    Title: Prevention of left ventricular thrombi in acute myocardial infarction with low-dose heparin and aspirin: A prospective randomized trial

    Summary: 80 patients with the first episode of transmural acute myocardial infarction (AMI) entered a double blind controlled clinical trial aimed at assessing the effectiveness of low-dose subcutaneous heparin together with low-dose oral aspirin in preventing left ventricular (LV) thrombi. Seven patients died and were therefore excluded shortly after being included in the study. The remaining 73 patients who were divided into two groups were followed through the acute and subacute phases of AMI. 32 patients in group I were treated with 2x5000 units of subcutaneous heparin and 100 mg oral aspirin daily, starting at least 24 hours from the onset and continued to the fifth hospital day of AMI. 41 patients in the control group received placebo. Serial 2-D echocardiograms were performed to detect LV mural thrombi. 31% of patients in group I (10/32) and 41% in group II (17/41) developed LV thrombi on 2-D echocardiograms. There was not statistical difference in the incidence of LV thrombi between the twogroups (p>0.05, Xý=0.81). These results suggest that low-dose heparin s.c. in combination with 100 mg oral aspirin daily do not prevent LV thrombus formation in the early course of AMI.

    Summary: The aim of this study was to determine clinical value and a feasibility of transesophageal atrial pacing (TAP) in diagnosing of myocardial ischemia in patients with a coronary artery disease (CAD). Forty patients with CAD and with significant ST depression in a 12-lead ECG during bicycle-stress testing underwent TAP. Rapid atrial stimulation was performed using very flexible six-polar polyurethan pacing lead introduced through the nares into the esophagus and connected to the stimulator which allows programmable selection of rate, output voltage and pulse duration. Of 4O patients underwent TAP ischemic ECG changes wereinduced in 35 and were similar to that occurred during bicycle-stress testing according intensity and affected ECG leads with comparable peak rate-pressure products. Four patients had negative test for myocardial ischemia and in one TAP was interrupted because of intolerable chest discomfort. We concluded that TAP is reliable alternative technique for the assessment of CAD. In combination with some other noninvasive methods (echocardiography, scintigraphy, i.v. digital angiography) TAP became widely used diagnostic procedure in cardiac patients.

    Summary: In order to evaluate LV diastolic function during myocardial ischemia in 30 patients with CAD we provoked myocardial ischemia using rapid TEAS. In the same time assessing of LV diastolic function was estimated by measuring transmitral flow velocity using Doppler echocardiography from apical four-chamber view. We measured peak and time velocity integrals of early (E, Ei), late (A, Ai) and total transmitral flow (VTI) and their ratios as well. We have observed different patterns of left ventricular diastolic filling correlating primarily with the degree of CAD, LV systolic function and by ischemia induced mitral regurgitation. We concluded that TEAS together with Doppler is very usefull method to evaluate the effects of myocardial ischemia on the LV diastolic function.

    Summary: 30 patients with CHD underwent this study. Myocardial ischemia was induced by rapid TEAS and transmitral flow measured by PWD from apical four chamber view.     Summary: 40 patients with CAD and positive bicycle stress testing, underwent rapid transesophageal atrial stimulation (TEAS). In 35 patients we induced ECG ischemic changes comparable with that on stress testing. We concluded that rapid TEAS is useful diagnostic procedure in detection of myocardial ischemia.
