Summary: The questionnaire on general condition of health (FDI 1989), filled in by the patient himself, is of a great contribution in establishing a patient-dentist relationship. The questionnaire enables the patient to recollect in peace the data on his or her diseases and also instigates the patient to convay to the dentist his or her possible being infected, espetially by the HIV virus. To the dentist, on the other hand, the questionnaire is a useful set of guidelines toward an all-encompassing anamnesis, the therapy plan, the patient protection procedures and the infection prevention measures. A negative answer produced by the patient relating to his or her contagiousness does not at all free the entire dental team from the obligation to use pretection gloves, masks and overalls at work.

Summary: The root canals of the single rooted teeht were obturated with Ultrafil technique, modified Trifecta technique with both SuccessFil gutta-percha and Ultrafil regular set gutta-percha, and standard lateral condensation technique. The sealing ability was evaluated according to the extent of methylene blue dye penetration into the apical canal space. There was no significant difference in apical dye leakage among these three techniques however, overextrusion of the gutta-percha was found in 80% of the samples obturated with Ultrafil technique. Modified Trifecta technique seems to be effectiveas well as lateral condensation technique in both apical sealing ability and extrusion control.

Title: Prevention of the microleakage at the time of root canal obturation using a laser

    Authors:
    Anić, Ivica (159102)
    Journal: Dental Outlook
    Number: 2
    Volume: 86
    Year: 1995
    Pages: from 169 to 173
    Number of references: 22
    Language: japanski
Summary: The high temperature at the target area induced by argon, CO2 and Nd:YAG laser can produce vaporisation of the created smear layer and melting of dentine. Such modified surface can cause the changes in permeability of the dentinal tubules. Before the intracanal lasing, the smear layer should be removed. If not, the laser energy can disturb the surface of the smear layer, and as a consequence increase of the permeability instead of decrease can develop. This can be express particulary in the middle third of the root canal when laser beam is parallel rather then perpendicular to the canal wall. Inside the root canal the output laser power should not exceed wW and the pulsed or repeated mode should be used. The single irradiation time should not exceed one second. However, intracanal heat produced by the laser can be used for softening the gutta-percha particle positioned at the apical stop. The argon laser may be a better choice for this purpose than CO2 or Nd:YAG lasers.

Title: Laser polymerization of the composite resin inside the root canal by argon laser

    Authors:
    Anić, Ivica (159102)
    Journal: Dental Outlook
    Number: 2
    Volume: 86
    Year: 1995
    Pages: from 222 to 225
    Number of references: 22
    Language: japanski
Summary: The argon laser has emission lines at 488 and 514.5 nm which should make it well suitable for initiating the polymerization of light activated composite resin. Homogenous penetration of the composite resin by argon laser reduces shrinkage towards the curing light and some properties of the resin can be enhanced. This can be accomplished with only about 10 s of polymerization time. Another advantage of the argonlaser is the possibility of argon light beam to transmit through optical fiber that allowed introducing the laser energy into the root canal. In order to approve the penetration of the composite resin into the dentinal tubules the smear layer, created during instrumentation, should be removed. After removing the excess bonding agent, composite resin should be plugged into the canal. Laser action should be started in a front of the canal orifice and than the fiber must be introduced into the canal and slowly pushed toward the apical stop. Second lasing is performed during withdrawal of the fiber. After withdrawing the fiber, an additional amount of composite resin should be plugged into the canal and polymerized in the same manner mentioned above.

Additionaly, the effect of polishing of the amalgam filling on elevtrical potential, as well as the presence pf prosthetic metal and their influence on electrical potential were also investigated. The measuring of electrical potential was performed at the gingiva and fresh amalgam filling. After seven days, the potential was measured again at the same fillings before and after polishing of the amalgam surface. The difference in electrical potential between the polished and non-polished amalgam fillings was statistically significant in all tested dental amalgams (p<0.01). Presence of prosthetic metals in oral cavity was nod influenced on the electrical potential of amalgam filling. It was observed that electrical potential of amalgam fillings lowers due to the ageing and polishing.

Summary: Diet, dietary habits and working environment belong to ecologic factor that have influence on dental health. This epidemiologic study was conducted to get an insight into dental morbidity and possible differences in the frequency and spread of caries. Three groups of workers were examined:122 confectionery industry workers, 121 slaughtering industry workers and 126 metal manufacturing industry workers. Results are presented using internationally adopted parameters for dental health recording: DMF, DMFS, Caries Prevalence and DMFT. Data analysis revealed the confectionery industry workers to have consideravly higher values of mentioned parameters than other two grouops of subjects. DMFS and DMFT were 19.5 and 63.0% in confectionery industry, 14.3 and 48.8% in slaughtering industry and, 11.8 and 38.3% in metal manufacturing industry, respectively. The observed differences were atributed to dietary habits and working environment. Confectionery industry workers were considered to be at risk for dental morbidity.

Summary: This in vitro study evaluated the ability of commercial amalgambond liner to reduce microleakage between amalgam restorations and tooth structure. At the occlusal surface of the 24 extracted human third molar, conventional Class I cavity was prepared. The teeth were randomly divided in two groups. Cavity walls of the samples in the first group were covered with two layers of Amalgam liner before the amalgam was packed into the cavity. In the second group, the cavities were filled only with amalgam. After 7 days in 0.9% saline solution at 37 degree C, the samples were thermally stressed by subjecting them to 300 thermocycles between 0 and 55 degree C bath. After that, the teeth were immersed into the 1% methylene bluedye solution for 7 days. The teeth were sectioned in a buccal-lingual direction through restorations. The issue was examined by direct measurement of methylene blue dye penetration between amalgam and cavity walls. Amalgam fillings prepared with Amalgam liner demonstrated statisticaly significant less dye penetration (p<0.05) compared with those without liner.

Summary: Transverse and tensile resistance of a dentin-bonded composite resin was tested. The composite resins (Fluorocore, Coradent and Coracore) were retained routinely by a dentin adhesive (Syntac) on dentin samples. The specimens were mounted on a mechanical testing device, and each of the 23 specimens were loaded perpendicularly to its long axis. The transverse force was applied at a crosshead speed of 0.5 mm/min until failure occured. The next 22 specimens were loaded in parallel with their long axis in the same conditions. All three materials showed satisfactory compressive shear bond strength, but transverse resistance was inadequete and restorative pins had to be used.

Summary: Fifty-one patients were investigated in order to determine the presence of Trichomonas tenax in various oral structures (saliva, dental plaque from physiological clean and unclean tooth areas, carious dentin, root canal contents). Samples were cultivated in liquid Diamond's axenic broth medium. The results were considered positive if living oral Trichomonas was found in smears. The prevalence of Trichomonas tenax in the study patients was 35,3%. Trichomonas tenax was most frequently found in dental plaque (23,2%) and in the root canal content (30,0%) (p>0.05). The variable observed in this study (age, sex, oral hygiene, gingivitis, number of carious teeth, smoking and coffee intake) did not influence the presence of Trichomonas tenax. According to the results obtained, a mature dental plaque appears to be the oral structure favoring the growth and survival of oral Trichomonas in the physiologic conditions. It is possible that trichomonas tenax spreads from dental plaque into diseased endodontium.

Summary: Dental amalgams has been the material of choice for the posterior fillings for the past 150 years. Presented in this paper are the historic developments of amalgam, its obligatory composition, as well as the division of different types of amalgam according to the quantitative content of copper and the shape of the particles. Also, the properties of amalgam with special regard to its toxicity are listed. It is well known that, over past few years, attacks on dental amalgam have been intensified, both in scientific and popular literature, concerning its possible pathologic impact on human body. Enumerated in the paper are recommendations by the American Dental Association (ADA) on the use of dental amalgam. In view of the scientific research available, there is no scientifically founded evidence of the amalgam being noxious.

Summary: The study deals with a complex relationship of an altered saliva quantity and pH on caries appearance in individuals suffering from diseaseof the upper digestive tract (n=101) in relation to healthy individuals (n=78). Greater caries prevalence was found in individuals suffering from diseases of the upper digestive tract (p<0.01).. Caries prevalence was significantly greater (p<0.01) as stimulated salivary secretion and pH was lower in the both groups. There was no significant difference in caries prevalence and different pH of nonstimulated saliva in the control individuals (p>0.05). Individuals suffering from cronical disease of the upper digestive tract are exposed to an increased caries risk when accompanied with a longlasting or frequent low salivary flow and decreased salivary pH.

Summary: Endodontic treatment has biological economical and social advantage over tooth extraction and prosthetic substitution. Root canal length measurement, as a part of endodontic treatment, is considered as "conditio sine qua non" for efficient endodontic therapy. Along with x-ray and tactile.sense method, electronic method of root canal length measurement is helpful in dentistry. Physiological foramen localisation is based on different electronic resistance in hard and soft tissues. However, observed results of endodontic treatment in dental office without x-ray apparatus, where measurements were done by Dahlin dentometerm show 20% obturation to physiological foramen and 62.5% to apical foramen. We can conclude that for the successful endodontic treatment electronic method is not good enough such as combination of electronic and x-ray method.

Summary: In total, 265 resin fillings on the anterior teeth of the third, fourth and fifth classes in patients aged >20 years were clinically examined. Changes in the color and marginal color, marginal adjustment and filling morphology, and the occurence of secondary caries were assessed using the USPHS method parameters. The filling materials were found to be more resistant to wear by use than to changes in the filling color and marginal color. Low success was more frequently recorded with poor then with good oral hygiene. In 7% of the fillings, changes in the color and marginal color, poor marginal occlusion and secondary caries occured within not more than a year, which is criticaly high percentage of failure.

Summary: Periapical cyst biopsy was performed in 16 subjects to assess their immune response. Total proteins were determined by Lowry's method, and IgG, IgA and IgM by radial immunodiffusion. Immunoglobulin walues were expressed in mg/g total protein. The study showed IgG, IgA and IgM to be present in different percentage and amounts. IgG and IgA were found in 100%, and IgM was detected in 33% of the samples. Mean values of IgG, IgA and IgM in periapical cyst were 159.4, 25.6 and 1.8 mg/g total protein, respectively. The values of IgG, IgA and IgM found in the periapical cyst suggested their possible local synthesis and immune response, which may play a role in the pathogenesis of periapical cysts.

Summary: Measurement of the apical foramen position was done with the Endometer ES-02 device. The device contains the scale with the sign AF (apical foramen), which represents the foramen physiologicum. Ninghty teeth with 171 root canals were examined. After extirpation of the pulp, the length of the root canals were measured by the Endometer ES-1 device. After the endodontic treatment the root canals were obturated with adequate remedies (Apexit and gutta-percha points ) and X-rayed. Control radiographic showed that the accuracy of the Endometer ES-02 was very high (97.7%).

Summary: Chronic periapical lesions are body defense reaction to the irritans comming from the root canal. Pathohistologyc stady of chronic periapical lesions, that were removed surgecly was done. The results show that only 12.1% of them were cysts. Rest of surgecly removed lesions were dental granulomas. Such a small amount of radicular cysts found in this samples, led us to the conclusion that root canal therapy should be done with more precisen and carefulness.

Summary: The effect of carbon dioxide and Nd:YAG laser beams on the extracted human teeth were examined. Laser effect on dental tissue is manifested as a consequence of temperature changes. Different wavelengths are absorbed differently by dental tissue. The obtained results indicate that the Nd:YAG laser beam causes bigger destruction on dentin (carbonification and cracks) when compared to the CO2 lasers of the same energy. Temperature changes of the enamell under influence of the CO2 laser were measured by the indirect method. X-ray diffraction analysis revealed alfa Ca3(PO4)2 (but not Ca4(PO4)2O) which is a high temperature modifaction of heated human dental enamel. This high-temperature modification of the human enamel indicate that the enamel was melted on the temperature higher than 1000 degree C at the area of the CO2 laser beam applications. Contact temperature changes were measured before and during lasing by inserting the contact digital thermometer probe inside the pulp chamber in previously prepared spaces at the bifurcation level and at the enamel surface on the approximal side of the crown. CO2 laser energy applied at the bottom od the Classs I and III cavities vaporised and carbonised dentin with very little influence on the pulp chamber temperature (2.5 degree C). The temperature was mormalised within 40 to 60 s.

Summary: The project has been conducted to investigate the morphologic, temperature and permeability changes of the root canal dentin wall induced by argon, CO2 and Nd:Yag lasers. During this stage, the two completely new root canal obturation techniques have been developed. The first technique is based on the intracanal heating of the apical constriction positioned gutta-percha using all three laser devices. The argon laser showed better results than other two lasers. The second technique is based on the intracanal photopolymerization of the composite resin positioned in the apical portion of the root canal. In addition, the possibility of using the KCP-2000 (the product of the American Dental Laser industry) for air-abrasing of the dental enamel and dentin has been used. The results have been analysed by SEM.
