokers, men and women (age range18-86y). Lung function parameters were analysed as functions ofheight and age using full model multiple linear regressionanalysis. The reference values are reliable and can be used indaily medical practice.

    Summary: The possibility of the implementation of the McBride rating scalein invalidity evaluation was explored in a sample of retiredpersons. A sample of recently retired disabled men (N=71) wascalssified in groups (correctly and incorrectly assessed). Ratingscale was compared between the groups. There was a significantdifference in rating between the two groups of examinees. McBride's scale can be useful as an adjuvant instrument ininvalidity reevaluation.

    Summary: Chronic alcohol consumption produces a toxic effect on the liverleading to the disturbance of all metabolic processes in theliver. The type and degree of disturbance and their dependenceon daily alcohol consumption were investigated. Our subjectswere classified into groups according to mean daily alcoholintake. In order to evaluate the state of health of our subjectsand the degree of alcohol induced impairment, 27 variables weremeasured with particular reference to lipid and apoprotein Bmetabolism. According to our results determination of apoproteinB, A/G ratio, IgA and gama GT values are the indispensablelaboratory tests to be applied as screening procedures fordetecting liver damage and alcohol induced metabolic changes.

    Summary: Lung functions and diffusing capacity for carbon monoxide wereexamined in 299 shipyard workers occupationally exposed toasbestos. Workers were classified into groups in accordance withthe 1980 ILO International Classifications of Pneumoconiosis. With regard to the profusion of the parenchyma, workersclassified in category 2 had significantly lower FVC and FEV1values than the workers in categories O (p<0.01) and 1 (p<0.05). The changes detected on the radiographs of the lungs are earlyindicators of possible asbestos disease in shipyard workers.

    Summary: To determine the incidence of urinary incontinence among femaleworkers 383 women with different jobs from three firms wereexamined over a three-year period. The examination comprised asystematic gynaecological check-up and a questionnaire. Theestablished incidence was 4.7%. The number of births, physicalwork and body weight failed to show the expected impact, whereasage was highly significant in respect to the symptoms ofincontinence (p<0.001). The age-related degeneration andinvolution of the genital tract can be cured if treatment isstarted early enough. Regression of symptoms was noticed in 66%of the treated women.

    Summary: Respiratory function was examined in 110 workers occupationallyexposed to asbestos-cement dust for periods of 7 to 34 years. Inrelation to the length of exposure the workers with exposurelonger than 16 years had significantly lower FVC and FEV1(p<0.001). This phenomena only occurred in smokers.Workers exposed for longer than 25 years, in relation to thoseworkers exposed for up to 16 years, had significantly lowerFEF25-75 (p<0.001), both smokers and nonsmokers. Deteriorationwas significantly greater than anticipated with regard toincrease in age.

    Summary: The aim of the study was to establish the level of thenonspecific nasal responsiveness (modified method of van Wijk etal. Clin Allergy 1987;17:563) and the influence of occupationalexposure to irritants, atopy and smoking habit in 84 workersoccupationally exposed to irritants and to compare it to it'slevel in healthy subjects (N=60). Significantly higherpercentage of hiperreactors was found in the group of workers. Asignificantly icreased nasal responsiveness was measured whenworkers were exposed to irritants, and higher smoking rate wasassociated with the lower levels of nasal responsiveness.

    Summary: Allergic reactions in relation to respiratory symptoms andventilatory capacity were studied in 41 textile workers employedin the processing of jute and sisal. Only 5% of the jute workersand 9.5% of the sisal workers demonstrated positive skinreactions to jute and sisal allergens. Increased IgE was foundin 9.8% of the textile workers. Among 35 control workers 11.4%reacted with positive skin reactions to the jute or sisalallergen and 2.9% had increased IgE.

    Summary: Immunological testing was carried out in 24 cotton workers and 30control workers. Skin prick testing was performed with allergensprepared from cotton dust and cotton seed. Skin tests werepositive in 33.3% of cotton workers, and IgE level was increasedin 62.5% of the cotton workers with positive skin tests. Onlyone cotton worker with positive skin tests had symptoms ofbyssinosis. Among control workers only one had positive skintests and none had increased IgE.     Summary: The relationship between immunological status and respiratoryfindings was studied in 42 hemp workers and in 49 controlworkers. The prevalence of positive prick skin tests wasrecorded for a mixture of hemp and flax (64%), followed by flax(48%),hemp on combing machine 41%), hemp on carding machine(38%), hemp on spinning and weaving machines (33%), and hemp on asoftening machine (20%). The prevalence of positive skin testsin control workers was smaller and varied from 21 to 5%. Increased IgE was found in 35.7% of the hemp workers and in 5% ofthe controls (p<0.05).     Summary: Immunological and respiratory findings were studied in 19 malesoybean workers and 20 control workers. All soybean workers hadpositive intradermal skin reactions to soybean extract, as did19/20 control workers. Similarly 18/19 soy workers reacted to soyantigen prepared after separation from oil, but only 3/19 to soylecitin antigen and one to soy oil antigen. The majority of soyworkers 13/19 reacted to house dust. Only 3/19 soy workers hadincreased levels of soy-specific IgE. Workers with increasedspecific serum IgE or positive skin tests to house dust did nothave more symptoms than workers with negative tests. In vitrostudies demonstrated a dose related contractile effect ofwater-soluble soybean shell extract.

    Summary: Immunological reactions and respiratory symptoms were examined in35 animal food workers. Positive skin prick test reactionsoccurred to fish flour 82.9%, carotene 77.1%, cornflour 65.7%,clover 62.9%, sunflower 34.3%, chicken meat 31.4%, soy 28.6% andyeast 22.7%. The IgE serum level was increased in 40% of theanimal food workers and in 2.6% of the controls. The workers withpositive skin tests to fish flour demonstrated significantlylarger FEF25 reductions than those with negative ests. Anextract of animal food caused constriction of isolated guinea pigtracheal smooth muscle in vitro.

    Summary: The possible occurrence of occupational contact dermatitis wasassessed in 79 pupils from a metal industry school. Afterleaving school the pupils were expected to work in the metalindustry, exposed to hard metal salts. Clinical examinationshowed all pupils to be in good health. None had a family orpersonal history of metal sensitivity. Patch testing withcontact allergens of hard metal salts caused positive reactionsto nickel, chromium and cobalt in 28 out of 79 pupils, althoughthey were still not occupationally exposed.

    Summary: Long-term, low levels of antineoplastic (AN) drugs and ionizingradiation (IR) in the work environment may have genotoxic effectson chromosomes in personnel. The effects of IR and the measuresto prevent them are well known in occupational medicine. However, for workers exposed to AN drugs there are no protectivemeasures. Metaphase chromosome preparations were analyzed in 126nurses and radiology technicians.     Summary: The history of dynamometric registration of maximal voluntary isometric strength of hand grip is described. Results are presented o epidemiological studies in a sample of the population aged from 25 to 45 years, in which right hand grip strength was recorded by using a mechanical hand dynamometer: males (n=626) 105.7 +/- 21.72 kg; females (n=636) 52.12 +/- 16.18. In the second group of examinees, males with cervicobrachial syndrome (n=70), values for the dominant hand amounted to only 36.9 +/- 29.7 kg. In the sample of females (n=50) values amounted to 15.4 +/- 19.2 kg, and in healthy subjects 51.4 +/- 20.5 kg. The need to establish standard values for hand dynamometry is emphasized for the active working population and older persons.

    Summary: The results of an investigation of fatigue and pain in the locomotor system are presented. The investigation comprised a group of workers (N=56) who spend most of their working hours at video display terminals (VDTs), and a control group of 39 workers with a mainly free work rhythm and insignificant physical strain. Fatigue and pain occurred frequently in the muscles and joints of the workers working on VDTs (78.8:59%; p>0.05). On the basis of a rheumatological examination cervico-brachial syndrome was determined more frequently in the VDT group (23.2% : 10.3%), while low back pain was diagnosed equally in both groups (17.5%). According to a subjective estimation disorders were associated with unsatisfactory work conditions in 46.9% of VDT workers and in 17.9% of the control group. An ergonomic approach to work with VDTs, relaxation and isometric exercises at the workplace is emphasized.

    Summary: After 272 days of imprisonment, 762 defenders of Vukovar were exchanged. Of this number 636 reported for a systematic examination. One hundred and fifty-nine were randomly selected for this study. As many as 62.4% were beaten on the whole body. Occasionally or constant heachaches lasting longer than one month was reported by 62.8% of them. The occurrence of headaches was statistically significant (p<0.0001) and correlated to the blows received to the head. One hundred and eleven (69.8%) of those examined suffered from dizziness, which was statistically correlated with the blows to the neck (p<0.0001). Impaired hearing was found in 29.5% of all subjects.

    Summary: The paper presents data on the health disorders after imprisonment of 159 defending Croatian soldiers captured by the Serbo Chetnik Army during the battle of Vukovar. On the basis of anamnestic data, Index of Torture in the area of the head and body and functional tests, we estimated that only 232 (14%) of the ex-prisoners were unfit for any kind of work. Treatment or rehabilitation was necessary for 36 (22.6%), with restrictions 29 (18.2%) were fit for their work, and change of job was recommended for 41 (25.8%) of all those examined. The need for a further follow-up of the health status and adequate treatment and rehabilitation of this group of people is emphasized.

    Summary: This study examines the current prevalence of psychiatric disorders occurring concomitantly with PTSD in two groups of released POWs: in a group of patients examined shortly after release (<50 days; N=40), and in a group of patients examined later (>100 days; N=12). The data were analyzed by descriptive statistive methods, by the hi-square test and by the Wilcoxon-Mann-Whitney test. The differences in frequencies of multiple disorders in two groups, investigated by the hi-square test, are statistically significant. The age and length of captivity in these two groups were examined by the Wilcoxon-Mann-Whitney test. Differences in age were not significant; those for the length of captivity were however statistically significant: p<0.01. The results indicate an early tendency of the occurrence of concurrent psychiatric disorders among former prisoners of war with PTSD, in a sub-group of patients that for various reasons did not receive psychiatric help early. It should be emphasized that in the group of late examinees, among seven patients with conconitant disorders, four had a diagnosis of alcoholism and one of benzodiazepine abuse. This indicates that in the group of patients (late examinees) with secondary disorders, an important problem is substance abuse disorders, especially alcoholism.

    Summary: A considerable number of people released from camps manifest psychiatric disorders, among which posttraumatic stress disorder (PTSD) is especially frequent. It should be expected that among some of them these disorders will be long term and among others, who do not manifest any symptoms now or these are minimal, the disorders can occur later, or even much later. In view of the large number of these people, their age, duration of disorders and high percentage of disability, the problem of working ability of these patients could be a significant health, social, economic and human problem in the future. So we have good reason to be afraid of a real time bomb whose long term effect cannot be estimated at this moment. In this study some of the problems that may be encountered in the future are presented. The necessity of organizing a prevention and treatment program has been emphasiyed. Some principles regarding the evaluation of disability of patients with psychiatric disorders are pointed out. It is stressed that: 1: the diagnosis is an important factor in estimating working capacity, but not the only one, 2. motivation for recovery and work can be a crucial factor for coming back to work; 3. a complete evaluation includes a detailed history, treatment and rehabilitation.

    Summary: In a sample of prisoners of was (POW) (N=80), who had been imprisoned in concentration camps for four and nine months an examination of body mass, BMI, skinfolds and biochemical indicators was carried out. Data were compared with a nomogram and data on participants in a study of chronic diseases carried out in 1976. It would appear that BMI is the best indicator of the loss of body mass during war conditions.

    Summary: During a systematic medical examination of the locomotor system in 325 prisoners of war in Croatia (age X=34.2 +/- 10.2 years) the medical prognosis of work ability was given. Initially a temporary prognosis was given and after a comprehensive treatment and rehabilitation a permanent evaluation of work ability was issued. Work ability was preserved in approximately 44.9% of subjects, while work ability was completely lost in 22.1%. Four characteristic case histories are presented showing a significantly reduced functional capacity of the locomotor system as a result of maltreatment in Serbo-Chetnik camps. For two of the prisoners of war, aged 54 and 44 years respectively, invalidity retirement was proposed, and in two younger prisoners a temporary reduction in work and fighting ability was found.

    Summary: From 15th November 1991 the Department for Occupational Health has participated in the systemic assessment of the health status of prisoners of war in Croatia, who had been imprisoned for most of the time in camps run by the Serbo Chetnic Army. Out of the 325 prisoners examined because of the disease of lthe locomotor system 60.3% were members of the Reserve Corps of the Croatian Army. The majority (81.8%) had not earlier had rheumatic diseases. During the examination in rheumatologic department the most frequent diagnoses were painful lumbar syndrome (25%) and contusions of the trunk (13.5%). High frequency of periarthritis humeroscapular (5.8%) and fibrositis can be correlated to maltreatment in the camps, by blows, coldness and starvation.

    Summary: Not so long ago the unfavourable influence of antineoplastic drugs on personnel exposed to them was reported. Since then research has been carried out with the aim of clarifying and objectivizing this occupational danger. Agreement was reached on the need for measures of protection at work and regular medical lcheck-ups of personnel exposed to these cancerogenic substances, which can also have a mutagenic and teratogenic effect. This paper presents the possible consequences of the effect of antineoplastic drugs in long-term exposure to low concentrations, and the means of protection from the unfavourable effect of exposure. In our country such means of protection and regular medical check-ups of personnel are not carried out in any oncological department, which is a serious oversight, particularly if we accept the fact that today 70-90% of all carcinomas are due to agents within our environment and diet.

    Summary: Bronchial reactivity to methacholine was examined in three groups of smokers (N=93): subjects with secondary inactive pulmonary tuberculosis (TPI) 2 years after the onset of treatment (N=44), subjects with asymptomatic chronic bronchitis (N=25) and healthy controls (N=24). Mean RC20 FEV1 was 9.54 (SD 6.05), 11.12 (SD 5.42) and 13.74 (SD 4.7), respectively (tuberculosis vs control p<0.02). According to the arbitrary criteria for bronchial asthma nonspecific hyperreactivity RC20 <8.0 mg/ml) was present in 50% of subjects with TPI in 37.5% of bronchitis patients and in 16% of control subjects. No difference was found with regard to RC20 FEV1 the chemotherapeutic regimen (9 or 12 months of theralpy), results of tuberculin test and X-ray findings determined before the onset of treatment. Mahalanobis statistical analysis with respect to smoking indexc, basal ventilatory function tests and nonspecific bronchial reactivity grouped together healthy volunteers and subjects with a history of pulmonary tuberculosis apart from the group with asymptomatic chronic bronchitis.

    Summary: Subclinical poisoning with inorganic lead (SPL) today is more frequent than acute poisoning. In an exposed population it can have a chronic effect on health, which is impossible to evaluate by standard clinical examinations. However there are specific tests by which intoxication of the organism by inorganic lead can be proved. The methods of choice are determination of blood lead (PbB), erythrocyte protoporphyrin (EP) and dehydratase delta aminolevulinic acid (ALAD). None of the 87 workers exposed to inorganic lead, who were examined in the Outpatient Clinic for Occupational Health, Institute for Medical Research and Occupational Health, had clinical signs of lead poisoning. Only 25 patients (25.7%) had values of PbB 1.95 umol/L or less; L = 2.53 +/- 0.č89 umol/L, range 0.84-4.91 umol/L. The aim of this study was to indicate the need for specific examinations of persons exposed to inorganic lead, regardless of clinical signs or nonexistence of subjective symptoms.

    Summary: During the war in Croatia 34.05% of the 8882 victims were civilians. As many as 62% of those forcedly taken away, imprisoned and missing were civilians. More than 3,500 parents were left by the killed defends, of which one third are older than 60 years. One third of those questioned feel the need for company, 24% consider that permanent housing is a priority and 26% consider that additional home help is a priority. Possible legal intervention, better humanitary care and psychosocial and spiritual support is proposed.

    Summary: As a part of medical surveillance chromosome aberration assay was proceeded on 30 persons occupationally exposed to ionizing radiation working in the Department of Nuclear Medicine. In eight persons bicentric chromosomes were found. On the basis of anamnestic data it could be presumed that the appearance of these pathological changes might be caused by diagnostic irradiation and not by occupational exposition. We suggest that for such persons chromosome aberration assay should be performed before any diagnostic procedures, which can affect genetic pool.

    Summary: Nine individuals occupationally exposed to X-ray radiation doses of between 8.5-58.2 mGy were monitored by haematologic parameters, as well as chromosomal aberrations. A physical examination and haematologic parameters did not show any changes which could be linked to ionizing radiation.     Summary: In a group of 32 radar technicians, followed up during the periodof 5 years, psychological and EEG analysis was performed. Results obtained on CRD series for all subjects are within thelimits expected for age and education. They suggest the "processof accommodation" to the aparatures to be in progress.     Summary: Measuring the vasoconstriction as a response to cold stress bydigital photoplethysmography and vasodilatite ability by skinthermometry were performed in 29 male chain-saw workers and 16healthy persons. With a 75% reduction in photoplethysmographicamplitude, the sensitivity to detect vibration-induced whitefingers was 62% and specificity 58%.     Summary: On the basis of an investigation in 317 workers who spend most ofthe time at work in a sitting position, only 22.1% did not reportfatigue and/or pain in the cervical and lumbal spine. In 10% ofthe workers such pain occurs as early as the third hour of work,while the most intense pain occurs in the sixth hour of work onvideoterminals or typewriters. The pain was significantly morefrequent in the right shoulder and right hand than in the left(16.4% : 7.9% in shoulders and 24.5% : 8.2% in hands).

    Summary: In an epidemiological study of samples of a middle-agedpopulation in Croatia increased frequency of arthrosis in thejoints of the extremities was registered in a rural population -69% compared to 54% in a non-agriculatural population. Unphysiological positions of the body and repeated hand movementswere most frequent in females, who suffered from cervicobrachialsyndrome (80% : 50% ; p<0.01). Fatigue and/or pain in thecervical spine was reported by 87% of female workers onvideoterminals, who connect this to the method of work.

    Summary: In a group of 290 prisoners, 99% of whom were young men (26.4 +/-6.5 years) 78.6% were members of the Croatian Army. The mostfrequent diagnosis was back pain (50.5%). As earlier back painwas reported in only 21% of the prisoners 29.5% can be considereda direct result of maltreatment in the camps. A CT in 1.7% ofthe prisoners confirmed protrusion of the intervertebral disk,and X-rays in 35% of those examined showed fractured ribs. Relatively frequent fibrositis (4.6%) is considered the result ofchills and depression.

    Summary: We established the existence of impaired reactivity in pressworkshop workers in 44.7% of cases (N=38) (PC50 Rt=3.08 +/- 0.9mg/ml histamine) while in mounting workshop workers (controlsN=36) hyperreactivity was much less (5.5%) (PC50 Rt=12.32 +/- 4.8mg/ml histamine) ASTROGRAPH-Chest Corp.     Summary: Anxiety index (IA) was the highest rank in chronic bronchiticsand and slightly lower with symptoms of asthma (sample: 823 M,725 F adults). Females on the whole had a higher IA rank. IA didnot correlate significantly with the values of FEV1. We foundsignificant correlation between IA and VC% in the population offemale-chronic bronchitics, and the control group of females.Similar data have been reported in clinical investigations.

    Summary: The study presents data for 1987. In a population from Croatiathe incidence of chronic cough amounted to 128/100000 (92.38 M,36.08 F) which is similar to data from the pneumophtisiologicalservice (164/100000 inhabitants). On the basis of the foregoingstudy it can be concluded that the frequency of chronicbronchitis is similar to the frequency in other Europeancountries and that a specific register should be developed forfollowing-up such patients.

    Summary: The aim of the study was to identify and define differencesbetween diseased and non-diseased asbestos exposed shipyardworkers. Both data on medical histories and clinical andlaboratory surveys were statistically evaluated. When comparedto the non-diseased group, a significant increase in erythrocytesedimentation rate, monocyte number, gamma-globulin andimmunoglobulin levels was found in diseased subjects, followed bya decrease in segmented leukocyte percentage. These findingsshow the influence of locally inhaled asbestos fibers on systemiccirculation.

    Summary: In order to determine whether the appearance of occupationalasbestosis is related to cleared asbestos bodies (AB) andmacrophages (AM) the occurrence of AB and AM in the sputum ofdiseased and non-diseased workers was investigated.     Summary: By using standard physical examinations a group of 52 workers,occupationally exposed to pesticides, were examined and theresults evaluated. Duration of exposure varied from 3.5 to 27years. The control group comprised a matched group of workersnot exposed to pesticides at the workplace. A statisticallysignificant difference was seen between the groups with regard totremor, muscular tonus and ability to retain balance in astanding position, and within the group of pesticide exposedworkers in respect to the length of exposure.

    Summary: The object of the study was to test the hypothesis that certainneurobehavioral abnormalities might be present in workerschronically exposed to pesticides. A group of 32 patients from afactory producing pesticides were examined. Reaction times weremeasured by Complex Reaction Meter Drenovac (CRD system). Statistical analysis of results showed that age, years of smokingand length of exposure to pesticides result in prolonged reactiontimes in workers chronically exposed to pesticides.

    Summary: The purpose of the study was to examine the relation betweenepilepsy and other diseases in disabled workers and to examinethe dynamics of this relation in the last three years. Arepresentative sample was selected out of 60 subjects (disabledworkers) who had retired in 1988, 1989 and 1990 and in whomepilepsy was one of the diagnoses. The results of the studyshowed that epilepsy was the main reason for retirement in 20subjects in 1988, 26 in 1989 and 32 in 1990.

    Summary: Out of 638 prisoners of war, examined up until the end of 1992 in the Department of Occupational Health, Institute for Medical Research and Occupational Health, 51% complained of disorders of the locomotor system. Arthrosis of the joints was diagnosed in 8.4%, and contusion in 6.2% Although the subjects were relatively young, 34.2 +/- 10.2 years, the share of arthrosis was significantly greater than anticipated for this age group. Consequently arthrosis can be causally connected with physical maltreatment in the camps.

    Summary: A group of 66 farmers (P) was compared with 65 tractor drivers (T) working in similar conditions in the public sector and 38 teachers (N) with a free work rhythm and satisfactory microclimatic conditions. In 6.1% of the farmers and only 1.5% of the tractor drivers, the hips were the most burdened part of the body. Restricted rotation was registered in 21.2% (P), 13.8% (T) and 5.3% (N) (p<0.05). Coxarthrosis was diagnosed in 7.6% (P), 4.6% (T) and only 2.6% (N). The possible effect of work methods, physical burdening and vibration are discussed with regard to the increased frequency of coxarthrosis in farmers.

    Summary: In a random sample of 53 patients, examined in the Rheumatology Out-patient Department because of painful joints, possible risk factors for the development of gonarthrosis were examined. A control group, matched according to age and sex, comprised 32 men and 21 women, with painful syndrome of the spine. Gonarthrosis occurred significantly earlier in women (53.2 +/- 5.2 years, p<0.01). Subjects with gonarthrosis reported that at work they either stand or walk (p<0.05), and some reported trauma of the lower leg 13.2% : 3.8%, in the control group; p<0.05. Adiposity was significantly more frequently registered in women with gonarthrosis (18.2% : 9% with adiposity but without gonarthrosis). Work ability was impaired more than in athe control group.

    Summary: The significant incidence of rheumatic disease is connected with nonergonomic strain at work. Of the workers who predominantly sit at work 58% complain of pain in the cervical spine, and 17% of pain in the shoulder joints. Pain in the hands, particularly the right hand are frequent (24.5% : 8.2% in the left). As many as 52% of workers who burden the shoulder joints during the technological process, complained of pain in the joints of the upper limbs, and only 22.5% of subjects in the control group of met workers (p<0.01). A clinical finding indicating diseased shoulders was found during an examination in 47.5% of the examined group and 10.0% of the control group, indicating the need to eliminate known risk factors arising from nonergonomic strain.

    Summary: Out of 55 professional musicians, who spend more than 8 hours a day sitting, 83% complkained of pain in the locomotor system. Work chairs were extremely unsuitable. Out of 337 workers on videoterminals, who work in similar conditions but use better chairs, 21% feel fatigue during work and as many as 60% fatigue and pain in one segment of the locomotor system. Only 33% were satisfied with the work chair. Unsuitable chair back was reported by 75% and chair angle by 64% of those questioned. When designing the work chair antrhopometric characteristics of the user and ergonomic principles should be respected.

    Summary: Q questionnaire was completed on fatigue and pain in the cervical spine and joints of the upper extremities for 59 female workers in the meat industry, aged 33.8 +/- 5.2 years and 14.2 +/- 5.1 years of service, who work in unsatisfactory ergonomic and microclimatic conditions. Pain and fatigue was felt at the same time by 98%, and only 25% had no disorders of the locomotor system. In 66% of the women disorders increased during work and were more intensive during the sixth hour. In 37% of subjects the disorders did not disappear after work.

    Summary: The group examined comprised patients with clearly expressed cervicobrachial syndrome (CBS). The control group, matlched by age and sex, comprised patients without CBS or disease of the upper extremities. Grip strength of the right hand, measured by mechanical dynamometry, was 48.6 +/- 34.41 kg, compared to 51.8 +/- 35.1 kg for the left; p<0.05. Values in the control group were significantly higher (for the right hand 73.0 +/- 36.4 kg, left hand 67.9 +/- 32.3 kg; p<0.01). Markedly reduced strength was determined in 43% of subjects for the right hand and 20% of subjects for the left, which greatly reduced the ability to work safely.

    Summary: Between m. sternocleidomastoideus and deep vertebral (fascie) n. accesorius may be compressed, resulting in the occurrence of motor canalicular syndrome. The results are presented of treatment for pain in the cervical spine, which the patients connect with an unsatisfactory position of the head during sleep. Out of a group of 40 patients 20 were treated by massage only and nonsteroid antiflogistic drugs, and the remaining 20 by relieving the pressure on the n. accessorius by the "Jouni test". In the second group speedier recovery was achieve.

    Summary: During an investigation of chronic disease in a sample of 3,369 middle-aged inhabitants in Croatia we registered the frequency of injuries in 8.4%. Women were twice less likely than men to be exposed to injury. Fractures were the most frequently diagnosed - 46.3% of all injuries. During a medical examination of the locomotor system arthrotic changes in the extremities were graded. The possibility of correlation between macrotrauma (particularly fractures of the lower extremities) and arthrotic changes is discussed.

    Summary: By an active epidemiological approach, applied in a sample of the population of Croatia (N=2,835) age range 35-55 years, it was found that every fourth woman and fifth mean suffered from spinal disease. On the basis of an examdination by a general practitioner diseases of the cervical spine were diagnozed more often in women than in men (21% : 15% ; p<0.01). In 317 workers on videoterminals (VDU) cervicobrachial syndrome was diagnozed in 32% and in subjects in the control group with a free work rhythm 20%; p<0.01). Preventive relaxation exercises during work on VDUs, recreation programme, gymnastics and ergonomic adaptation are elaborated.

    Summary: Out of 1,550 prisoners of war (POWs) 26% were examined because of rheumatic disorders. Only 18% of those examined complained of earlier rheumatic disease, and of this number only 7.2% had been medically treated because of the disease. Only 1.3% of POWs were not beaten. Back pain was most frequently diagnosed (44.1%), chestl pain (21.8%) and pain in the cervical spine (17.8%). High frequency of fibromyalgia, humeroscapular periarthritis and arthroses in relatively young patients (34.6 +/- 10.2 years) was connected with unsatisfactory climatic conditions and physical maltreatment in the camps. Only 44.9% of the POWs were able to continue their earlier employment.

    Summary: Nonoccupational exposure to formaldehyde may reach levels which exceed occupational standards and produce subjective symptoms and perhaps worsen laboratory findings. This survey of 276 employees demonstrate seemingly high prevalence of headache, irritation and asthmalike symptoma and skin disorders. Measurement of indoor air atmosphere showed mean values of 1-199 ugm-3 for formaldehyde. The occurrence of asthmalike symptoms was related to smoking habit and to hemoglobin level. Skin disorders was related to erythrocyte sedimentation rate and inversely related to immunoglobulin E. Observation presented suggests that immune system may be suppressed rather than activated in some chemically sensitive workers.

    Summary: The aim was to study the effect of occupational exposure to irritants, smoking and atopy on nonspecific nasal responsiveness. Our results suggest that nasal trouble in workers exposed even to low concentrations of irritants may be associated with nonspecific nasal hyperresponsiveness. The method proved to be a good diagnostic tool and could be used in monitoring the effect of occupational respiratory hazards on respiratory tract (health monitoring).

    Summary: In a group of children with obstructive expiratory syndrome (OES) and healthy children (N=136) indicators of maximal expiratory flow were examined. The results of the examination in both groups of subjects indicated good discrimination of V max tests depending on the effort (FEV1,FEF) and better discrimination of V max. tests independently of effort (FEF 50, FEF 25, FEF 25-75). During estimation of obstruction in children it is unnecessary to use expensive equipment and complex tests.

    Summary: Preemployment testing for atopy and bronchial hyperreactivity (BHR) has been done on 1010 subjects before being admitted for work in high risk working places. 414 (41%) of them were educated for high risk occupations. For nonadmittance were:: positive skin-prick test to one or more allergens and/or increased global and/or specific IgE or BHR. 10% of all subjects and 9% of subjects educated for high risk occupations had one or more positive criteria.

    Summary: The sample comprised 48 smokers and 47 non-smokers (age 28.0 +/- 11.0 yrs). Smoking index was 144.35 (range:20-750). Examination of sensitivity and nasal non-specific reactivity was carried out with histamine diphosphate. Slope dose-response was not significantly lower in smokers in relation to non-smokers (FEV 1, MEF 50). PC (nasal non-specific reactivity) correlated with smoking index. Slope dose-response correlated with basic ventilatory functions.

    Summary: Visual evoked potentials (VEP, cognitive evoked potentials(VEP-P300) and brain stem evoked potentials (BAEP) were performedin two groups of subjects: 49 workers in a printing-press and 59controls. The extent of exposure to toluene was evaluated bydetermining toluene concentrations in peripheral blood andhippuric acid and o-cresol u urine.     Summary: In this study we have investigated the level of nonspecific nasal reactivity in 60 healthy subjects, 34 subjects with pollinosis and 86 workers professionally exposed to irritants. Nonspecific nasal provocation with nasal resistance as a parameter for evaluation of the response expressed as provocation concentrations a quantitative diagnostic method with good reproducibility and high specificity and sensitivity. We showed a significant rise in nonspecific nasal reactivity during the period of professional exposure to irritants.

    Summary: In this study qualitative and quantitative characteristics of the digitopalmar dermatoglyphs in patients with manic-depressive disorder,bipolar type,were analysed and compared with the same characteristics among healthy examinees.The sample comprised a group of hospitalized patients of both sexes and of different age (N=150; M=92; F=58) with the diagnosis of manic-depressive disorder according to the criteria from the DSM-III-R classification.A control group comprised 400 phenotypically healthy subjects.Digitopalmar prints were taken using the Cummins and Midlo method.The quantitative characteristics of the digitopalmar dermatoglyphs were analysed by the methods of descriptive statistics,multivariate and univariate analysis of variance and canonical discriminant analysis.The quality of discrimination was evaluated by classifying the examinees according to the extracted classification function.The qualitative characteristics of the digitopalmar dermatoglyphs were presented by the relative frequencies of the finger and palm patterns.Statistical difference between the groups was evaluated by the chi-squared test.Evaluation of significance of the differences in frequencies of various patterns of palmar dermatoglyphics between the patients with manic-depressive disorder and healthy examinees shows statistically significant differences among males on the 3rd P<0.01) and 4th (P<0.001) interdigital areas as well as on the hypothenar (P<0.01).Among females there are statistically significant differences in the 4th (P<0.001) interdigital area as well as on the hypothenar (P<0.01).The differences occur in the medial parts of the palm only,being especially significant (at the level: P<0.001) in the 4th interdigital area,both among males and females.Total finger ridge count is lower in the group of patients with manic-depressive disorder than in the group of healthy examinees,among males and females alike.Correlation analysis between discriminating variables and canonical discriminant functions in the group of males indicates that the primary discriminators among males are left atd angle and ridge count between triradii Ża® and Żb®.Among females the best discriminators are quantitative characteristics of the palmar dermatoglyphs,above all the ridge count between triradii Ża® and Żb®; and between Żc® and Żd® on both hands.Classification of the examinees according to the discriminant function indicates that in the group of patients it is possible to classify correctly 82.60% of males and 81.00% of females.By the methods of dermatoglyphic analyses it is possible to discriminate a group of patients with manic-depressive disorder,bipolar type,from a group of phenotypically healthy examinees.
