    Summary: Growth hormone (GH) levels following i.v. bolus injection of200Šg of TRH were measured in 18 female patients with NIDDM whowere divided into two subgroups according to relative bodyweight- 9 obese and 9 nonobese patients. The control groups werecomprised of 11 obese and 5 nonobese examinees. The GH levels innonobese NIDDM patients did not differ significantly from thosein nonobese control group. However, obese patients with NIDDM hadsignificantly higher GH values than the obese controls (p<0.01),but not significantly higher than the control group with normalbody weight. In the obese controls, TRH did not provoke GHsecretion, whereas there was one responder among the obese NIDDMpatients. Our results suggest impaired regulation of GH in NIDDMassociated with obesity.

    Summary: Serum osteocalcin levels has been found to be a specificbiochemical parameter of bone formation. We measured serum levelsof osteocalcin, PTH, total calcium and alkaline phosphatase in 26women with NIDDM (19 postmenopusal, 7 with regular cycle) and amatched control group (n=30). Osteocalcin serum levels weresignificantly decreased in NIDDM postmenopausal women, whencompared to control subjects. Serum levels of PTH, total calciumand alkaline phosphatase in diabetic patients were notsignificantly different from the matched control group. Our datagives evidence of the osteocalcin deficiency and decreased boneformation in postmenopausal women with NIDDM.

    Summary: Results of OGTT were compared in 30 proximal type obese personsand 12 healthy controls of normal body mass to establish insulin,C-peptide and glucose levels. Plasma triglyceride, totalcholesterol, LDL and HDL-cholesterol levels were also measuredand data from normal and obese persons compared. Persons withupper body obesity and normal glucose tolerance had increasedinsulin, C-peptide and plasma triglyceride levels and low levelsof HDL and LDL cholesterol compared to subjects of normal bodymass and normal glucose tolerance. Our results suggest thatproximal type of obesity is connected with abnormalities ininsulin and lipoprotein metabolism. Further investigation isnecessary to examine the mechanism of these abnormalities.

    Summary: Insulin, C-peptide and glucagon values were determined by OGTT at0,30, 60 and 120' in women with the proximal type of obesity. Thestudy included 22 subjects with normal glucose tolerance (NT) andrelative body weight (RW) 155.4%, 17 subjects with impairedglucose tolerance (IT) and RW 168.6% and 7 controls with normalbody mass. Compared to the controls, basal insulin and C-peptidelevels were significantly higher in both obese groups. Sixtyminutes following glucose administration, insulin levelsincreased in both obese groups, whereas C-peptide levelsincreased only in the obese IT group. Glucagon levels weresignificantly elevated in the obese IT group both basally andafter 30'. A significant fall in glucagon concentrations occuredbetween 30 and 60' in the obese IT group (p<0.02) and a slightlysmaller fall in the obese NT group (p<0.05). Our results indicateslower glucagon supression of hyperglycemia and hyperinsulinemiaand resistance of pancreatic alpha-cells in obesity.

    Summary: Plasma concentrations of cholesterol, LDL-cholesterol,triglycerides, HDL-cholesterol, insulin and C-peptides (basal andin response to stimulation with usual meal) were measured in 27obese patients with upper body obesity and NIDDM. The samemeasurements were done in 16 nonobese NIDDM patients matched forsex, age, duration of diabetes, therapy and HBA1 with the obesediabetics. Patients with NIDDM and upper body obesity hadelevated basal and stimulated C-peptide, cholesterol andtriglyceride levels with low levels of HDL-2-cholesterol inplasma compared to the nonobese patients. These data provideevidence that the obese NIDDM patients with upper body obesityshow abnormalities in C-peptide and lipoprotein metabolismcompared to nonobese NIDDM patients matched for sex, age,duration of diabetes, therapy, complications and HBA1 with theobese diabetics.

    Title: BODY FAT DISTRIBUTION, SEX HORMONES, SEX HORMONE-BINDING GLOBULIN AND INSULIN IN OBESE PREMENOPAUSAL WOMEN

    Summary: Androgens, sex hormone-binding globulin (SHBG) and responses of plasma insulin and C-peptide in OGTT were examined in 46 healthy obese premenopausal women, 16 with lower body obesity (WHR<0.80), 15 with pure upper body obesity (group P1;WHR>0.85) and 12 with predominant upper body obesity (group P2;WHR 0.81-0.85). Both fasting and glucose-induced insulin and C-peptide levels were higher in women with pure upper body obesity (group P1) than in those with lower body obesity (p<0.01 for each). Free testosterone (p<0.001) and androstenedione (p<0.01) were higher and SHBG (p<0.01) was lower in abdominally obese women (group P1) than in those with lower body obesity. After adjustment for BMI, basal insulin still significantly correlated eith free testosterone, while glucose-induced insulin correlated with androstenedione. After adjustment for BMI,WHR still positively correlated with free testosterone and androstenedione. After adjustment for WHR and BMI, SHBG still negatively correlated with glucose-induced insulin levels. In conclusion, study results indicated that in healthy premenopausal women, upper body fat predominance was associated with increasing androgenicity (higher free testosterone and androstenedione, lower SHBG) and increasing basal and glucose-induced insulin and C-peptide levels, the latter indicating that hyperinsulinemia occurred in part due to the increase in insulin secretion. SHBG levels correlated with insulin, regardless of the degree of obesity. Therefore, we concluded that upper body fat predominance in healthy premenopausal women was associated with hyperinsulinemia which might produce hyperandrogenism.

    Summary: Serum, red blood cell (RBC)and urine levels of magnesium (MG) were determined in 62 patients with non-insulin dependent diabetes mellitus (NIDDM,31 with well controlled and 31 with poorly controlled NIDDM), and in a control group of 63 normal subjects. Serum and RBC levels of Mg were significantly lower, and urine Mg levels significantly higher in NIDDM patients than in control subjects. The group of well controlled NIDDM patients had serum and RBC levels of Mg lower by 10.6% and 2.8% respectively, and urine Mg higher by 7.5% as compared to the control group (p<0.001 all). In the poorly controlled NIDDM group serum and RBC levels of Mg were 14.1% and 14.0% lower, respectively, and urine mg levels by 28.6% higher than in the control group (p<0.001 all). No correlation was found either between Mg levels and age of the groups, subgroups or samples examined. Results obtained in this study indicated magnesium depletion in NIDDM patients, which was more pronounced in poorly controlled NIDDM. Hypomagnesemia and hypermagnesuria in NIDDM patients with stabile glucose homeostasis were beyond the scope of this study, but obviously require futher investigation.

    Summary: Echocardiography M-mod 2 D simultanously with phonocardiogram andECG have been used. Sistolic parameters VCF, PET/ET and diastolicparameters IRT (isometric relaxation time) and RRF (rapid ratefilling of left ventricle) were compared. In diabetic patientsboth sexes IRT was significantly longer compared to the controlgroup. IRT is "active process" in function of myocardial highenergy phosphates. There is increasing pool of evidence forspecific heart disease of diabetic patients.
