Summary: This research is concentrated on evaluation of clinical
values of esophageal path and approach in diagnostics and treatment of
heart disease. We will test the role and place of transesophageal ECG as
addition to standard ECG in discovering and grading of ischemic heart
disease and heart arrhythmias. The applicability and clinical importance
of esophageal cardiac (atrial) electrostimulation as noninvasive and
intracardial ECG competitive diagnostic and therapeutic method in some
forms of ischemic heart disease and cardiac rhythm disorders will be
tested as well. And finally, we will be examining the value of
transesophageal echocardiography in finding and estimating the severeness
of ischemic heart disease and especially its role under conditions of
dynamic testing by applying transesophageal electrostimulation, and for
appropriate choice of antiischemic therapy. In the end we are interested
in all side-effects, complications and possible damages by using
electroenergy on mucous membrane of esophagus during TEAS.
Research goals: The central part of this project is the clinical
usage of transesophageal atrial stimulation (TEAS) which has already
produced certain positive experience in our laboratory. We will be testing
the diagnostic value of this procedure in examining the function of sinus
and AV node in bradyarrhythmias, tachyarrhythmias and especially stressing
the possibilities of inducement and termination using TEAS. The next aim
of this research is to test role of transesophageal ECG as in arrhythmias
so in the coronary heart disease as well, especially with purpose of
discovering AMI and ischemia being located in the posterior left ventricle
wall. Area particularly we are attracted to is transesophageal
echocardiography. We will examine its role in discovering and grading
coronary artery disease finding left ventricle contractility disorders
during by TEAS induced ischemia.
