    Summary: The autors evaluate the most frequent causes of seizures in 562patients admitted at the emergency out-patient ward because offits. 194 patients were admitted because of the firstoccurrence of seizures, in 12 of them the first manifestation wasstatus epilepticus of the generalized(8), or focal type(4). Thecommonest causes of seisures were alcoholism(82), disturbances ofthe brain blood flow(74), posttraumatic states(20), braintumor(4), or encephalopaties. In 11 patients the cause ofseisures was not found. Beside the causes, the authors stressedthe most frequent type of seizures, as well as therapeuticmeasures. They pointed out that in alcoholism generalizedseizures, and grand mal status were the commonest expression ofseizures. The same was found in patients with posttraumaticseizures, but they mostly suffered single attacks, and the samein encephalopaties. In the patients with vascular leisons focalseizures were not rare. The antiepileptic therapy is not appliedin the first occurrence of convulsions, therapy is causal, linkedprimarily to basic etiopathogenetic mechanism responsible fortriggering seizures

    Summary: Reactive hyperglicaemia is a phenomena well known to accur duringsome stress situations, such as surgical procedures, infestionsand severe posttraumatic and postburn states, as well as incerebrovascular accident The authors investigated glucose, glycosylated hemoglobin, and frucosamin values in bloodand cerebrospinal fluid following ischemic and hemorrhagicstroke in 148 patients Patients were divided in four groupsdepending upon prior anamnesis of diabetes, and depending upon blood glucose degrees after stroke. Good correlation was foundbetween values of glucose and fructosamine in blood, and poorbetween total protein amount and fructosamine. In cerebrospinalfluid significant correlation was found between fructosamine andtotal protein amount. The author discussed possibility offructosamine penetrate the blood brain barrier but the meshanismoh their transport has not yet been elucidated. Results of studypointed out certain role of stress and parenchymal damage inreactive hyperglicaemia with short duration.

    Summary: In order to determine genetic differences between 50 examineeswith ischemic cerebrovascular accident (ICVA) and those ofcomparative group, we have chosen 1883 persons from phenotypichealthy population divided into 5 different subgroup. The purposeof our investigation was exploring hereditary characteristic ofantigens linked to erythrocyte membrane. The highestdiscriminating value in genetic distance had MN and ABO geneticloci.The frequency of the blood group A in the patients with ICVwas 32,68 % and 27,16% in the comparative group. Blood group Bhad frequency in the patients 10,69% and in comparative group6,72%. O blood group had frequency in patients 56,73 and in thecomparative group 66,12%. Genetic distance between patients withICVA and comparative group were determined with gene frequencies,that was shown on the dendrogram. The dendrogram clearly showsthat patients with ICVA are separated from all other comparativesubgroups.The results of our investigation presented the highestdiscriminating value of MN and ABO genetic loci. The possibility of likage between genetic loci for theseerythrocyte antigenes (on 4th and 9th chromosome) and geneticloci which determine ICVA cannot be excluded. Finally we considerthat this method can support earlier identification of personswho belong to" high risk group for cerebrovascular disease".

    Summary: The presence of various heme derivates can be demonstratedspectrophotometrically in colorless cerebrospinal fluid (CSF).Because of high sensitivity of the method it may detect compoundsthat reflect s "traumatic tap" rather than a disease process.However, the presence of methemoglobin excludes the possibilityof a hemorrhagic CSF being caused by traumatic lumbar puncture.Here we describe a highly sensitive spectrophotometric methodinvolving measurement at the Soret band (400-420 mm) to detectmethemoglobin (.15%) in trace amounts of hemoglobin mixture(.0,3umol/l). We demonstrated methemoglobin in colorless CSFsamples in 9% of 454 patients with cerebrovascular pathology andin 4% of 449 patients with other neurological disease (n=449).Ina group of 21 patients with verified acute cerebral hematomasmethemoglobin was confirmed in 66% of colorless CFS samplea afterultrafiltration. We conclude that routine spectrophotometricanalysis of CSF samples is very useful, alowing detection ofxantochromic compounds of patients with small cerebral andsubdural hematomas, as well as in those with minimalsubarachnoidal hemorrhages, hemorrhagic infarctions, or bleedingsfrom aneurysms and neoplasms.

    Summary: Cerebrovascular diseases make major problem to the humanitynowddays. They are ranked third in the last of death causes,following cardiovascular and malignant disease. Rheology isdefined as a study on blood flow and deformations of corpuscleblood element and of the medium where corpuscle ellements aresituated.The investigation into factors influencing cerebralblood flow is significant to pathophysiological researches intoischemic cerebrovascular disease. This study was aimed to at thevalue of fibrinogen and blood plasma osmolarity. Along withevaluation of fibrinogen and blood plasma osmolarity, the degreeof motoric deficiency and consciousness disturbances weremonitored. All indicators set above, were being determined duringthe first 24 hours after the onset of the disease, then after 72hours and llastly on the ninth day of the disease. The sameprocedure was applied to the controls. The group of investigatedpatients as well as control one, consisted of 20 men and 20 womenfrom40-70 years of age. On the basis of the investigation into the above indicators, the conclusion about fibrinogen values inplasma of great significance in diagnostics and further follow-upof cerebrovascular insult was reached. Within first few daysafter the insult, blood plasma osmolarity values are ofparticular significance since after, these vallues becomeinsignificant for the further follow-up of ischemiccerebrovascular disturbances.

    Title: Lamotrigine as an add-on drug in management of refractory primary generalized seizures

    Summary: The antiepileptic effect of lamotrigine was assessed in an opentrial performed in 16 adult patients with refractory primarygeneralized seizures. Lamotrigine was added to the existingantiepileptic drugs. The dose of lamotrigine varied from 200-400mg daily. All patients completed the trial. The analysis of totalseizure counts during the 12-week treatmant period withlamotrigine showed a statistically significant reduction in theseizure counts (p<0.01). Nine patients experienced a 50%reduction in the frequency of seizures and two patients wereseizure-free. In four patients, there was no benefit from thelamotrigine treatment. Lamotrigine was well tolerated by allexcept two patients in whom adverse effects, i.e. drowsiness andthe loss of appetite with a body weight loss after 12 weeks oftherapy, were observed. No hematologic or biochemical changeswere recorded

    Summary: A comparative study of research to date was performed, withdetermination of possible differences between patients withcertain disease of assumed genetic etiology or unknownpredisposition and comparative group of healthy population fromZagreb. By sellecting a particular cluster, the healthypopulation clearly separates from diseased persons, while in thegroup of diseased person three clusters separate with particularcycles of the disorders themselves. We consider that it isnecessary to continue this research in different populations withmore diseases, and analysing quantitative dermatogluphic traitsas well, using other biostatisical methods.

    Summary: Poligenetically determined predisposition to multiple sclerosis(MS) defnes the way of immunological reaction to enviromentalfactors and leads to clinically manifest disease.Although theconnectionbetween MS and some loci of HLA system has beenestablised the hereditary predisposition to MS remains to beelucidated. We determined the phenotypesof monogeniccharacteristic linked to the surface of red blood cells that wereobtained from 46 MS patients and 458 healthy subjects. Theantigens on the erythrocytic surface of the ABO, RH,MN.Ss,Kell,Kidd, Dufy P and Lewis system were analyzed. Our results demonstrate that MS patients differ from normalsubjects with regard to the RH, ABO and Lewis erythrocyticantigens.

    Summary: steeply which requires proper diagnosis particularly in the earlystage of the disease.Although the majority of patients sufferingfrom dementia have Alzheimer's disease characterized by slowlyprogressive course searching for treatable causes will enableeffective support to a certain number of patients. Presence ofonly one clinical symptom and/or sign, like the memory impairmentis not sufficient for diagnosing dementia owing to itsnonspecificy. Proper diagnosis depends upon one's familiaritywith the criteria clinical features, associated symptoms and thenatural course of the disease as well as wide variety ofdifferent etiological factors manifested by similar clinicalpicture. In spite of certain shortcomings, the flow chart systemapproach of diagnostic procedure is intended for more accurateand less time consuming assessment of the clinical diagnosis ofdementia.Understanding and familitary with different aspects of the clinical picture lie behind rationale approach and selectionof various laboratory methods and tehnics to confirm or excludepresence of disease for the benefit of patients and avoidunnecessary wasting of considerable amounts of financialresorses.

    Summary: In children aged 1,5 to 15 years, clinical examination, thermography, and sensory and motor neurography of at least three nerves were performed. The authors studied 6 children with Guillain_barre syndrome, 1 with acute myeloradiculoneuritis, 3 with chronic demyelinating polyneurpathy, 2 with chronic bilateral lumbosacral plexitis and 2 with hereditary motor and sensory neuropathy type I and II. Infrared thermography and electroneurography were performed 7-20 days after the beginning of clinical symptoms. Thermographic asymmetry were in positive correlation with sensory nerve abnormalities and less so with the motor, imaging sensory disturbances that are usualy not attainable in children.

    Summary: A group of five patients who suffered an ishemic cerebrovascular accident (CVA) were studies between two and five months post-incident. They all were approaching or in complete recovery of motor function at the time of the study. Fine motor performance, attention and short-term memory were assesed and compared with performance of a comparable group of healthy volunteers. Although the small siza of the subject groups preclude any general conclusions to be made, the data obtained suggest that patients recovering from CVA, who demonstrate minor neurologic deficits on standard testing, still suffer from persistent, albeit subclinical, motor disturbances, a short-term memory deficit and other attention difficulties.

    Summary: Authors have observed a 25-year-old female patient who developed the first signs and symptoms of the peroneal muscular atrophy at the age of 10. Her mother was diagnosed as Charcot Marie Tooth disease type 1 too, her sister had the same disorder and is also in compensated status. Her son developed first signs in his early childhood. Ewing's sarcoma was diagnosed one year later and several months after that patient died. The question to be asked is whether the second generation is also more susceptible to other disorders as it is obvious with the described family.

    Summary: Neurovegetative tests of cardiovascular function are based on theresponse of heart-rate and blood pressure to variety of stimuli. Those testa are simple and nonivasive, reflective, if present,the damage of autonomic nervous system. In this work we tested183 patients who suffered mild to severe head trauma with batery of neurovegetative testa: orthostatic pressure reaction,cortical activation, Valsalva manuver, and respiratrory sinusarrhytmia. In our group 48,8 % of patients had pathologicalfindings. Our conclusion pointed out that patients who sufferedhead trauma have more often damage of autonomic nervous functionthan expected in healthy population, and tah neurovegetativetests can help in diagnosting of subclinical posttraumaticencephalopathy.

    Summary: CCTT findings have been analyzed in 35 patients with arterio-venous fistula due to chronic renal insufficiency andintermittent hemodialysis by means of computerized colorteletermography (CCTT). The location of AVF was in cubital (15),radial(10), and brachial (10) vascular system. The anastomoseswere predominantly of "Y" type. We compared the findings of CCTTin both hands, in vatious types and locations of AVF. CCTTfindinfs showed marked hemodynamic changes with impressivedifference in hand with and without AVF, and are helpful inearly, pre-clinical diagnosis of hemodynamic changes in handwith AVF.

    Summary: DST findings has been analyzed in 90 patients with primary andsecondary Raynaud's syndrome in the labaratory for autonomicfunctions of Department of Neurology in Zagreb, by using athermographic device Thermovision 782 Agema. In both groupclinical findings, native CCTT (computerized colorteletermography) findingsand DST recording after cold wateractivation were correlated. Our analyses showed a statisticallysignificant difference between patients with primary Raynaud'ssyndrome(PRS) and secondary, with repetitive vibration provokedRaynaud's syndrome (VRS) group. Thermograpic amputation inanative recordings was more pronounced in PRS, while anisotermiawas more often in VRS group. The difference was smaller aftercryoactivation, but still significant. Thermographic examinationand clinical correlation alow not only differentiation of thesetwo syndromes but also new aspects of consideration of autonomicnervous system function insufficiency.

    Summary: CCTT has been developed as an alternative technique of recording the status of nerves with cutaneous distribution. Skin temperature reflects disturbances in peripheral nerve function of sympathetic vasomotor control being visualised as skin temperature changes. CCTT has been performed in 6 children aged 1,5-15 year with GBS 7-20 days after beginning the clinical picture, and correlated with electroneurographic findings. Hypothermia mostly pronounced on lower extremities was found in all patients with GBS. Sensory disturbances together with paresthesias and radicular pain correlated with hypothermia on upper and lower extremities in two patients. Only one patients with normal sensory neurographic findings showed slight hypothermia localised distally on upper extremities. Distal leg hypothermia was associated with low distal CAMP and prolonged distal latencies in five patients. Decreased motor conduction velocities were associated with distal leg hzpothermia in 3 patients. Proximal conduction block on upper extremities correlated with hypothermia on upper extremities in 3 patients. Thermal amputation of hands and legs was found in a child with generalised absent sensory nerve potentials, proximal conduction block and slightly decreased MCV. Thermal abnormalities were found in all patients with GBS correlating better with sensory nerve than with motor nerves abnormalities. This observation lead to the conclusion that thermography demonstrates the parallel course of somatic sensory and sympathetic nerve fibbers and dependency of skin temperature on vasomotor tone. Therefore sensory disturbances could be detected by colour changes in thermographic images. Correlating CCTT and EMNG significant difference exist while EMNG evaluates alpha motor and beta sensory fibbers, and thermography is demonstrating information relative ti the sensory autonomic or C fibbers.

    Summary: Damaged tissue in either hemisphere is supposed to exaggerate the emotional and thinking patterns characteristic for that hemisphere. Wada registered that examinees whose left hemisphere has been inactivated report feeling of depression more often that their right hemisphere counterparts. In our material we used Plutchik and Kellerman's Profile Emotional Index (PIE) for the evaluation of lateralisation in patients of both gender suffering from cerebral infarction. Investigation was carried out on 56 individuals with stroke, divided in two groups: 1)28 examinees with left hemisphere infarction, average age 56,4+/- 9,47 years and 2)28 examinees with right hemisphere infarction, average age was 52,93+/-11,67 years. Interhemispheral differences in emotional reactivity were registered and expressed for each of 8 analysed personality habitual patterns and qualities of behaviour.

    Summary: Twentz-three healthy subjects(control) and 29 patient with multiple sclerosis were studied. In control group there were 6 male and 12 female, mean age 38+/-12,5. In patient group there were 12 male and 17 female with mean age 40+/14 years in patient group. Diagnosis of multiple sclerosis had been established on the basis of Poser criteria and neurological state was assessed according to the Kurdske Disability Status Scale(KDSS) and EDSS. None of patient and controls had clinical signs of cardiac disease or abnormalities in the EKG at rest. Orthostatic hypotension was found in three(10,3%), borderline values in (31%) patients. None of control group have Orthostatic hypotension. Variation of pulse reaction was abnormal in 20(6,8%) patients and slightly normal in 4(17%) examinees of control group. Valsalva ratio was abnormal in 18(62%) pateints(12 women, 6 male), and in 6(26%)examinees of control group. RSA was abnormal in 13(45%) patients with multiple sclerosis and none in control group. Results of study indicate that a significant proportion of these patients have evidence of autonomic dysfunction affecting the cardiovascular system. Autonomic disturbances could be results of patchy lesions in central connections of reflex pathways. The fact thatplaques in multiple sclerosis a common in periventricular region where the neurones regulating cardiac and respiratory functions are located support our observations. However lack of correlation between the abnormalities observed in the other tests demand further discussion.

    Summary: Fiftynine patients with Parkinson disease were divided into three gruops. According to Hoehn and Yahr rating scale and previous treatment in the first group (Hoehn and Yahr I-II) 18 patients, without previous tretment, in second group (Hoehn and Yahr II-III) 11 patients with anticholinergic therapy and in the third group (Hoehn and Yahr IV) 30 patients with Levodopa therapy (Madopar). Were selected Ortostatic pressure reaction (OPR), heart rate (HR), cortical activation (CA), Valsalva manoeuver (VM) and respiratory sinus arrhytmia (RSA) has been tested. Abnormality of RSA was the most pronounced pathological parameter in all three groups. Abnormalities of OPR and CA and HR pointed to the abnormality of efferent sympathetic pathways and they were present in relativly constant not high percentage in all three gruops. In concordance with similar studies they may indicate a central lesion of sympathetic pathways, but the fact that there is no pathological correlation for such explanation make this conclusion speculative, and demand further investigations in autonomic nervous system disorders in Parkinson disease.

    Summary: We have observed five patients with myasthenia gravis and localiyed atrophy. The first signs of muscular atrophy were usually perceived after five or ten years treatment only with anticholinesterase drugs. Three of them received only anticholinesterase therapy throughout a period of ten to twenty years. When we started to treat those patients, the immunosuppressive therapy was introduced. The activity of the affected muscles has shown mild improvement after three to four months of immunosuppressive therapy. Extensor muscle groups of the hands and feet have been perceived as more frequently given to atrophy. There is only one patient with anticholinesterase drug and immunosuppressive therapy and he developped a mild tongue atrophy after sixteen years of the disorder beginning. Only an anticholinesterase drug therapy may contribute to localized muscle atrophy but it is difficult to find an explanation for the special incidence of the extensor muscle group change.
