Summary: The mechanisms of allergic bronchial asthma and
rhinitis, as well as specific immunotherapy are still unclear. For
diagnostic procedure, specific immunotherapy, and study of the mechanisms
of allergic diseases, it is necessary to use the standardized allergen
extract. In collaboration with Institute of immunology, Zagreb, the
Croatian national standard for Dermatophagoides pteronissinus and Ambrosia
elatior, were produced. Standardised Dermatophagoides
pteronissinus (Der p) allergen extrac was used for the rush immunotherapy
which was performed in 40 patients with allergic rhinitis and/or bronchial
asthma. The effect of sepcific immunotherapy was determined by
evaluating the changes of humoral (total IgE, specific IgE, Der p specific
IgG4) and cellular immunity (CD2, CD4, CD8, CD4+CD8+, CD20, CD25).
During specific rush immunotherapy the change of skin tests, nonspecific
and specific bronchoprovocation were also evaluated. Before and
after bronchial chalenge tests by allergen Der p, the change of level of
peripheral eosinophils and lymphpcyte subpopulations were measured.
In supernatante of short culture of peripheral blood cells of patients
with allergic bronchial asthma some mediators of allergic inflammation
(histamine, triptase, PAF, TNF,ECP) after stimulation by Der p were
determined. Using the model of guinea pig tracheal ring we
investigated the role of neurotransmiters (tachykinins, supstance P),
prostaglandins and leukotriens in allergic bronchoconstriction.
Research goals: Allergic rhinitis and bronchial asthma are
common therapeutical problem. The specific immunotherapy is longlasting
procedure, of unpredicitible outcome. The mechanisms of allergic
bronchoconstriction and specific immunotherapy are still unclear.
Therefore, the goal of this study was to: 1. production of
standardized Der p and Ambrosia elatior allergen extracts, 2.
evaluate specific rush immunotherapy as possible method for the treatment
of allergic diseases of the respiratory system, 3. evaluate the
change in humoral and cellular immunity in patients with allergic disease
during specific rush immunotherapy, 4. evaluate the effect of
immunotherapy on change of specific skin reactivity, and nonspecific and
specific bronchial reactivity, 5. after specific
bronhoprovocation investigate the role of peripheral and tissue
eosinophils and lymphocyte subpopulations, 6. in culture of
peripheral blood cells from allergic subjects after allergen adition,
measurment of mediators (histamine, tryptase, TNF, PAF, ECP), 7.
investigate the role of neurotransmiters (Tachykinins, supstance P) in
allergic bronhoconstriction. We expect a significant decrease in symptoms
of allergic rhinitisand bronnchial asthma in patients treated with
rushimmunotherapy. Furthermore, we expect the improvement of theobjective
parameters af allergic disease.
