Summary: The value of serum neopterin in the assessment of activity
of
ulcerative colitis(UC) and Crohn's disease (MC) was evaluated
in the study that included 91 patients, 61 having UC and 30
having MC.Based on Powell-Tuck index,19 patients with UC had
active disease and those patients had higher serum neopterin
concentrations (p<0,01).Serum neopterin concentrations and
values of Powell-Tuck index were in positive linear correlation
(r=0,5065 p<0,001).The activity of MC was assessed with CDAI
and van Hees index.Based on CDAI, only 4 patients had active
disease.Using van Hees index, 17 patients were in partial
remission and 13 had active disease. Serum neopterin did not
correlate with activity regardless of the index used.
It can be concluded that serum neopterin correlates with the
activity status of UC but not MC.

Summary: A ten-year incidence of ulcerative colitis in Zagreb
ispresented.The study included both inpatients and outpatients
regardless of the extent and severity of the disease.The mean
annual incidence rate was 1,5/100.000.There was no increase
in incidence of ulcerative colitis during the study period.
A prevalence rate estimate of 21,4/100.000 was based on
July 1985 official estimated population.

Summary: A-ten year prospective study of Crohn's disease was carried
in

Summary: The activity of UC and MC was assessed by 5 acute
phasereactants:CRP,alpha-1-acid glycoprotein,alpha-1-antitrypsin,
haptoglobin and fibrinogen.We randomly chose 91 patients,
61 with UC and 30 with MC.Statistical analysis was performed
by non-parametric methods:the Kruskal-Wallis and Fisher's
exact test.The disease activity was assessed in UC with Powell-
Tuck index and in MC with CDAI and van Hees index.
Patients with active UC had significantly higher levels of all
acute phase proteins except fibrinogen (p<0,001).Analysis by
Fisher test indicated probability for all measured proteins
to be higher than normal, particularly CRP. In MC paatients,
according to CDAI we found alpha-1-glycoprotein and haptoglobin
to be higher in active disease(p<0,05). Fisher's exact test
indicated that sample was probably not representative.According
to van Hees index,patients with active disease had higher
concentrations of haptoglobin,CRP,alpha-1-antitrypsin and
fibrinogen.Our results confirm that there is no single para-
meter for IBD activity assessment, but these 5 acute phase
proteins with other variables could be useful in the more
precise IBD activity assessment.

Summary: The serum selenium concentration was measured in 202
healthy
volunteers, 45 patients with ulcerative colitis and 25 patients
with Crohn's disease.Mean serum concentrations were for the
control group 66,5 +/- 14,4 micrograms/liter, for UC group
66,5 +/- 14,4 and for MC group 62,0 +/- 14,5.Regarding UC group,
there was no difference from control group, within the group
itself related to sex, extension of disease and duration ofdisease or
activity judged by Powell-Tuck index.Selenium
concentration was lower in MC group than in both other groups.
There was no correlation between selenium concentration and CDAI,
but using van Hees index there was negative correlation.
UC is not expected to be accompanied by hyposelenemia. Crohn's
disease is associated with hyposelenemia, where hyposelenemia
correlates with the disease activity.

Summary: There are no completely reliable methods for predicting
relapsein Crohn's disease. We studied prospectively 45 patients withcrohn's
disease in remission for 8 months period. The activity ofdisease was
monitored by CDAI calculation and by followinglaboratory parameters: ESR
(first hour),fibrinogen,alpha-l-antitrypsin and alpha-l-glycoprotein.
Statistical differences in CDAI values and laboratorymeasurements between
patients who later relapsed and those whoremained in remission were
evaluated by Student t-test.

Conclusion: alfa-l-glycoprotein was the only acute phase proteinwhich
showed predictive value regarding impending relapse. Allstudied parameters
increased significantly at the time ofrelapse. Identification of patients
in clinical remission withhigh risk of relapse might indicate early

Results: decreased diffusing capacity (65,5% +/- 14,9% ofcontrols) was
found in 11 patients (26%), 6 with UC (all active
with Powell-Tuck 6,4+/-2,7) and 5 with MC (all active with CDAI
238+/-31). All of those patients were taking sulphasalazine
(7,2+/-7 years), none was anemic or having preexistent heart
or lung disease.Nine patients in this group reported exertional
dyspnea. 12 patients had increased residual volume measured by
body pletismography, most of them having decreased expiration
reserve volume and maximal minute ventilation, which mightindicate sceletal
muscle weakness. 10 of those patients were in
remission and only two with UC active.There were no restrictive
abnormalities noted. Among studied population there were 52%
smokers and 14% ex-smokers so that pulmonary function tests
were better than expected with exception of significant number
with decreased pulmonary diffusion.Possible explanations are
interstitial lung disease as extraintestinal complication of IBD
or sulphasalazine-associated alveolitis.Further studies as BAL
and transbronchial biopsies are needed.

explanation might be excesive colon transversum inflation. There were no
restrictive abnormalities noted.
Conclusion:
Among studied population there were 52% smokers and 14% ex-smokers so that
pulmonarz function tests were better than expencted with exception of
significant number with decreased pulmonary diffusion. Possible explanation
for that observation are interstitial lung disease as extraintestinal
complication of IBD or sulfasalazine-associated alveolitis. Further studies
as bronchoalveolar lavage and transbronchial biopsies are needed.

Summary: In order to identify reliable method for predicting
relapse in Crohn's disease, we studied prospectively 53 patients in
remission at the beginning of the study for 2 year period. The activity of
disease was monitored by CDAI and by following laboratory parameters: ESR
(first hour), fibrinogen (normal 1,8-3,5 g/L), alpha-1-antitrypsin (norm.
1,4-3,2 g/L) and orosomucoid (norm. 0,4-1,3 g/L). As statistical method we
used Spearman's rank correlation test.
Our results show that all parameters have statistical significant as
follows:ESR p<0,01, fibrinogen p<0,01, alpha-1-antitrypsin p<0,0l,
orosomucoid p<0,0l.
Conclusion: esr, alpha-l-antitrypsin and orosomucoid showed predictive
value regarding impending relapse. All studied parameters increased
significantly shortly before time of relapse. Identification of patients in
clinical remission with high risk of relapse using alpha-a-antitrypsin and
orosomucoid might indicate early treatment.

Summary: The selenium in the serum of the healthy population in our
country displays values among the lowest in the world. We used
method of atomic absorption spectrophotometry and sampleconsisted of 202
persons of both sexes (age range from 20
to 80 years) while values were 66,79 +/-9,13 ug/l.
The ulcerose colitis as a disease with increased risk fordevelopment of
colorectal cancer is not associated withhyposelenemia (n=45, Se (s)
66,5+/-14,43 ug/l), while in
Crohn's disease we found a significant decrease /n=25,
Se(s) 62,014,48 p<0,05).
Colorectal adenomas were associated with significanthyposelenemia which was
more pronounced in multiple polyps.
Colorectal carcinoma is associated with significant hyposelenemiawhich is
more pronounced in mucinous type than in adenocarcinoma
and clearly dependant on Dukes stages.
Patients with cirrhosis of the liver had significanthyposelenemia similar
to Dukes D colorectal carcinoma indicating
that proper judgement of serum selenium concentration as the
biochemical marker in colorectal tumors (adenomas and carcinomas)
implies the previous evaluation of hepatal status.
