Summary: The aim of our research is to use and develop new
gastroenterological, endoscopic and ultrasound technologies that would
enable application of new diagnostic and therapeutical procedures. Thus,
more qualitative work and incorporation into the international knowledge
transfer would be made possible. The formation of hemodyamic laboratory,
make us able to determine the risk for the development of esophageal
varices and their hemorrhage, and establish the role of liver
transplantation as the final method in solving the consequence of portal
hypertension. The introduction of transjugular intrahepatic portosystemic
stent shunt would extend the field of our work enabling us to effectively
treat the patients with portal hypertension. The application of alcohol in
primary and metastatic tumors has been introduced. By the launching of this
method and doppler ultrasound, follow-up of vascularization and necrosis of
a tumor, respectively, has been made possible, what is our modification of
the mentioned method. Over the past two years, methods of biliary dranaige
and wirsungography under the control of an ultrasound and X-ray have been
developed. The introduction of endoscopic biliary dranaige is just under
way. The use of endoscopic ultrasound and laser would enable the follow-up
of laser efficiency in the treatment of early gastric carcinoma. We would
try to produce our own software for endoscopic findings and elaboration of
educational video films.

Research goals: The main aim of the project is to introduce and
apply new endoscopic and ultrasound technologies in order to enlarge the
area and compentence of our work and to enable research within the field of
interventional gastroenterology. There are many goals. The first goal is to
found a hemodynamic laboratory within wich the changes on the level of
portal vein as well as all the complications of portal hypertension by
endoscopic, ultrasoun and radiology methods will be followed up. We expect
that we will be able to follow-up all the hemodynamic changes in the portal
vein pool and assess the risk of the development of varices and hemorrhage
by the above mentioned methods. This part of the project will be realised
within the PhD thesis. The second goal is to develop and investigate new
ways of endoscopic control of bleeding from gastroduodenal ulcers. Besides
laser and injection sclerotherapy, we have alredy in 1993 introduced a new
method of fibrin glue injection. In this field, we are collaborating with
dr. Friedrichs from Duisburg and the firm Behring, Marbur, Germany. We
belive that it would be possible to assess the efficacy of particular
hemostatic methods for different thypes of ulcer hemorrhage and that the
Forrest's classification of bleeding ulcers could be critically evaluated,
since it does not completely sutisfy all characteristics of bleeding
dynamics. In 1993, we have started to study the relationship between
gastric carcinoma (GC) and infection caused by Helicobacter pylori (HP).
The potential relationship between HP and the development of GC is under
study in the world at present, and we are planing to determine the
frequency of this infection and tumors in our population. The next aim of
the project is to follow-up the efficiency of laser therapy of early
gastric cancer, as a curative methods when surgical procedure is
inpossible. We hope that the treatment of patients with early gastric
carcinomas will be even more successful with the instituion of endoscopic
ultrasound. The futher goal is to modify a method of alcochol injection
into the feeding artery of hepatic tumor. We expect that this modification
will result in a quicker necrosis of a tumor. The results will be presented
in the PhD thesis. During the last year and half, we have started to
perform biliary drainage under the control of ultrasound. The aim of this
segment of interventional gastroenterology is to expand the way of biliary
drainage by help of endoscopy. We are just elaborating our own coputer
system for output and storage of endoscopic and ultrasound findings. We are
planing to investigate the possibility of use of endoscopic-computer-video
system in the work out of films for education in endoscopic and surgical
endoscopy and ultrasound.
