Summary: Concept of "Croatian Model", model of diabetes health care
could be useful in organization of health care for other non-communicable
diseases while the most of risk factors are simmilar to the risk factors
in diabetes mellitus. The project is elaborated regarding appropriate
methodology and evaluation of such a model, with the purpose to adjust and
implement in other environments. "Croatian Model" is the basic and
important part of European strategy for diabetes mellitus accepted by
Declaration from St. Vincent.

Research goals: 1. To initiate programs of diabetes detection, with
emphasis onself-protection. An early diabetes detection drive was carried
out in a transport firm, the prevalence of diabetes was 0.8%, the
prevalence of IGT was 1.6%. 2. To determine the prevalence of IDDM risk
markers in the population of Croatia. The study is in progress (12, 15).
3. To determine the prevalence of diabetic complications inpersons with
IDDM. A sample of 140 patients was examined,providing a data base for
analysing associations between complications and risk factors. 4. To
reduce the incidence of blindness in diabetic patients byone third. 5. To
reduce the incidence of end-stage renal disease in diabetic patients by at
least one third. 6. To reduce by half the incidence of foot amputation due
to diabetic gangrene. 7. To reduce coronary heart disease morbidity and
mortality in diabetic patients. Due to the natural history of diabetic
complications, it si too early to expect results, but themethodology of
evaluation is being developed (6, 7, 11, 14). 8. To achieve in diabetic
pregnancy a pregnancy outcome similar to that of the nondiabetic
population. In the population ofZagreb, the outcome of pregnancy in
diabetic women is similar to that in the gneral population, due to well
organized services. 9. To develop appropriate technology in the diagnosis,
follow-up and self-monitoring of diabetes. A simple method for diagnosis
and follow-up of diabetes has been developed, particularly suitable for
areas without diabetes services (10). Aid to displaced and refugee
diabetic patients has been organized. The Croatian Model of diabetes
health care has been organized. The Croatian Model of diabetes health care
has proved effective in war circumstance (2, 5, 8). The effect of
war-induced stress on the stress hormones and glycemia control in diabetic
patients has been investigated (1, 3, 4, 9). The format of the existing
Diabetes Registry has been modified according to the European Diabetes
program (EURODIABETA). 10. To develop programs of continuing education of
secondary care personnel, education of primary health care teams on the
methodology of small-group patient education, continuing education of
primary care in the field of diabetes and its complications. Biannually
(except in 1991), education of health personnel was carried out in the
form of meetings with various topics. A course on patient education was
held for primary care teams (13, 16). The "Patient guide to diabetes care"
wastranslated into Croatian. The Institute for Diabetes participated in
the formulation of the European Consensus of IDDM Treatment. Both the IDDM
and NIDDM Consensus Report were translated into Croatian and adapted to
local circumstances. An educational meeting for persons working in
diabetes centres and consulting units was held in every county. 11. To
develop a national program for the education of nurses in the field of
diabetes. The "LEMON" Project was started in Croatia, as part of the WHO
project to develop high quality learning material for nurses in Central
and Eastern Europe.
