Title: How geat is risk for the newborns with increased IgE to
get atopy during the first year of life

Summary: During the prospective study of screening the total IgE
takenfrom the cord venous blood of 205 newborns, it has been found outthat
in the 41% the IgE concentrations were >/ 0,9 KJ/L and inthe 37% their
parents had allergic disorders.
In the courese of the first year of life, 32 newborns got sick.Besides the
changes's in skin, the respiratory system was themost frequent shock-organ.
AAllergy in family, as a possible risk factor, hasn't been soinfluential
for the clinical manifestation of aatopy during thefirst year of life. But,
there is a significant clinicalcorrelation between increased total IgE
concentrations and atopyin the children of our aarea.

Summary: In the last 10 years, the milk formulas have been developed
whichdecrease the quantity of taken antigens and thus they minimizethe risk
of sensibilization. Accordingly, in the case when motherdoesn't have enough
brast milk, pediatricians are to choose oneof the many, so caled,
hypoallergic formulas, against a greatatopy risk. The hydrolized formula
has been developed with aim todecrease of eliminate allergenic features of
cow's milk proteins.
We have been observing 63 infants who were admitted to clinicalhospital for
children's disease "Kantrida" Rijeka, in the courseof 1992, for the reason
of relapsed obstructive bronchitis,changes in skin (uricaria, oedemas,
atopic dermatitis) and colis.
During their hospitalization, the infants were fed onhypoallergic milk
(hydrolized) ox casein, hydrolyzed whey,hydroized soy proteins and cow's
milk). The observed childrenwere devided in two groups, accoonding their
age (months). Thefirst croup consisted of the infants up to 3 months as
they hadbeen fed exclusively on milk before thier hospitalization.
Thesecond group consisted of the infants over 3 months and they hadbeen fed
oon mixed food.
In the first group, 48,3% of the patients had allergicmanifestations in the
form of urticaria and/or oedemas, ad in the38,7% in the form of respiratory
disorders.
61,2% of the infants had IgE valves over 10 KJ/L.
In the second group respiratory disorders dominated (78,1%). Itis to be
said that all the babies had no problems withhypoallergic milk.

Summary: We have analyzed the total IgE concentrations taken from
the cordvenous blood of the 566 uneselected newborns anual earlyappearance
of allergic diseases in children. The IgE values equalor greater than 0,9
KJ/L haave been marked as increased anualsuch values have been found in
41,2% of our subjects. Thosechildren have been considered the candidates
with great risk forearly occuring of allergic diseases. Following up of the
childrenduring thier first year of life, we have notices that 15% of allthe
children and 53,1% of the children with incveared IgEconcentrations at
birth, got some form of aallergic disease. Themost frequent forms of
allergc manifestations were skin changes(28,7%) then, respiratory system
problems, and gasterointestinaldisorders, as a part of other chinical
pictures, were on the 5thplace (6,3%).

Summary: Mother's smoke during pregnancy, aas a possible risk
factorresponsible for increasing of the total IgE concentrations
inchildren, has been of the parameters observed in this prospectivestudy.
Among our respondents, mother - smokers during pregnancyprevailed (35,62%).
In 16,4% of the children up to 2 years oldwhose mothers smoke, there are
clinical pictures of skin andrespiratory system allergy. In the group of
the children withnon-allergic mother-smoking, it is significantly oreater
numberof children whose reagins are increased (IgE >/ 0,9KJ/L).

Summary: The three years study involved 228 children with increased
valuesof reagins (>/ 0,9 KJ/L). IgE values determined for umbilicalcord
vein blood samples.
Gastrointestinal disturbances occured in 12 (14,1%) children.isolated
gastrointestinal allergy was noticed in two patients.
First disturbances found in 4 patients just after formulaapplication. In 5
cases disturbances (diarrhoea, colic, vomiting)occured during breast
feeding.
Skin changes were the most common type of allergic reacxtions(54,0%).
Problems with respiratory sistem (nose, throath, lungs,ears) were the
second most common type of allergic reaction.
