Summary: Nonmalignant lymphadenopathy occurs in 80% of all
lymphadenopathies in age up to 40 years, and after this age about 40% of
clinically manifested lymphadenopaties are nonmalignant. In the whole
world 18-30% of nonmalignant lymphadenopathies are of unknown etiology,
being a major problem in treatment, in prognosis of the outcome of the
disease and in evaluation of working ability of such a patient. This work
should help to find the solution to these problems as follows: a) by
clinical and etiological researches b) by morphologic, cytochemical and
immunocytochemical researches c) by objective evaluation of obtained
results for making a long - term prognosis. A total number of 104
patients with lymphodenopathy has been treated so far in the work, partly
in the out-patient department and partly in the hospital. After
diagnostic, clinical and cytological procedure the malignant disease was
excluded in all of them. During the period of 1994-95 other 34 patients
were treated, according the proposition of this project. Their diagnoses
were: Syndroma mononucleosis inf.caused by EBV 9; Syndroma mononucleosis
inf.caused by CMV 3; TBC of lymph nodes 3; Cat scrach dis. 5;
Toxoplasmosis 6; AIDS 3; Unknownn 3; Total 34 The etiology was known in
74 cases (infectious mononucleosis, toxoplasmosis, cat scratch disease,
EBV lymphadenopathy, tuberculosis, lymphogranuloma venereum, AIDS), and
in 27 cases the etiology was unknown. The most frequent types of
cytologic reaction in examined lymph nodes were: reactive hyperplasia
(various types) and granulomatous reaction. The cytologic status of
control fine needle aspirates was sometimes different in relation to the
previous one, that was reported as a dynamics in the course of evolution
of inflammation. The subpopulations of T-lymphocytes (CD 4, CD 8, NK
cells) and the cells with monocytic marks were examined by
immunocytochemical methods. At the beggining of 1995 the PCR methods were
performed in some special cases of unclear nonmalignant lymphadenopathy.
For the time being we can prove the genetic material for AIDS, TBC and
Chlamidiae.
Research goals: The aim of this examination is to range by a modern
processing(as under a,b,c,d and e) the patients with
nonmalignantlymphadenopathies into some clinical and cytologic entities
andthus to estimate the course, treatment and prognosis of thedisease, as
well as to give final evaluation of working ability.It is important
specially in those cases when routine diagnosticprocedures are not
available in making etiologic diagnosis (23), as per our research.
