Title: Influence of early surgery in traumatic CSF fistulas and on the occurence of menigitis.

Summary: Differences in the occurence of meningitis during conservative and early surgical treatment of posttraumatic CSF fistula were analysed. Till 1985, conservative treatment was usuallz applied at the Dpt of Neurosurgerz, Zagreb Universitz Hospital Center, and the incidence of meningitis was 23%. During the 1989-1990 period, 1910 patients suffering from head and brain injury, 67 (3,5%) of them with CSF fistula, were examined. 52 patients were operated on, meningitis occured in 1,9% (one patient) and one died after surgery. Four patients died before surgery due to severe injuries. 11 patients had spontaneous closing of CSF fistula, in 5 patients CSF fistula relapsed and 3 were subsequentlz operated on. Among these 11 patients, previouslz conservativelz treated, meningitis was observed in 3(27%) patients, significantly higher than in patients operated on. In our group of 67 patients with CSF fistula, four (5,9%) patients developed meningitis, which was significantlz lower as compared to patients conservatively treated before (23%). The authors conclude that earlz surgerz for dural defect closing is the only long-term prophzlaxis for CSF fistula and meningitis relapse.

Title: Agressive intensive management after severe war head injury

Summary: The aim of this study was to find the outcome according to the GOS depending on the GCS using intensive management in patients after severe head injury from war. During the war in Croatia, 262 head-injuried patients were admitted to Universitz Department of Neurosurgery Zagreb. 73 had scores less than 8 GCS on admission or 48 hours after admission. After radiologic examination and operation, it was necessary, aggressive intensive management was performed. All the patients were mechanically ventilated. When it was possible according to injurz, ICP was monitored using Leeds boult (from 3 to 21 dazs). Arterial pressure was monitored invasivelz. According to ICP and CPP the authors used manitol and/or thiopenton, trzing to keep ICP below 20 mm Hg, and CPP near 65 mm Hg. All patients received phenitoin. Prophzlactic antibiotic therapz was used routinely for 7 days.In patients with GCS 3 or 4, mortality rate was 82,5%, while in those with scores 5 to 6, and 7 to 8, mortality was 35,3%, and 20%, respectivelly. Comparing these results with those gathered during the Lebanese conflict in Israel, the authors can conclude that they are similar.

Summary: During war in Croatia 262 injuried persons who sustained mostlz missile head injury were treated at Department of neurosurgery Zagreb. 29 of them developed CSF fistula. Five of them developed meningitis, an incidence of 17,24%. These numbers are higher then the incidence of CSF fistulas at peace time /3,5%).and the incidence of meningitis (5,9%).The probable explanation of the number of liquorrheas at the wound side is the inadequate operations in the war hospitals. In most cases the cause of liquorrhoeas was direct fracture of scull base while the small number of them were caused bz distant fracture from the entrance wound because of missile kinetic energy effect on the scull. Our approach to the problem is immediate operation depending on the patient condition, with previous antibiotic profilaxis. Diagnostic methods are: the scull base x-raz thomograms, computed thomograms and CT cisternographz, which is the most adequate. Operation tehnics is plastic the defect of the scull base with fascia, muscle or liodura using two-component fibrin glue (Beriplast). We also use this glue for our original method in closure the dura defect after transsphenoidal approach to CSF fistulas in sellar region.

Summary: In this studz a new concept of mechanical ventilation for neurosurgical patients is described. It is based on arteriojugular oxygen ssaturation monitoring, which gives us information in global cerebral oxygenation. Based od the narrow, normal or wide cerebral extraction of oxzgen values the so called optimized hzperventilation can be achieved.

Summary: From april to september 1991, 415 wounded subjects were treated at the Rebro Clinical Hospital in Zagreb, 362 at the Dpt of Surgery, and 53 at the Dpt of Neurosurgery. Infections developed in 15,7% of the wounded (wound infection in 14,6% and sepsis or meningitis in 1,1%). All cases of sepsis and meningitis, as well as 88,2% of wound infections, were hospital-acquired infections, whereas 7,9% occured within 48 hours from injury. Aerobic bacteria (Enterobacteriacae, Pseudomonas aeruginosa, Staphylococcus aureus, Acinetobacter spp) were found in 90%, whereas 9% of cases referred to mixed aerobic-anaerobic flora. In one injured subject, a clinical picture of gas gangrene developed. No streptococcal infections or tetanus were recorded.
