    Summary: The influence of atmospheric factors on the frequency of varicealbleeding was studied from April 1, 1984 to March 31, 1990,consequently trough 2191 days. The everage daily atmosphericpressure, average daily temperature and the relative humiditywere correlated with episodes of bleeding from esophageal andgastric varices. There were 571 cases of bleeding varices. Theanalysis showed that there were 483 days with bleeding fromvarices and 1708 days without bleeding. In conclusion, the changes in atmospheric pressure and relativeair humidity are significant biotropic factors in patients withvariceal bleeding. It should be emphasized that it is necessaryto distinguish the correlation of phenomena and their causality,thus it would be erroneous to state that the atmospheric pressureand the air humidity, as isolated variables, can be essentialfactors in ther pathophysiology of variceal bleeding.

    Summary: The influence of atmospheric factors on the frequency of bleedingfrom the peptic ulcer was studied within the period from April 1,1984. to March 31, 1989, consequently through 1826 days. Theeverage daily atmospheric pressure, the everage daily temperatureand the relative humidity have been examined. There were 1102cases of bleeding peptic ulcers, 537 bleeding gastric ulcers and565 bleeding duodenal ulcers. During the study period there were454 days with bleeding from gastric ulcers and465 days withduodenal ulcers. There were 793 days with bleeding from eitherulcer. We concluded that the incidence od bleeding from thepeptic ulcers of the stomach and duodenum correlates in greatmeasure with low atmospheric pressure in the days prior tobleeding and in the days of bleeding, as well as with fallatmospheric pressure in the days of bleeding with respect ofprevious day. It is necessary to distinguish the correlation ofphenomena and their causality, thus it would be erroneous tostate that the atmospheric pressure as an isolated variable canbe eseential factor in the pathophysiology of ulcer bleeding.

    Summary: We report a case of fulminant toxic hepatitis as a result of adverse effect of tizanidine wich was used as symptomatic therapy for amyotrophic lateral sclerosis. The onset of fatal fulminant liver insufficiency occured 8 weeks after starting the drug. Clinical signs, laboratory parameters and pathohistologic findings suggested a rare idiosyncratic reaction caused by the production or accumulation of hepatotoxic tizanidine metabolites.

    Summary: A diagnostic method rarely used in practice has been described.The procedure is indicated in patients in whom endoscopicretrograde pancreatography was ineffective out of various reasonsor where there is a specific indication for presentation of thewhole pancreatic duct. Percutaneous pancreatography has been undertaken in 17 patients,and it was efficacious in 14 (82%). It was uneventful in all ofthem. We conclude that this method is very valuable in thediagnosis of chronic pancreatitis and pancreatic tumors.

    Summary: The aim of this prospective study was to establish wheter thereare changes in the diameter of the portal vein and portalvenous blood flow velocity during elective sclerotherapies ofesophageal varices, measured by color doppler, and wether, incase these changes occur, they are helpful in assessing varicealbleeding. In 30 patients who already presented with varicealbleeding, the hemodynamic parameters were followed up at thear admission and prior to each elective sclerotherapy.Sclerotherapies were performed every 7 day until a satisfactoryresult was achieved, i.e. coplete obliteration, an avarage of 3sclerotherapies. Results of statistical anlysis showed thatchanges in the blood fow velocity in the portal vein were not ofstatistical significance, while changes in the diameter of theportal vein was noted as the sclerotherapies progressed. Howeverin 7 patients blood flow velocity increased for 50% after thesecond elective sclerotherapy and increase of the diameter of theportal vein was noted. The result show that in case of anelevated blood flow velocity and increased portal vein diameterthere is a risk of rebleeding. Noninvasive color dopplertechnique is evaluable method of followin up hemodinamic changesof the portal vein.

    Summary: The Forrest s classification of peptic ulcer bleeding, witch isthe most frequently applied, does not identify the mode, naturalcourse and activity of bleeding as well as emergency ofendoscopic hemostasis. Therefore we have made our own scheme forthe division of endoscopic bleeding. Effectiveness of hemostasis with respect to activity of bleedingwas analysed on a group of 315 patients. 160 patients underwentsclerotherapy and 155 laser teratment. Both methodes wereindentically effective in venous hemorrhage. In arterial bleedingIa, a significantly higher rate of definite hemostasis wasachieved by photocoagulation. In visible vessel sclerotherapy wasfound to be more effective than photocoagulation. In conclusion,according to endoscopic criteria of hemorrhage, laserphotocoagulation is superior to sclerotherapy in active arterialbleeding, and injection sclerotherapy to laser treatment invisible vessels.

    Summary: For management inoperabile malignant diseases of biliary tree we used, in 9 patients, percutaneous biliary drainage: 4 patients with carcinoma of pancreas, 3 patients with Klatskin tumor and 2 with metastatic carcinoma in porta hepatis. PBD was successful in 7 patients (78%)- after decopmression of biliary tree, all symptoms were regressed. PBD is simple and successful method for management inoperabile diseases of biliary tree.

    Summary: The aim of this trial was to assess the effect of somatostatin on the control of hemorrhage from gastric and duodenal ulcers in case when endoscopic methods were not efficacious and surgical procedure represented a risk due to other diseases. For endoscopic hemostasis, photocoagulation with ND-YAG laser or sclerotherapy with 1% polidocanol were used. If inicial hemostasis was not successful or recurrent bleeding occured twice within 48 hours, somatostatin at dosage of 3 mg every 12 hours over 72 hours was applied in those patientsin whom surgery represented a risk (cardiac and pulmonary patients, liver cirrhosis, renal transplantation). Of 321 patients with bleeding from gastric and duodenal ulcers admitted over period of 12 months, 19 (5,9%) were administered somatostatin. In 16 (84%) patients bleeding was successfully controlled, while 3 (16%) patients required operation. One (5%) patient died after surgery. Before administration of somatostatin, mortality rate overall was 33% related to bleeding or surgical procedure in patients who presented a risk. Conclusion; In case of unsuccessful endoscopic hemostasis in patients in whom operative procedure would represent a tisk due to other diseases, it is reasonable to use somatostatin, which significantly decrease mortality.

    Summary: ERCP is the procedure of choise in evaluation of pancreatic and biliary ductal anatomy. In some patients ERCP is unsuccessful, and ductal opacification is of great importance, especially in planning of surgery drainage procedures in chronic pancreatitis. 21 patients in which ERCP was unsuccessful, percutaneous pancreatography (PP) under US guidance was attempted. The procedure was successful in 18 (86%). In three patientsthe puncture of main pancreatic duct was unsuccessful. Only minor complications: transient hyperamylasemia in two, and contrast extralumination in one patient were observed. The authors conclude that PP is good and safe method for ductal anatomy opacification in patients with previously unsuccesful ERCP.

    Summary: The aim of the trial was to investigate the efficacy of bleeding arrest from gastroduodenal ulcers with injection of fibrin glue and the achievements of daily endoscopic control with futher injection of fibrin glue, the final stage of Forrest III. The results were compared with our own expirience with laser photocoagulation and injection sclerotherapy. 30 patients with Forrest Ia,b and Forrest IIa,b were included into trial. We have introduced throgh the endoscop Duo-probe and injected 1 ml of fibrin glue, directly besides the bleeding vessel, followed by daily endoscopic control with futher injection until Forrest III. Definitive hemostasis was achieved in 97%, 1 pt required surgery (3,3%), and neither of patients died. We have achieved Forrest III in 86% of patients after 48 hours, and in all patients after 72 hours. Comparison with other two methods, showed that all 3 methods are equally effective in achieving definitive hemostasis (97% in laser group, and 96% in sclerotherapy group), but rebleeding occured in 21% of sclerotherapy patients and in 10% of the laser group. 5% of sclerotherapy patients and 4% of the laser group required surgery. Mortality rate in both groups was 1,5%. Sclerotherapy was associated with slower ulcer healing. Conclusion: submucosal fibrin adhesion with daily endoscopic follow-up is an effective hemostatic method, The main advantage is the possibility of repeated injectino until Forrest III stage is achieved without damaging or desroying the tissue.

    Summary: Percutaneous ethanol injection therapy has been used with increasing frequency for curative and palliative treatment of hepatocellular carcinoma (HCC). Majority of HCC lesions has internal vascularity detectable by the Doppler colour imaging. We treated 10 patients with HCC topically in the area of the feeding tumor artery, doing five applications on everage. The everage amount of 96% sterile ethanol injected in each tumor was 70 ml. Six patients had significant fall of blood flow velocity in the feeding tumor artery after therapy. Two patients lost completely the Doppler signal in the tumor. The selective angiography in these two patients showed complete obliteration of tumor vessels. In our opinion, PEIT with ethanol of unresectable HCC guieded by Doppler colour imaging is an alternative to transarterial chemoembolisation.

    Summary: The aim of the investigation was to establish wether there are hemodynamic changes in the portal vein due to obliteration of significant pathways through esophageal varices and to examine the value of hemodynamic parameters in prognosis of rebleeding from esophageal varices. Endoscopic examination was carried out by means of a standard endoscope combined with duplex-sonography. Hemodynamic parameters the inner diameter of portal vein (IDPV), its cross-sectional area (CSA), mean blood velocity (MBV) and blood flow volume (BFV) as well as perfussion pressure gradient (PPG) were followed up by duplex-sonography. Sclerotherapies were performed every 7 days until complete obliteration had been achieved. Before each sclerotherapy duplex-sonography was performed. Forty five patients with liver cirrhosis and first episode of variceal bleeding were included. It was shown that hemodynamics of the portal vein was altered during elective sclerotherapies: IDPV and CSA are increased, wherease MBV decreased. The BFV and PPG were higher at the beginning of sclertherapy, and remined unchanged until obliteration. If variceal rebleeding occured, the IDPV increased for 26%, CSA for 46%, MBV for 40% and BFV for 100%, while PPG remained unchanged. Non-invasive duplex-sonography is a valuable method of following up hemodynamic changes in the portal vein and helpful in predicting rebleeding risk from esiphageal varices.

    Summary: Forty two year old female was treated with interferon and percutaneous ethanol injection therapy (PEIT) for the metastatic carcinoid of the liver. PEIT procedure precipitated flushing and serious tachycardia, wich promptly disappeared following administration of octreotide (0.1 mg 4 hours before and 0,1 mg immediately prior to PEIT). Sixty seven year old male with history of partial pancreatic resection for VIPoma 4 years ago presented with liver metastases and massive diarrhoea. PEIT was performed accompanied with significant worsening of diarea. Octreotide was administered with prompt normalisation of bowel movements. We suggest that PEIT for metastatic neuroendocrine liver tumors should be combined with octreotide therapy.

    Summary: The aim of the investigation was to establish wether there are hemodynamic changes in the portal vein after obliteration of esophageal varices and to examine the value of hemodynamic parameters in predicting the development of new varices followingobliteration. Following obliteration of esophageal varices, endoscopic examination was carried out every three months within the period of one year by means of a standard endoscopy combined with duplex-sonography. Hemodynamic parameters: the inner diameter of portal vein (IDPV), its cross-sectional area (CSA), mean blood velocity (MBV), blood flow volume (BFV), and estimated perfusion pressure gradient (PPG=8vn/q2 Pa/cm2) were followed up by duplex-sonography before each endoscopic examination. 35 patients were included in the study after obliteration of esophageal varices. It was shown that IDPV and CSA decreased, while MBV and PPG increased. However, BFV remained unchanged. These results suggest that there is an improvement of the liver function and decrease of portal hypertension in those who did not develop new varices. The development of new varices is followed by more pronounced changes in the portal system: IDPV decrease for 28%, CSA for 28%, whereas MBV increase for 28% and PPG for 58%. The BFV remains unchaged. There is no correlation between the development of new varices, liver function and the size of esophageal varices before elective sclerotherapies. Non-invasive duplex-sonography is a valuable method of detecting hemodynamic changes in the portal vein and helpful in predicting the development of new esophageal varices following obliteration.

    Summary: In this study, hemodynamic changes in the portal vein assessed by duplex-sonography during elective sclerotherapies of esophageal varices until their obliteration and one yeare following obliteration were followed up. The aim of our work was to establish whether there are hemodynamic changes on the level of portal system due to obliteration of significant pathways through esophageal varices, and what occurs in the portal circulation after obliteration of esophageal varices within one year. The objective of this investigation was also to examine the value of hemodynamic parameters in anticipating rebleeding from esophageal varices and the development of new varices following obliteration. Forty-five patients with liver cirrhosis and the first episode of bleeding from esophageal varices were included into the study. Their hemodynamic values were compared to those of the controls. The first control group comprised 30 patients with liver cirrhosis and non-bleeding esophageal varices, and second consisted of 35 healthy individuals. Endoscopic examination was carried out by means of a standard endoscope, and duplex-sonography by an ultrasound. A sclerosing agent was 1% polidocanol. Hemodynamic parameters were followed up by duplex-sonography: inner diameter of portal vein (IDPV), its cross-sectional area (CSA), mean blood velocity (MBV) and blood flow volume (BFV) as well as perfussion pressure gradient (PPG). Funcional state of the liver was determined according to the Child-Pugh criteria, and the size of esophageal varices according to Paquet. In patients who bled from esophageal varices in comparison with those who did not, there is a significant larger CSA and BFV. This two parameters, according to our results could be regarded as indicators of the patients condition at least within the seven days following esophageal hemorrhage. The results also suggest that the degree of liver impairment and indirectly the size of portal hypertension may be assessed by PPG. The hemodynamics of portal system is altered during elective sclerotherapies. The IDPV and CSA are increased, whereas the MBF is decreased. The BFV is higher at the beginning of sclerotherapy and it remains unchanged afterwards until the obliteration as well as PPG. Following obliteration of esophageal varices, within the period of one year, the IDPV and CSA decrease, while the MBV and PPG increase. BFV remains unchaged. These results suggest that there is an improvement of the liver function and decrease of portal hypertension in those who did not develop new varices. In case of rebleeding from esophageal varices, the IDPV increases for 26%, its CSA for 46%, MBV for 40% and BFV for 100%. PPG remains unchanged. There is direct correlation between rebleeding from esophageal varices, degree of liver impairment and size of esophageal varices. In case of development of new varices the IDPV decreases for 26%, its CSA for 28%, MBV increases for 28%, and PPG for 58%. The BFV remains unchanged. There is no correlation between the development of new varices, liver function and the size of esophageal varices before elective sclerotherapies. In conclusion, by follow up of hemodynamic parameter by means of duplex-sonography, and especially by the inner diameter of portal vein, mean blood velocity and perfussion pressure gradient the liver function, size of esophageal varices may be estimated and the rebleeding risk and development of new esophageal varices forseen.
