Summary: In the examination anamnestic, clinical, laboratory and
hormonal data will be evaluated in the obese subjects with normal and
impaired glucose tolerance and in diabetics: nutritional habits, clinical
examination, ECG in the rest and during exercise, expiratory/inspiratory
index), sonographic measurement of kidney area, sonographic measurement of
sistolic and diastolic parameters of left ventricle, measurement of
lipoprotein fractions, creatinin, aminotranspherases, gama GT, HBA1c,
insulin , C-peptide, glucagon and growth hormnone, androgens, SHBG, as well
as microalbuminuria and dinamic renal scintigraphy. All examinations will
be performed in matched control group. Some examination will be repeated
after reducing diet and body weight loss to determine reversibility of
metabolic and endocrinologic abnormalities and significance of reducing
diet. After estimation of magnesium serum and eritrocyte concetrations we
will evaluate influence of magnesim on peripheral insulin sensitivity in
adipose subjects. Results of the examination will be reported in medical
journals, on congresses etc. Untill now five reports have been published
and one in the form of summary on the congress. Two doctoral dissertations
and two master's thesis had been aproved but not yet finished. Another two
examinations are still going on but are not finished.

Research goals: Goal of the project is to performe examination of
obese persons with respect on regional distribution of adipose tissue and
evaluate anamnestic, clinical, laboratory and hormonal data in subjects
with normal and impaired glucose tolerance and with diabetes
mellitus.Contribution of egsogenic factors such as life style and
nutritional habits on the beginning and maintance of obesity will be
evaluated. We will try to define obesity as diabetogenic factor and
determine significance of adiposity and insulin response in glucose
tolerance test. We want to determine significance of reducing diet and body
weight loss on reversion of metabolic and endocrinological impairments. We
will search for abnormalities in cardiovascular function which could be
ethiopathogenic associated with adiposity. Androgens and SHBG will be
studied to determine correlation between hyperinsulinemia in OGTT and
hyperandrogenismus in obese women. Hemodinamic paramethers of renal
function will be investigated to recognize early changes in renal functions
in adiposity and impaired glucose tolerance. Magnesium status will be
estimated and determine its influence on peripheral insulin sensitivity in
obese subjescts and diabetes mellitus. Insulin resistance in muscles and
adipose tissue will be determinated in the obese subjects. According to the
results of our investigation we will propose the best treatment of obesity
including the type of reducing diet and physical activity. Other information about the project.
