    Summary: Trauma may cause stretch, compression, crushing or nervetransection. The nerve fibre itself can respond in only two ways:by segmental demyelination and remyelination of a minor injury orby axonal degeneration and subsequent regeneration in a severeinjury. Because the nerve injury consists of motor and sensorydeficit there is a need for an evaluation of motor and sensorfunctions. rehabilitative tretment before and during thereinervation period includes electrostimulation, analgesia,kinesitherapy and EMG-biofeedback. Besides motor reeducation,sensory reeducation should complete the rehabilitation program.

    Summary: The goal of the study was to evaluate the effect of tizanidine (acentral myotonolytic agent) and tenoxicam (a NSAID) on thedynamic function of the spine in the patients with low backpain (LBP) using electrokinesiologic analyses. Thirty-twopatients were included, divided in two groups. One group wastreated with tizanidine 3x2 mg, and the other with tenoxicam 1x20mg. The study confirmed the assumption that the increased sacrospinalmuscle activity was a reaction to pain, and not its cause due tocirculus viciosus pain-spasm-pain. Electrokinesiology analyses, which were used for the first timein the evaluation of these drugs proved to be an objective anduseful techique for the evaluation of therapy in LBP patients.

    Summary: The medical rehabilitation of 130 patients who had been wounded,mostly young soldiers and some older civilians, started in theperiod from October 1991 to February 1992 in the hospital used bythe IV Echelon. Most of the injuries affected the locomotorsystem, most frequently the lower extremities. The role of thephysiatrist and the physiotherapist consists in developing, andobserving dynamically, pre- and post-operative rehabilitationprogrammes, and in carrying out agreed procedures in the domainof respiratory, circulatory and locomotor physiotherapy. Thewartime plan of action of the IV Echelon rehabilitation team hasconfirmed the efficacy of the physiatrist-physiotherapistinterdisciplinary unit and of multidisciplinary cooperationcoordinated by the surgeon.

    Summary: The aim of this study was to test H or F wave latencies of medialhead of gastrocnemius muscle for electrodiagnostic evaluation ofpatients with suspected S1 radiculopathy with or withoutadditional L5 radiculopathy. A group of 97 patients withclinically suspected unilateral radiculopathy were studied. Theresults of this study have proved that measurements of H or Flatencies provide the objective evidence of S1 radiculopathy,presenting with the unilateral increase of latency or the absenceof response. Abnormal H response latencies without EMGabnormality confirm the condition of sensory root affection only. Mild radicular affection in S1 distribution presented from earlybeginning or following recovery, was accompanied by normal H or Fresponse, and therefore this test has prognostic value for thecourse of disorder.

    Summary: Assessment of walking ability of patients with spastic cerebralpalsy has long been a problem. In an attempt to obtain moreprecise and objective findings, we used the Polgon goniometrymethod. The study involved testing the gait of 36 spsticpatients. We compared clinical findings and Polgon diagrams ofhealthy children and spastic patients pre- and postoperatively.The results were clasified as very good to good, satisfactory,and poor. The advantages of Polgon are that it is painless, doesnot irritate the patient, and is a simple, objective method. Itsdisadvantages arise from a relatively large amount of artifactsand inadequacy when it is used on spastic patients.

    Summary: Computer reconstruction of EEG records will enable the curvesegments of special interest to be simultaneously analyzed inseveral different derivations. Although the methodology ofsimultaneous polyderivational analysis has some significanttheoretical advantages, its practical value has not yet beensufficiently explored. In this study, some aspects of thisapproach were explored on 35 EEG records showing focal interictalepileptiform changes. The results obtained showed the morphologicidentificaiton of complex epileptiform transients to be easier tomake in reference than in bipolar derivations, while the sourcederivation had an intermediary position. Of considerablepractical importance seems to be a conclusion according to whichit is very useful, in case of insufficiently discriminative focalchanges, to reconstruct the given curve segment in variousderivations. This procedure may considerably increase thediagnostic value of the findings.

    Summary: The EK scan contains quantified kinematic and myoelectrical data on the LS spine recorded during motion of the body in the sagittal plane. A total number of 110 patients with low back pain and/or ischialgia of degenerative etiology were examined and compared with 42 healthy controls. Clinical examination was used to classify the patients into 4 groups according to the severity of the existing symptoms. A deterioration in values of the kinematic indicators (prolonged duration and reduced amplitude and speed of the movement) was observed in patients with more severe degree of clinical classification. The kinematic data are in accordance with clinical classification of painful condition of the LS spine, while deviation is more prominent in patients classified as rather severe clinical cases.

    Summary: Carpal tunnel syndrome is a common complication in dialysed patients. The 139 ones were clinically and electroneurographically investigated. In 19 patients (13,6%) carpal tunnel syndrome was detected. The occurrence of the very syndrome was more frequent in longer dialysed patients. In seven surgically treated patients clinical and electroneurographic improvement occured.

    Summary: Electrokinesiologic analisys of sacrospinal muscles during lateroflexion movements was applied in 21 healthy subjects and 74 patients with chronic low back pain. All the examined kinematic indicators were in patients of significantly worse values. Speed of movement to the right was decreased with maintained amplitude (duration of movement was prolonged) and to the left was lowered because of decreased amplitude. EMG activity was higher in all the phases of movements in patients. The activity of left muscle was higher than the activity of the right one, except the movement from maximally lateral decline to the left in vertical posture. During lateroflexion movement to the right, EMG activity was significantly lower bilaterally, than at lateroflexion movement to the left, both in healthy subjects and in patients. The very method of kinesiologic analysis can be very practical for functional diagnostics of spine and for the follow-up of therapy effect.

    Summary: A total of 143 patients (127 hemodialysis, 16 CAPD) uderwent clinical examination. Clinical signs of carpal tunnel syndrome were found in 23 patients. The syndrome was not found in patients receiving dialysis for less than five years. Tinel's sign was positive in all patients with carpal tunnel syndrome. Pathologic values of nerve conduction velocity and distal lateny of the median nerve were found in 19 patients, all of them with clinical signs. The incidence of carpal tunnel syndrome correlates with the length of treatment by dialysis. It seems that the incidence is similar in hemodialysis and the CAPD group of patients.

    Summary: A total of 44 pts, dialysed for 11.5 (5-20) years underwent clinical and electroneurografic (ENG) examination. Clinical signs of carpal tunnel syndrome (CTS) were found in 25 pts, in 14 bilaterally. By ENG, CTS were documented in 20 pts. Hand dysfunction was founr in 22 pts. The parameters of hand function did not differ between dominant and non-dominant hand or the arms with or without A-V fistulae. Severity of hand dysfunction corelate with dialysis duration, presence of neuropathy and CTS.

    Summary: Muscle electrostimulation (MES) of shoulder-neck region was usedin five patients during rehabilitation after soft tissue neckinjury (whiplash). Electrokinesiologic analyses (EK), whichincludes quantitative surface EMG, range of movement (ROM), andspeed of movement was used to determine the functional status ofcervical spine. Measured changes can not be directly attributed to MES, but MESshould be considered as having probable positive effect onkinematic and EMG data in 4/5 patients, demonstrated by theincrease in ROM, speed, EMG of maximal isometric contraction, EMGindex, and decrease in EMG of peak flexion.

    Summary: Electrokinesiologic analysis is an objective and useful technicfor evaluation of therapy in low back pain (LBP) patients. Thestudy confirms assumption that increased sacrospinal muscleactivity is reaction on pain and has protective function in LBPsyndrome. NSAI improved kinematics data and reduced pain, but didnot influence EMG activity. The study is not agreement thatincreased muscle activity is a course of pain because of circulusviciosus pain-spasm-pain, while central myotonolytic (after 2weeks of therapy) neither decreases the EMG activity, norimproves dynamic function of spine (except increase of speed ofmotion).

    Summary: Twelve chronic LBP patients were included. Elektrokinesiologic measures were compared at three successive testings: at baseline, two hours after first intake of tizanidine (2mg) and after two weeks of treatment with tizanidine (3x2mg). EMG of paralumbar muscles decreased significantly after tizanidine intake, which was even more pronounced after two weeks. Duration and speed of movement were significantly improved after two weeks of tizanidine treatment. ROM did not change at follow-ups. It appears that tizanidine may be one choice for the beneficial therapy of chronic LBP.

    Summary: EK assessment of spine is a method of measuring kinematic dataand EMG activity of paravertebral muscles during movements ofspine in sagittal plane using EMG device and polarized lightgoniometer. Computerized protocol is programmed for immediateaccess to the measuring set-up allowing analysis of data. The investigation included 110 patients with low back pain (LBP)obtained in the group of 42 healthy subjects. Kinematic and EMGdata of LBP patients showed significantly different values incomparison to reference values. Monitoring and analysis of both mobility and EMG activity ofspine available by novel use of electrophysiologic measurementsis recommended as a valuable measurement tool for the clinicalassessment of dynamic function of spine.

    Summary: The aim of our study was to determine the relationship betweenpain intensity and EKD of lumbar spine during dynamic activity ofspine. The intensity of pain was quantified on visual analoguescale (VAS) by 42 healthy subjects and 110 patients with LBP.     Summary: In the group of 26 stroke patients standard protocol for theassessment of functional disorders at the beginning and at theend of hospital treatment was used. There was a correlationbetween the mobility status and the time before the start ofphysiatric treatment, but so far the correlation between thedisturbance of consciousness, speech disorder and changes ofmuscle tonus, has not been proved. We consider that it would benecessary to include stroke patients in the exercise program assoon as possible. The protocol for the assessment of motilitystatus improves evaluation of rehabilitation effects, so wesuggest its systematic use in all neurological patients duringrehabilitation.

    Summary: Twenty amputated patients with hip EPS were examined. Theirwalking pattern was tested clinically, by Polgon goniometry andon a treadmill. All patients with hip EPS and leg prosthesisexhibit assymetrical gait. Assymmetry is manifested by the speedof walking, step length, and increased and uneven width ofwalking. An EPS and a prosthesis should be designed to preventincreased sideways (varus) lean, flexion and rotation stressmotions, and bumping of heel against the ground.

    Summary: A total of 44-pts dialysed for 11.5 (5-20) years underwent clinical (Tinel, Phalen, Flick sign, 4 dinamic and 8 static parameters of hand function) and ENG examination. ENG signs of polyneuropathy were found in 41 (93%) pts, clinical signs of carpal tunnel syndrome (CTS) in 25 (57%) pts, in 14 of them bilaterally. Hypesthesia was found in 19, positive Tinel sign in 19, Flick sign in 14 and Phalen in 9 pts. By ENG, compression of the median nerve was documented in 20 pts. Impaired hand function was found in 22 pts, in 19 of them bilaterally. Dynamic grasping ability was most severely affected in 15 pts for the right hand and 13 for the left hand. The "needle grasping" was the most affected static parameter. It was impaired in 13 pts for the right hand and in 11 for the left hand. The parameters of hand function did not differ between dominant and non-dominant hand and the arms with the A-V fistulae or without it.

    Summary: Group of 86 whiplash injured subjects was assessed with the aim to find electrokinesiologic variables most suitable for functional evaluation of spinal dysfunction after whiplash injury. Hierarchical cluster analysis demonstrated 4 functional subgroups according to the speed of movement and EMG quotient among paravertebral muscle activity during extension and flexion phases of movement. Further discriminant analysis using original variables resulted with two significant functions for discrimination, first (p<0.0001) composed of speed and range of movement and second (p<0.0041) composed of EMG variables. This study demonstrated that electrokinesiologic variables during hyperextension of cervical spine should be considered as an objective tool for functional evaluation after whiplash injury.

    Summary: Group of 274 subjects was assessed with the aim to find electrokinesiologic variables most suitable for the evaluation of cervical spine function. Electorkinesiologic testing of spinal movement in sagittal plane allow simultaneous recording of kinesiologic electromyography (EMG) and kinematic variables. Hierarchical cluster analysis demonstrated 4 functional subgroups according to the speed of movement and EMG quotient among paravertebral muscle activity during extension and flexion phases of movement. Seze of correlation within functions ordered variables as follows: speed of hyperextension (0.58), speed of flexion (0.50), speed of extension (0.48), range of flexion (0.43) range of hyperextension (0.34), EMG during flexion (0.21). This study showed that wpeed of movement seems to be the mostreliable measure of cervical spine function.

    Summary: Slow healing inoperable wounds in the lower extremities present a significant problem of treatment. The grat majority of these wounds are caused by veins disorders. We have treated 16 patients with wounds persisting for at least 4 weeks. Treatments were done by low frequendy electromagnetic generator NESI 1203 producing 2 miliseconde rectangular voltage pulses at frequency of 50 Hz and peak magnetic field density of 8x0.001 Tesla in two locally applied Helmholtz's type induction coils. Treatments were given every other day and each lasted 60 minutes. Wounds were 0.2x0.5 - 2.5x3 cm large (x=2.1x1.1cm). After 5-38 treatments in 11 patients wounds healed completely and in 5 there was fair improvement. It can be concluded that magnetotherapy is a very promising treatment of slow healing inoperable wounds in the lower extremities.

    Summary: The elektrokinesiologic data of 763 patients with chronic low back pain were collected during five years period (1988.-1993.). Range of flexion movement was significantly higher in women than in men patient's group followed by higher values of normaliyed kinesiologic EMG activity of spinal muscles during flexion, peak flexion and extension movements. Lower EMG activity was recorded during maximal voluntary contraction in women than in men. The results of this study should be considered as an evidence of characteristic electrokinesiologic pattern of flexion-extension movement for women with low back pain.

    Summary: Low back pain patients (LBP) (N=763) were divided in five age groups. All tested electrokinesiologic data were wignificantly different by age. The youngest group demonstrated the best spinal function. Next three age groups showed progressive restriction of spinal mobility followed by increased mobility in group V. EMG data indicate deterioration of muscle function in LBP patients continuously by age. Resulrts of this study should suggest conclusion that body mobility in LBP patients show a changeable course with age: restriction of spinal function after age of 25 years, with the deterioration until the age of 55 but after this age some improvement of spinal mobility was demonstrated by kinematic performance.

    Summary: Functional diagnosis with electrophysiologic and electroneurographic examinations (ENG) has been performed in 44 patients on chronic hemodialysis. Subjective difficulties resulting from the carpal tunnel syndrome were observed in 57% of the patients. The most common complaint was hypestesia (43%), night pain (34%), while wasting of the thenar muscle was noticed in 36% of the patients. Tinel sign was positive in 43%. Dynamic grasping ability was most severely affected. ENG test enabled the diagnosis of peripheral polyneuropathy in 93% patients. The carpal tunnel median nerve was surgically decompressed bilaterally in 6 patients and they all experienced significant subjective improvement, and 5/6 also ENG improvement.

    Summary: Pulsed electromagnetic fields (PEMF) are being widely used in therapy of locomotor diseases. The goal of this work was to resolve possible mechanisms of PEMF acting. For this purpose rectal temperature of anesthetized rats were measured, during ten-hours exposition to PEMF (frequency 9 Hz, strength 3,5 mT) and compared with control group (n=10). rats exposed to PEMF (n=10) showed significantly lower rectal temperature. Maximal difference, reached after the third hour, was 2.2 degrees C (p<0.01), although the difference became significant already after the first hour (p<0.05) of application of PEMF. It can be concluded that blocking of sympathetic system and consequently appearing vasodilatation might be reason for significantly higer decrease of rectal temperature at rats exposed to PEMF

Significant difference (p<0.001) speed andamplitude data of total movement in saggital plane, movement offlexion and retroflexion between the control subjects andpatients should be considered as a useful indicator fordistinguishing functional damage and normal stage. Values ofspeed extension were significantly different in control group, inearly LBP patients and chronic LBP patients. Mean values forkinesiological EMG were significalntly different between bothearly and chronic LBP patients and between patients and helathysubjects. There was recorded connection between EMG activityduring flexion movement and at peak flexion and intensity ofpain. EK could be useful tool for objective evaluation of bacpain condition.

    Summary: The basis thesis in this paper is to prove that usingsimultaneous polyderivational analysis (SPA) some relevantinformation about focal activity can be achieved which otherwise,using conventional methods would not be possible. The thesis wastested upon the sample of characteristic segments of EEG findingsin 58 patients. To investigate the hypothetical possibilities ofSPA the integral analysis of focal EEG changes was divided into afew components. The investigation was made in separate butclosely connected parts. In this way localization procedure,morphological identification and characterization ofbioelectrical field of focal potentials were investigated. Theresults of the investigation showed that SPA can significantlyimprove conventional analysis of focal changes inelectroencephalogram in most important components of topographicanalysis: in focus locaslization, in analysis of potential fieldfeatures and identification of morphology of focal transients.

    Summary: The investigation was undertaken in 132 patients with chronic low back pain (63 subjects with work capacity (WC), 69 with work disability (WD)) and 54 healthy subjects. Healthy examinees statistically significantly differ according to kinematic parameters at fast and normal speed movements. The natural relation of magnitudes of single movements has been preserved in healthy and sick subjects. Amplitude of flexion and retroflexion is higher at fast toward normal speed movements, but statistically insufficient. In WD toward the WC, there is statistically significant less speed of extension at normal speed movements, while at fast ones, speed of trunk flexion is less significant too. Sensitivity of kinematic values of POLGON test of patients with chronic low backache is satisfactory and figures out 70-93%. Sensitivity of speed flexion, extension, retroflexion and anteflexion is greater at speed movements toward normal speed ones, more expressed in work WD. Sensitivity of flexion amplitude and retroflexion amplitude is greater at speedy movements toward normal speed ones in patients with WD, and retroflexion only in those ones with work capacity. Positive predictive value of kinematic parameters of POLGON test is 68-87%. The highest positive values (the least number of falsely pathologic findings) are found at speed flexion of fast movements both in WC and the WD, and therefore the use of analysis of this variable in work capacity assessment is recommended. Negative predictive value of kinematic values is 60-90%. The greatest values (the least number of falsely normal findings) at speed flexion and extension of fast movements are in WD, and the use of analysis of this variable is recommended in routine diagnostics of chronic low back pain diagnostics. Predictive belonging to the WC and WD is greater, besides the analysis of kinematic values and normal speed movements and fast ones, toward the analysis of only fast or only normal speed movements. By discrimination analysis it has been proved that the greatest discrimination power of WC toward WD has the assessment of pain, and for kinematic parameters of retroflexion amplitude of fast and normal speed movements, and among ergonomic factors of occupational activities: standing, bending and heavy lifting.

    Summary: The study was randomized, double-blind. Thirty-seven chronic LBPpatients, age range 25-57 years were included, divided into threegroups: two therapeutic groups of 16 patients each (tizanidine3x2mg, or tenoksikam 1x20mg during 2 weeks); 5 patients wererecorded to test the reproducibility of method. Tested measureswere compared at 3 successive testings: at baseline, two hoursand two weeks after treatment. After 2 wks on tizanidine thespeed of motion and intensity of pain on VAS showed astatistically significant improvement. Decreased EMG activity wasrecorded only 2 hrs after tizanidine. After 2 wks on tenoxicamalmost all kinematic parameters, except amplitude ofretroflexion, showed a significant improvement. Intensity of painwas also decreased, but EMG activity was not significantlychanged. Two hrs after tenoxicam no parameter was changed. Aftertreatment with tizanidine and tenoxicam no changes in reflexwaves latency of the calves were registered. Both therapeuticdrugs were well tolerated. Sleepiness occurred in 3 of 16 femalepatients in the tizanidine group.
