Summary: In patients with autoimmune systemic diseases, systemic
lupus erythematosus (SLE) and rheumatoid artrhitis (RA), psychological,
sociological and epidemic as well as the other factors are studied. These
factors together with traditional way of treatment of patients with
autoimmune disease give quite valuable data that may be applied in everyday
clinical practice with regard to the course and outcome of the disease,
quality of life and clinical and laboratory values that are needed to
establish the diagnosis (according to the accepted criteria of the American
Rheumatology Association) and assess the activity of the disease, the
precise status of cell immunity using two-color flow cytometric analysis
for peripheral blood mononuclear cells bearing T, B and NK-cell marker and
NK-activity were also studied. The value and reability oflymphatic
subpopulation determination of the disease activity and its use in every
day clinical practice have been also investigated. Futhermore, in patients
in whom organic imparirment of the central nervous system due to the basic
disease has been excluded, repeated psychological testing is undertaken.
The effectiveness of these phychological as well as psychiatric treatment
is evaluated as well. Patients included into this research parallely with
psychological and psychiatric treatment are managed by a standard therapy
fogether with usual clinical and laboratory control of the disease. Results
are analysed by tests of correlation and multivariant analysis.
Research goals: 1. To analyse the value of the study of
psychological, sociological and epidemiologic factors together with
clinical, laboratory and immunological parameters in patients with SLE and
RA with regard to the occurence, course and outcome of disease. 2. To
analyse the differences in clinical and laboratory parameters with respect
to the activity of disease, especially, to precisly analyse the changes in
the cell and humoral immunity by the introduction of new dignostic methods.
3. To analyse the changes in the results of psychological tests and the
value of evaluation of psychological status of patients with SLE and RA
outcome of disease. 5. To use a systematic approach in the diagnosis and
treatment of autoimmune diseases and to parallely analyse and treat
psychosomatic and immunological disorders, respectively. 6. To determine
the value of information that co9uld be used in "nonpharmacological
interventions during treatment of autoimmune diseases. 7. To promote the
quality of life of patients, secondary prevention of disease complications
and morbidity. 8. To advance the working ability of patients and decrease
the economic consequens due to long-term disease.
