Summary: The aim of the study was to investigate magnesium (Mg)
metabolism in diabetics with insulin-dependent diabetes mellitus (IDDM). Mg
was measured from serum, red blood cells (RBC) and urine of 107 subjects
with IDDM and 63 healthy subjects of the control group. Diabetic group was
divided into three groups: a) well controlled IDDM (n=39), b) poorly
controlled IDDM (n=68), c) poorly controlled IDDM patients with ketonuria
(n=27). Serum Mg (0.79+0.09 mmol/l) and RBCMg (2.11+0.28 mmol/l) were lower
in IDDM than in controls (SMg 0.85+0.07; RBCMg 2.16+0.23;p<0.001 for both).
Magnesuria was higher in diabetics (5.3+2.4 vs 4.0+2.4 mmol/24h, p<0.001).
Extremely low serum Mg were found in poorly controlled IDDM (0.76+0.07)
especialy in those with ketonuia (0.73+0.08,p<0.001). The latter subgroup
had higher magnesuria than controls (6.7+2.0 vs 4.0+1.4,p<0.001) and whole
diabetic group (5.3+2.4 p<0.001). RBCMg and magnesuria were significantly
higher in well controlled IDDM than in the control group (RBCMg 2.3+0.30 vs
2.16+0.23, p<0.001; UMg 42+1.4 vs 4.0+1.4, p<0.01).In conclusion, our
results imply the depletion of total body Mg in IDDM: significantly lower
serum Mg level, RBCMg content and higher urine magnesium which was more
pronounced as metabolic control was worsened. Higher RBCMg content in well
controlled IDDM than in controls may be due to insulin action on Mg
transport from extra- to intracellular space.
