Summary: In a prospective, randomized, clinical investigation
approximate ly 500 persons, including school children, adolescents and
adults , with moderate arterial hypertension and diastolic blood press
ure below 115 mm Hg, will be studied. In the group of school chi ldren
and adolescent persons, we shall continuously investigate the following
parameters: prevalence and incidence of arterial hy pertension, standard
and relative weight, serum cholesterol and triglycerides, as well as the
influence of physical activity on those facors. The children with positive
family history of hypert ension or myocardial infarction will be also
followed up. The role of the human atrial natriuretic factor (ANF),
endoxin, a nd insulin in pathogenesis of hypertension will be assessed.
The group of adult hypertensives will be randomly assigned to either
diuretic or beta-blocking agent during six months, followed by the
alternative six months therapy regime. Changes in blood pressure,
cholesterol and triglyceride levels, electrolytes and serum glucose, as
well as the potential vasoactivity of those antihypertensives, will be
studied. In the third group of hyperte nsives with left ventricular
hypertrophy, four different antihype rtensive drugs, and their impact on
regression of hypertrophy, wi ll be investigated. 24-hours blood pressure
monitoring will be us ed in the fourth group of mild to moderate
hypertensives in order to assess the prevalence of the false positive
results and especially the prevalence of the "white coat hypertension", as
well as to determine the possible prognostic implications of such
results.

Research goals: 1. To assess the influence of the endoxin, atrial
natriuretic fac tor (ANF) and insulin on blood pressure. Measuring the
baseline v alues of the ANF and endoxin, as well as the values induced with
the glucose and insulin, before and after antihypertensive treatm ent, to
evaluate the influence of insulin, and insulin-induced hy poglycaemia on
ANF and endoxin levels in normotensive and hypert ensive examinees. 2. To
assess the family aggregation of the cardiovascular risk fa ctors in school
children and adolescents. To point out the most a ppropriate diagnostic and
therapeutic procedure. 3. Using the 24-hours ambulatory blood pressure
monitoring to evaluate the earlier established hypertension, and to assess
the prevalence of the "white cout hypertension" in the region of Dalm atia.
4. To assess the efficiency and metabolic side-effects during the log-term
treatment with tiazide diuretics and beta blockers. 5. To determine the
effects of four group of antihypertensives (i.e. diuretics, beta blockers,
calcium channel blockers and vaso dilatators) on regression of left
ventricular mass, diastolic fun ction, and especially excitability of left
ventricle.
